Fetal therapy with mTOR inhibitors in cardiac rhabdomyoma and lymphatic malformations.

Fetal therapy has advanced with novel pharmacological approaches to address congenital tumors and malformations, including cardiac rhabdomyomas and lymphatic malformations (LMs). Cardiac rhabdomyomas, often associated with tuberous sclerosis complex (TSC), can lead to significant morbidity due to arrhythmias, left or right ventricular outflow tract obstruction and heart failure, particularly in neonates. LMs, resulting from abnormal development of the lymphatic system, can cause considerable morbidity, including airway obstruction, disfigurement, recurrent infections, bleeding into cysts, and

Short- and mid-term outcomes of total correction of Taussig-Bing anomaly.

Double-outlet right ventricle (DORV)/Taussig-Bing (TB) anomaly is the second most common type of DORV. This study evaluates our experience and outcomes of total correction of DORV-TB anomaly at King Abdulaziz Cardiac Center. We conducted a retrospective study for all cases of TB anomaly repaired between June 2001 and April 2009. Patients were divided into two groups: Group A included patients repaired with arterial switch operation, and group (B) included patients repaired with Rastelli procedure. Thirteen patients with TB anomaly underwent total correction. There were 5 male (38%) and 8 femal

Outcome of surgical myectomy after unsuccessful alcohol septal ablation for the treatment of patients with hypertrophic obstructive cardiomyopathy.

OBJECTIVES: We sought to determine the outcome of myectomy after unsuccessful alcohol ablation. BACKGROUND: Alcohol septal ablation results in symptomatic improvement and a reduction in dynamic obstruction in most hypertrophic obstructive cardiomyopathy patients. However, a few patients remain with severe symptoms and obstruction and need surgery. The outcome of these cases is not well known. METHODS: The medical records of 375 patients who underwent alcohol ablation at our institution were reviewed. Twenty patients (5.3%, mean age 53 +/- 18 years, 17 women) subsequently needed surgical myec

Long-term results of surgical repair in patients with congenital subaortic stenosis.

OBJECTIVES: The aim of the study was to analyze the long-term results of congenital subvalvar aortic stenosis (SAS) relief and the risk factors associated with recurrence and reoperations. METHODS: Between January 1960 and March 2005, 190 patients underwent surgical correction for discrete (n=140) and tunnel (n=50) congenital subaortic stenosis. There were 115 male and 75 female patients ranging in age from 1 week to 36 years (mean age, 8.2+/-4.4 years). Preoperatively, 133 patients were in NYHA functional class I or II, 57 in class III or IV. There were several initial surgical procedures pe

Late results after resection of discrete and tunnel subaortic stenosis.

From May 1969 to June 1988, 84 consecutive patients ranging in age from 6 months to 61 years (mean 18 years) underwent surgery for fixed subaortic stenosis (SAS). A discrete fibrous or fibromuscular structure was present in 81 patients, while 3 presented with a tunnel type of obstruction. SAS was treated by sharp resection of the tissue and routine myotomy or myectomy of the hypertrophied left ventricular (LV) muscle (57 patients, group 1), while more recently, the lesion was treated by simple fibrous tissue enucleation (27 cases, group 2). There were 3 hospital deaths (3.6%) and 3 late deaths

Acute Obstructive Hydrocephalus Due to Posterior Reversible Encephalopathy Syndrome Successfully Treated With Continuous Ventricular Drainage: A Case Report.

Posterior reversible encephalopathy syndrome (PRES) is an acute-onset neurological disorder characterized by various symptoms, including headache, visual disturbances, and seizures. The causes of PRES include hypertension, eclampsia, sepsis, renal failure, autoimmune diseases, and the use of chemotherapeutic agents or immunosuppressants. Although the exact pathophysiology remains unclear, it is thought to involve vasogenic edema and endothelial dysfunction. PRES generally has a favorable prognosis, and with appropriate early intervention, neurological function typically recovers without residu

Percutaneous transluminal septal myocardial ablation for hypertrophic obstructive cardiomyopathy in patients with situs inversus totalis: A two-case series.

