SETTING: University hospital in Lusaka, Zambia.
OBJECTIVE: To determine the effects of childhood bacille Calmette-Guerin (BCG) immunization and human immunodeficiency virus (HIV) infection on dual skin test reactions to purified protein derivative (PPD) and Mycobacterium avium sensitin (MAS) in a developing country.
DESIGN: Descriptive cross-sectional study.
RESULTS: Dual skin testing was performed on 112 adults, 58 HIV-positive and 54 HIV-negative. Forty-seven (42%) of 112 had PPD reactions > or =10 mm and 52 (46%) had MAS reactions > or =10 mm. PPD reactions > or =10 mm were present in 30

The evidence regarding the transmission of tuberculosis and risk of infection and disease in several specific clinical situations has been reviewed. There is considerable epidemiologic evidence that contagiousness is not an all-or-nothing phenomenon and is affected by several factors, only one of which is the bacteriologic status of the patient's sputum. Nine M. bovis-infected cattle on a diet deficient in both protein and energy for 133 days lost approximately 17% of their original body weight. Sarcoidosis is a diagnosis often made by exclusion. We present a case of what we believe is sarcoidosis presenting as cervical lymphadenopathy. Complicating issues in this patient included B lymphocyte abnormalities, and a positive PPD reaction. The patient continues to be steroid dependent. This case illustrates the difficulties in diagnosis and management of sarcoidosis.

In patients with pulmonary tuberculosis serum immunoglobulin levels are found to be increased, quantitative deviations from the norm are observed on analysis of the mononuclear cells, there is an increase in the number of circulating immune complexes, and, in the event of extensive pulmonary involvement, an attenuation of cell-mediated cutaneous reactions is seen. In accordance with the chronic course of the disease, a return to normalcy of these parameters in the healing phase occurs only gradually.

In 1964 Sweet described a new syndrome which was characterized by pyrexia, neutrophilia, and the abrupt appearance of erythematous, painful, cutaneous plaques, primarily on the upper extremities, head, and neck. Case records of 37 patients with tuberculosis were analysed, out of 4 000 patients admitted to our department of internal medicine over a period of four years. The most common localisations are still pulmonary, but these are often atypical. Lymph node involvement is found in immigrants. More exceptional localisations (liver . . .) are reported. Tuberculosis has not yet become obsolete.

Cell-mediated immune responses were measured in 91 patients with dermatophytosis by means of delayed-type skin hypersensitivity to a purified trichophytin preparation (ethylene glycol method) and to tuberculin (purified protein derivative, PPD). The findings indicate that dermatophytes differ in their sensitizing capacity as measured by trichophytin skin sensitivity. Lymphocyte transformation in response to a crude freeze-dried prepartion of whole jejunal mucosa was studied in fifteen patients with coeliac disease and compared with the findings in fifteen age-matched controls. The transformation ratio was markedly increased in three patients. These observations suggest the possible existence of cell-mediated autoimmunity to jejunal mucosa in some patients with coeliac disease and might be relevant to the development of some of the complications of the disease.

BACKGROUND: High immigration rates from tuberculosis (TB) endemic countries to low-incidence countries have caused new TB guidelines in these countries to reconsider latent TB infection (LTBI) screening in these immigrants.
