BACKGROUND: Although the 1-dose varicella vaccination program, introduced in 1996, has led to significant declines in varicella disease, outbreaks continued to occur, which led to the adoption of a 2-dose vaccination program in 2007. We previously reported an 88% decline in varicella-related hospitalizations and a 59% decline in outpatient visits during 1994-2002. We now update data on varicella healthcare utilization with 10 years of additional data, during a period of stabilizing first-dose coverage and rapidly increasing second-dose coverage.

BACKGROUND: In randomized clinical studies, over 11,800 children, 12 months to 6 years of age, were administered ProQuad(®), a combination measles, mumps, rubella, and varicella vaccine (MMRV). This paper describes the safety following a 2-dose regimen of MMRV administered to children in the second year of life.

We report a case of fetal varicella infection following the diagnosis of maternal varicella infection at 13 weeks of pregnancy. Anomalies noted sonographically at 26 weeks' gestation included oligohydramnios, symmetrically impaired fetal growth, limb anomalies, a thin placenta, and widespread dystrophic calcification of the abdominal cavity and chest, including the lungs and myocardium. Some of these sonographic anomalies have been reported previously. However, to our knowledge, prenatal diagnosis of calcification of the lungs and myocardium has not been described.

A boy, 4 years, 9 months of age, presented with acute hemiplegia, lethargy, ataxia, and dysarthria 24 hours prior to the eruption of typical varicella exanthem. Magnetic resonance imaging findings were typical of multiple cerebral ischemic infarcts. It is suggested that during the period of secondary viremia varicella zoster virus invaded the cerebral blood vessels causing vasculopathy and cerebrovascular infarcts.

We report a case of a 76-year-old patient who developed an atypical form of varicella zoster virus (VZV) reinfection. In addition to histological and serological confirmation of diagnosis, direct demonstration of VZV-DNA was possible by means of the nested polymerase chain reaction. Although VZV infection is usually diagnosed by clinical examination, the nested PCR is a sensitive and specific diagnostic procedure that can be useful especially in atypical cases.
