Arthroscopic repair of full-thickness tears of the rotator cuff.

We present the results of arthroscopic repair of full-thickness tears of the rotator cuff in seventy-three patients (thirty-nine men and thirty-four women). The average age of the patients at the time of the operation was 60.7 years (range, thirty-one to eighty-two years). All of the patients were followed for at least two years (average, thirty months; range, twenty-four to forty months). The shoulders were evaluated with the rating scale of the University of California at Los Angeles, the shoulder index of the American Shoulder and Elbow Surgeons, and the functional rating scale of Constant

MR arthrogram findings of luxatio erecta in a pediatric patient-arthroscopic confirmation and review of the literature.

Luxatio erecta or inferior glenohumeral dislocation is a rare type of shoulder dislocation, accounting for less than 1 % of all reported shoulder dislocations. We describe a 15-year-old male who presented with luxatio erecta following an injury to his shoulder that resulted from a mountain biking accident. Clinically, the patient had shoulder pain and fixed abduction of the arm. Radiographs confirmed the diagnosis of luxatio erecta. A magnetic resonance arthrogram (MRA) performed 9 days after presentation demonstrated both a greater tuberosity fracture and avulsion of the anterior and posterio

Hemiarthroplasty in the treatment of comminuted intraarticular fractures of the proximal humerus.

Thirty-eight patients with four-part proximal humerus fractures and fracture dislocations were treated with humeral head replacement between 1989 and 1995. At followup (mean, 37 months; range, 12-48 months) the patients were evaluated for postoperative pain, active range of motion, muscular strength, overall function in every day activities, and patient satisfaction. Complications developed in five patients and consisted of humeral component malposition (one shoulder), rotator cuff insufficiency (two shoulders), and heterotopic ossification (two shoulders). According to the Neer criteria, the

Superior capsular reconstruction for irreparable rotator cuff tears: A literature review and specialist practice report.

BACKGROUND: Recent evidence continues to confirm the effectiveness of arthroscopic superior capsular reconstruction (SCR) in relieving pain and improving function in an irreparable cuff deficient shoulder. This paper presents an up-to-date literature review on SCRand a tertiary referral unit experience in the United Kingdom. METHODS: Data was prospectively collected on patients undergoing SCR for irreparable rotator cuff tears using a dermal allograft. Patients with already established arthritis were excluded. The first 49 consecutive cases performed by 3 surgeons over 3 years (including thei

Central microglia-adenosine-related signaling impairs bone-tendon interface repair via sympathetic ADRB2 activation.

Bone-tendon interface (BTI) injuries pose a major clinical challenge because surgical repair often fails to restore the native enthesis and its structural and mechanical integrity. Although local repair mechanisms have been extensively studied, whether central neuroimmune circuits contribute to BTI healing remains unclear. Here, using a murine rotator cuff injury model, we provide evidence that BTI injury engages a sensory-central-sympathetic regulatory axis that contributes to impaired repair. BTI injury activated sensory afferent signaling and was associated with microglia-mediated neuroinfl

Intra-articular Loose Body with Concomitant Bankart Lesion after a Traumatic Shoulder Dislocation: A Case Report.

INTRODUCTION: The spectrum of pathoanatomic lesions encountered in anterior shoulder dislocation is broad. There could be a presence of loose bodies, chondral and osteochondral, in the shoulder joint and also concomitant rotator cuff partial tears resulting from acute and chronic shoulder instability. CASE REPORT: We present one case report of a 46-year-old male Chinese with an uncommon case of Bankart lesion, with a full thickness chondral defect over the superior glenoid articular surface manifesting as a large intra-articular loose cartilaginous body. The patient presented with persistent

Overuse activity in the presence of scapular dyskinesis leads to shoulder tendon damage in a rat model.

Shoulder tendon injuries are common clinical conditions and are a significant source of pain and dysfunction. These conditions are more common in individuals who perform repetitive overhead activities and in individuals who have abnormal scapular kinematics, termed scapular dyskinesis (SD). However, the long term consequences associated with overuse activity in the presence of SD are unknown. Therefore, the objective of this study was to determine the effect of overuse in combination with SD on joint mechanics and properties of the rotator cuff and biceps tendons. A rat model of scapular dyski

Recurrent posterior shoulder instability after rifle shooting.

Rifle shooting produces a sudden counterforce against the body thorough the anterior shoulder, which may produce a traumatic injury in soldiers. Posterior instability of the shoulder can occur in soldiers who practice rifle shooting. To the authors' knowledge, few reports have examined shooting-related injuries in soldiers. This article describes the case of a 27-year-old male soldier who presented with left shoulder pain and instability after rifle training. He developed symptoms, and presented radiographic findings consistent with a posterior Bankart lesion. Intraoperatively, while in the la

Posterior instability of the shoulder with secondary impingement in elite golfers.

