Speech outcome regarding overall intelligibility, articulation, resonance and voice in Flemish children a year after pharyngeal flap surgery. A pilot study.

OBJECTIVE: The main purpose of this study is to determine the treatment effectiveness of pharyngeal flap surgery by measuring speech outcome 1 year after surgery. The authors hypothesized that flap surgery is an effective technique for velopharyngeal inadequacy resulting in improved intelligibility, decreased hypernasality and nasalance scores and normal voice characteristics. PATIENTS AND METHODS: Objective (Nasometer, Dysphonia Severity Index) as well as subjective (perceptual evaluations) assessment techniques were performed in 7 subjects. Speech evaluations were performed 1 year after fla

The effects of mastoid aeration on autophony in patients with patulous eustachian tube.

The effect of the mastoid pneumatization on sound transmission from the nasopharyngeal cavity to the inner ear, which is one of the important factors to indicate the severity of the patulous eustachian tube (ET), are investigated in patients with patulous ET. Twenty ears of 13 subjects with patulous ET were retrospectively analyzed. Sound transmission from the nasopharyngeal cavity to the inner ear was evaluated by observing the masking effects of noise presented in the nasopharyngeal cavity on the auditory threshold (nasal noise masking audiometry). The relationship between this masking effec

Neurochemical markers in the nervous plexus of the canine glottis.

The structure of the nervous network and the distribution of tyrosine hydroxylase (TH)- and various neuropeptide-containing nerves were immunohistochemically studied in the glottis of the dog. The nervous network in the glottis revealed apparent regional differences in morphology. The nervous network in the cartilaginous vocal fold of the posterior glottis consisted of nerve bundles running parallel to the edge of the vocal fold. Only a small number of nerve bundles were observed in the anterior glottis, specifically in membranous vocal fold. In the subepithelial layer of the posterior glottis

Analysis of Voice Quality in Children With Smith-Magenis Syndrome.

UNLABELLED: The production of phonation involves very complex processes, linked to the physical, clinical, and emotional state of the speaker. Thus, in populations with neurological diseases, it is possible to find the imprint in the voice signal left by the deterioration of certain cortical areas or part of the neurocognitive mechanisms that are involved in speech. In previous works, the authors determined the relationship between the pathological characteristics of the voice of the speakers with Smith-Magenis syndrome (SMS) and a lower value in the cepstral peak prominence (CPP) with respect…

Intraoperative Management of the Recurrent Laryngeal Nerve Transected or Invaded by Thyroid Cancer.

Thyroid cancer often invades the recurrent laryngeal nerve (RLN), causing vocal cord paralysis. In such patients, the invaded portion of the … d, the vocal cord on that side is paralyzed. Symptoms include hoarseness, mis-swallowing, and short phonation. RLN reconstruction using one of the reconstruction techniques leads to the recovery of phonatory and swallowing function, although the normal motion of the vocal cord on the side of the anastomosis is not restored. We used direct anastomosis, free …

[Hypodontia: therapeutic strategy elaborated from 30 cases].

INTRODUCTION: Hypodontia is defined as the absence of at least six permanent teeth due to the hypodevelopment of tooth germs. The prevalence is weak and varies according to the ethnic origin. This familial abnormality is due to various mutations or polymorphisms of genes. It may be associated with other dental abnormalities. In children, it is responsible for growth, mastication, and phonation disorders, hypo-alveolia and lingual interposition in the toothless sites. We report the management of 30 patients presenting with oligodontia. MATERIAL AND METHOD: The diagnosis was made on clinical an…

Glottal Features in Vocal Assessment Following Thyroidectomy.

OBJECTIVES: Vocal dysfunction is a frequent complication following thyroidectomy that can be associated with a negative impact on patients' quality of life. Although the effect of thyroidectomy on acoustic features has been widely studied, the examination of glottal flow characteristics to assess vocal outcomes following thyroid gland surgery has not been included in empirical research, to date. The goal of our study was to evaluate early and short-term vocal outcomes following thyroidectomy based on the analysis of glottal acoustic features during speech production. STUDY DESIGN: Prospective

Relationships Between Laryngoscopic Analysis Metrics of Supraglottic Compression and Vocal Effort in Primary Muscle Tension Dysphonia.

PURPOSE: Supraglottic compression is thought to underlie vocal effort in patients with primary muscle tension dysphonia (pMTD). However, the relationship between supraglottic compression and vocal effort in this clinical population remains unclear. Gold standard laryngoscopic assessment metrics for supraglottic compression are also lacking. The goals of this study were to identify metrics proposed in the literature that could distinguish patients diagnosed with pMTD from typical voice users and determine their relationships to the vocal effort. METHODS: Flexible laryngeal endoscopy was perfor

The Utility of Pulmonary Function Testing in Patients Presenting With Dysphonia.

OBJECTIVE: We aimed to evaluate the utility of pulmonary function testing (PFT), particularly forced expiratory flow (FEF) 25-75%, in patients presenting with dysphonia. STUDY DESIGN: A retrospective chart review was carried out. METHODS: Records of 199 patients who presented with dysphonia were reviewed to determine whether in-office PFTs, which we perform routinely, lead to new pulmonary diagnoses or the need for additional pulmonary medications, after assessment by a pulmonologist. Of particular interest was evaluating if FEF25-75% of predicted values less than 80% can be used as a marker

Investigation of Flexible High-Speed Video Nasolaryngoscopy.

OBJECTIVE: High-speed videolaryngoscopy is widely used in voice practices as a complement to videostroboscopy, especially when it is desired to visualize asymmetric and nonperiodic vocal fold vibration or voice onset and offset. Because of the requirement for greater illumination at higher frame rates, the high-speed exam is usually performed with a rigid transoral laryngoscope. Although it is possible to obtain color high-speed video images with a flexible fiberoptic nasoendoscope, the results are often disappointing because of the inability to provide adequate lighting inside the larynx. Kinematic measurements of the vocal-fold displacement waveform in typical children and adult populations: quantification of high-speed endoscopic videos.

PURPOSE: This article presents a quantitative method for assessing instantaneous and average lateral vocal-fold motion from high-speed digital imaging, with a focus on developmental changes in vocal-fold kinematics during childhood. METHOD: Vocal-fold vibrations were analyzed for 28 children (aged 5-11 years) and 28 adults (aged 21-45 years) without voice disorders. The following kinematic features were analyzed from the vocal-fold displacement waveforms: relative velocity-based features (normalized average and peak opening and closing velocities), relative acceleration-based features (normal

Analysis of Measured and Simulated Supraglottal Acoustic Waves.

To date, although much attention has been paid to the estimation and modeling of the voice source (ie, the glottal airflow volume velocity), the measurement and characterization of the supraglottal pressure wave have been much less studied. Some previous results have unveiled that the supraglottal pressure wave has some spectral resonances similar to those of the voice pressure wave. This makes the supraglottal wave partially intelligible. Although the explanation for such effect seems to be clearly related to the reflected pressure wave traveling upstream along the vocal tract, the influence