UNLABELLED: Percutaneous transluminal septal myocardial ablation (PTSMA) is an established treatment for hypertrophic obstructive cardiomyopathy (HOCM), but its application in patients with situs inversus totalis (SIT) is complicated by mirror-image anatomy. We describe two successful PTSMA procedures in women, aged 82 and 69 years, with HOCM and coexisting SIT. The second patient had additional complexities, including Kartagener syndrome and chronic steroid therapy for rheumatoid arthritis. Both patients presented with severe, drug-refractory symptoms and significant left ventricular outflow

Hypertrophic cardiomyopathies requiring more monitoring for less atrial fibrillation-related complications: a clustering analysis based on the French registry on hypertrophic cardiomyopathy (REMY).

AIMS: Defining the risk of atrial fibrillation (AF) in hypertrophic cardiomyopathy (HCM) patients is an important clinical and prognostic challenge. The aim of this study is to determine HCM phenogroups with different risk of AF occurrence at 5 years. METHODS AND RESULTS: We applied retrospectively the Bayesian method, which can analyze a large number of variables, to differentiate phenogroups of patients with different risks of AF and prognoses across a French prospective on-going hospital-based registry of adult HCM patients (REMY). Clinical and imaging data were prospectively recorded, and

A haemodynamic conundrum: a case report of a patient with concurrent pulmonary arterial hypertension and hypertrophic obstructive cardiomyopathy.

BACKGROUND: The co-existence of hypertrophic obstructive cardiomyopathy (HOCM) and pulmonary arterial hypertension (PAH) is extremely rare and poses a management conundrum. This is the first case report in the published literature to describe the diagnosis and management of a patient with both conditions. CASE SUMMARY: A 49-year-old female with a history of HOCM and recently diagnosed scleroderma presented to the clinic with progressive dyspnoea. Transthoracic echocardiogram demonstrated left ventricular outflow tract (LVOT) obstruction at rest, and elevated pulmonary artery (PA) pressure. Ca

Current management and surgical advances in patients with hypertrophic obstructive cardiomyopathy.

Hypertrophic cardiomyopathy (HCM) is a genetic disease and the most frequent primary cardiomyopathy, affecting 1:500 of the general population. Integrated multimodality imaging, including transthoracic echocardiography, 2- and 3‑dimensional transesophageal echocardiography, stress echocardiography, and cardiac magnetic resonance, has provided answers to questions on the management of HCM, leading to standardized protocols. The late 1990s brought the news of a nonsurgical treatment of obstruction in HCM. It is now increasingly evident that septal ablation cannot address all the mechanisms of th

Transcatheter edge-to-edge repair for severe mitral regurgitation in an octogenarian woman with hypertrophic obstructive cardiomyopathy: a case report.

BACKGROUND: Hypertrophic obstructive cardiomyopathy (HOCM) is characterized by asymmetric septal hypertrophy, diastolic dysfunction, and dynamic left ventricular outflow tract (LVOT) obstruction, and is often associated with systolic anterior motion (SAM) of the mitral valve and secondary mitral regurgitation (MR). Established treatments of HOCM include beta-blockers, calcium channel blockers, surgical myectomy, and alcohol septal ablation (ASA); however, elderly patients with a high surgical risk or an unsuitable coronary anatomy have limited therapeutic options. Transcatheter edge-to-edge re

Transposition of the great arteries complicated by tricuspid valve incompetence.

BACKGROUND: Tricuspid valve insufficiency secondary to structural anomalies of the valve itself or to an iatrogenic complication of the Rashkind procedure is very rarely associated with transposition of the the great arteries. This condition represents an interesting perioperative challenge. Rapid restoration of the tricuspid valve to a low-pressure system by arterial switch operation associated with tricuspid repair should theoretically improve the outcome in terms of myocardial and valve function. METHODS: Thirteen of 839 patients who underwent an arterial switch operation for various forms

Moving Past Tonsil Position: Craniocervical Junction Crowding Shapes Cerebrospinal Fluid Effective Motility in Chiari I Malformation.