BACKGROUND: Shoulder injuries in golf are related to the biomechanics of the golf swing and typically occur in the lead arm at the top of the back swing. PURPOSE: We report a newly recognized entity in a series of elite golfers: posterior glenohumeral instability associated with subacromial impingement. STUDY DESIGN: Retrospective review. METHODS: Eight elite golfers were treated between March 1991 and July 1998 for pain occurring in the nondominant, lead shoulder at the top of the back swing. Posterior instability was diagnosed in all eight patients; six of the eight also demonstrated sign

[Arthroscopic repair of partial subcapularis tendon lesions. Simplified approach].

We present a technical variation for arthroscopic subscapularis tendon repair. Sutures of isolated partial subscapularis tendon avulsions can be passed and fixed securely through an isolated anterolateral portal just over the rotator interval (mono-tunnel technique). Primarily a shoulder free of fixation devices is needed. Patient on a beach chair position or lateral decubitus without traction is advised. The best view can be obtained from a posterior portal with the shoulder on 75 degrees forward elevation and 90 degrees medial rotation. In a shoulder 30 degrees adducted at a good view can be

Effect of ultrasound-guided interscalene brachial plexus block using liposome bupivacaine combined with continuous esketamine infusion on quality of recovery in patients undergoing arthroscopic rotator cuff repair: study protocol for a randomized controlled trial.

BACKGROUND: Arthroscopic rotator cuff repair is the standard of care for rotator cuff tears. However, postoperative pain and recovery remain a significant clinical challenge. While ultrasound-guided interscalene brachial plexus block with liposomal bupivacaine provides sustained analgesia for up to 72 h, the addition of esketamine intravenous infusion may offer further benefits through enhanced pain control, mood stabilization, and cognitive support. This study aims to evaluate the synergistic effect of this combination on the quality of postoperative recovery in patients undergoing arthroscop

Non-inferiority and cost-effectiveness trial of isolated biceps tenotomy versus tenotomy with rotator cuff repair in patients with stage 2-3 Goutallier fatty degenerative cuff lesions (TenCuRe study): protocol of a multicentre randomised controlled trial.

INTRODUCTION: For patients who are diagnosed with lesions of the rotator cuff that present advanced levels of fatty degeneration, arthroscopic repair of the rotator cuff remains controversial. This controversy can be attributed to the frequently reported high failure rate of the tendon fixation and the fact that it remains unclear why repair for these tears results in significant clinical improvement independent of the occurrence of such a re-tear. Recent publications have reported comparable clinical improvements when merely a tenotomy of the long head of the biceps tendon was performed and t

Does acromioplasty result in favorable clinical and radiologic outcomes in the management of chronic subacromial pain syndrome? A double-blinded randomized clinical trial with 9 to 14 years' follow-up.

BACKGROUND: The treatment effect of acromioplasty for chronic subacromial pain syndrome (SAPS) on long-term shoulder function and rotator cuff deterioration has still to be determined. This study aimed to determine the long-term clinical and radiologic treatment effect of arthroscopic acromioplasty in patients with chronic SAPS. METHODS: In this double-blind, randomized clinical trial, 56 patients with chronic SAPS (median age, 47 years; age range, 31-60 years) were randomly allocated to arthroscopic bursectomy alone or to bursectomy combined with acromioplasty and were followed up for a medi

Treatment of Massive Irreparable Rotator Cuff Tears: A Cost-effectiveness Analysis.

Massive irreparable rotator cuff tears cause significant shoulder pain and dysfunction. Physical therapy (PT), arthroscopic debridement with biceps tenotomy (AD-BT), and hemiarthroplasty (HA) are treatments shown to reduce pain and improve quality of life. Reverse total shoulder arthroplasty (RTSA) is a newer surgical treatment option that may offer improved function. A cost-effectiveness analysis of these interventions has never been performed, and no head-to-head comparative effectiveness trials currently exist. A Markov decision analytic model was used to compare RTSA, HA, AD-BT, and PT as

The benefit of synthetic versus biological patch augmentation in the repair of posterosuperior massive rotator cuff tears: a 3-year follow-up study.

BACKGROUND: Rotator cuff repair typically results in a satisfactory, although variable, clinical outcome. However, anatomic failure of the repaired tendon often occurs. HYPOTHESIS: Patch augmentation can improve the results of open rotator cuff repair by supporting the healing process, protecting the suture, and reducing friction in the subacromial space. STUDY DESIGN: Cohort study; Level of evidence, 3. METHODS: A total of 152 patients with a posterosuperior massive rotator cuff tear were treated by open repair only (control group; n = 51; mean age, 67.06 ± 4.42 years), open repair togethe