Chiari I malformation (CM-I) is conventionally defined by cerebellar tonsil position, yet tonsil position is an indirect surrogate for the anatomic obstruction that impairs cerebrospinal fluid (CSF) flow across the craniocervical junction (CCJ). We hypothesized that CCJ crowding, quantified as subarachnoid space narrowing at the foramen magnum and C1, would explain CSF flow impairment more directly than tonsil position. Using non-invasive low b-value diffusion-weighted MRI (low-b dMRI), we quantified effective CSF motility, indexed by mean pseudo-diffusivity (MΨ), across the upper cervical spi

Obstructive hypertrophic cardiomyopathy: from genetic insights to a multimodal therapeutic approach with mavacamten, aficamten, and beyond.

BACKGROUND: A cardiac condition marked by excessive growth of heart muscle cells, hypertrophic cardiomyopathy (HCM) is a complex genetic disorder characterized by left ventricular hypertrophy, microvascular ischemia, myocardial fibrosis, and diastolic dysfunction. Obstructive hypertrophic cardiomyopathy (oHCM), a subset of HCM, involves significant obstruction in the left ventricular outflow tract (LVOT), leading to symptoms like dyspnea, fatigue, and potentially life-threatening cardiac events. With advancements in genetic understanding and the introduction of novel pharmacologic agents, incl

Hypertrophic cardiomyopathy: state of the art on diagnosis and current therapeutic approach.

BACKGROUND: Hypertrophic cardiomyopathy is the most common genetic cardiomyopathy and is characterized by unexplained myocardial hypertrophy. This condition exhibits marked clinical and prognostic heterogeneity. Advances in cardiovascular imaging techniques and genetic testing have significantly improved the diagnostic accuracy, phenotypic characterization, and prognostic assessment. Hypertrophic cardiomyopathy can present with a broad spectrum of genetic substrates and clinical presentations, ranging from asymptomatic individuals to those with heart failure, arrhythmias, and even sudden cardi

Evaluating the AltaValve as a novel method for transcatheter mitral valve replacement.

Mitral regurgitation (MR) is the most common heart valve disease, and severe MR is associated with a poor prognosis if left untreated. Although surgical repair or replacement constitutes the standard therapy when indicated, many high-risk patients are considered ineligible for surgery. Transcatheter mitral valve replacement (TMVR) offers a less invasive alternative to conventional surgery and may also overcome some of the limitations of percutaneous repair techniques. Currently, multiple TMVR devices are undergoing clinical evaluation, showing promising results. However, challenges mainly rela

Tetralogy of Fallot: The Burden of Pulmonary Atresia in the NICU Set-Up: Two Case Reports and a Literature Review.

Tetralogy of Fallot (ToF) with pulmonary atresia (PA) and complete right ventricular outflow tract obstruction (RVOTO) represents one of the most critical forms of congenital heart disease in neonates. These cases require complex and timely interventions to ensure survival and optimize long-term outcomes. While surgical correction offers a favorable prognosis, the period from birth to surgery is often marked by significant hemodynamic, respiratory and nutritional challenges, particularly in neonatal intensive care units (NICUs). This study aims to outline a structured, physiology-guided approa

Pathophysiology and management of adults with complex congenital heart disease after biventricular repair.

Surgical management of congenital heart disease encompasses a spectrum of procedures, ranging from biventricular repair to univentricular palliation, each tailored to the specific anatomical and hemodynamic features of individual cases. Among these, biventricular repair, which preserves a functional ventricle to sustain pulmonary circulation, is prioritized whenever feasible. Advances in approaches have significantly improved outcomes, enabling many patients with congenital heart disease to reach adulthood, including the majority who have undergone biventricular repair. Despite these advanceme

Cardiac amyloidosis in severe aortic stenosis patients: an updated review unraveling their complex interplay.

Aortic stenosis (AS) and cardiac amyloidosis (CA) often coexist in elderly patients, complicating diagnosis and treatment. AS is characterized by the narrowing of the aortic valve, leading to left ventricular outflow obstruction, primarily due to age-related calcification and congenital defects. CA, the most common cause of restrictive cardiomyopathy, results from amyloid protein deposits in the myocardium, causing diastolic dysfunction. Accurate diagnosis of CA in AS patients is challenging due to overlapping symptoms and the need for advanced imaging and biomarkers. Aortic valve replacement …
