Effects of escitalopram treatment and chronic mild stress induced from peripuberty on spermatic parameters of adult rats.

BACKGROUND: The prevalence of depression in adolescents has significantly increased worldwide. Escitalopram is a selective serotonin reuptake inhibitor approved for treatment of psychiatric disorders in children and adolescents by the Food and Drugs Administration. AIMS: This study aimed to evaluate the sperm parameters of adult rats exposed to chronic mild stress (CMS), from peripuberty to adulthood, treated or not with escitalopram. MATERIALS AND METHODS: Sixty-two male rats were distributed into four groups: S - submitted to CMS; E - Escitalopram (10 mg / kg, via gavage); ES - CMS + ES; S

Medications with Potential for Fetal Risk Prescribed to Veterans.

Background: Women service members of the past 20 years experienced high rates of traumatizing events resulting in pharmacological treatment. Post-military lives may include having children. Typically, Veterans Health Administration (VHA) patients' pregnancies are managed outside the VHA. This study examined medication exposures during pregnancy. Materials and Methods: The Center for Maternal and Infant Outcomes Research in Translation (COMFORT) study collected primary survey data and linked secondary health care data from the VHA from 2015 to 2021. Medication fills and covariates were extracte

Behavioral coping phenotypes and associated psychosocial outcomes of pregnant and postpartum women during the COVID-19 pandemic.

The impact of COVID-19-related stress on perinatal women is of heightened public health concern given the established intergenerational impact of maternal stress-exposure on infants and fetuses. There is urgent need to characterize the coping styles associated with adverse psychosocial outcomes in perinatal women during the COVID-19 pandemic to help mitigate the potential for lasting sequelae on both mothers and infants. This study uses a data-driven approach to identify the patterns of behavioral coping strategies that associate with maternal psychosocial distress during the COVID-19 pandemic

A systematic review of the effects of intimate partner violence on HIV-positive pregnant women in sub-Saharan Africa.

BACKGROUND: Intimate partner violence (IPV) affects more than one in three women in sub-Saharan Africa (SSA). It is associated with both pregnancy and HIV, adversely affecting women in this region. This is the first systematic examination of the effects of IPV on HIV-positive (HIV+) pregnant women in SSA. METHODS: A systematic review of the literature on HIV+ pregnant women experiencing IPV in SSA was carried out. Searches were carried out in PubMed, Web of Science and African Journals Online databases. Articles published between January 2010 and June 2020, in English, were included. Data ext

A model of perinatal stress and childhood wheezing: ELSPAC-CZ cohort.

BACKGROUND: Prenatal origins of wheezing are not fully understood. This study develops a model of mechanisms linking perinatal stress exposure to wheeze phenotypes in children. METHODS: Data were obtained from 1880 mother-child dyads participating in ELSPAC-CZ birth cohort. Wheeze phenotypes assessed between birth and age 7 years included "never wheeze," "early-onset transient (EOT) wheeze," "early-onset persistent (EOP) wheeze," and "late-onset (LO) wheeze." Prenatal and postnatal stress exposures were assessed in mid-pregnancy and 6 months after delivery, respectively, using an inventory of

Predicting Severity of Acute Pain After Cesarean Delivery: A Narrative Review.

Cesarean delivery is one of the most common surgical procedures in the United States, with over 1.3 million performed annually. One-fifth of women who undergo cesarean delivery will experience severe pain in the acute postoperative period, increasing their risk of developing chronic pain and postpartum depression, and negatively impacting breastfeeding and newborn care. A growing body of research has investigated tools to predict which patients will experience more severe pain and have increased analgesic consumption after cesarean delivery. These include quantitative sensory testing, assessme

Schedules for home visits in the early postpartum period.

BACKGROUND: Maternal complications including psychological and mental health problems and neonatal morbidity have been commonly observed in the postpartum period. Home visits by health professionals or lay supporters in the weeks following the birth may prevent health problems from becoming chronic with long-term effects on women, their babies, and their families. OBJECTIVES: To assess outcomes for women and babies of different home-visiting schedules during the early postpartum period. The review focuses on the frequency of home visits, the duration (when visits ended) and intensity, and on

New Antidepressant Development in the Treatment of Depression.

Major depressive disorder (MDD) continues to pose a major therapeutic challenge due to its clinical heterogeneity. This chapter looks at the development of antidepressant treatments, starting with early interventions such as electroconvulsive therapy (ECT), monoamine oxidase inhibitors (MAOIs) and tricyclic antidepressants (TCAs). Although these treatments targeted the monoaminergic system, they had significant limitations in terms of safety and efficacy. The introduction of selective serotonin reuptake inhibitors (SSRIs) and serotonin-norepinephrine reuptake inhibitors (SNRIs) improved tolera

Maternal self concept as a provider and cessation of substance use during pregnancy.

OBJECTIVE: Maternal substance use during pregnancy is a common modifiable risk factor for poor birth outcomes, and is associated with long term psychological risks to offspring. As self concept is known to affect substance use behaviors in non-pregnant women, we hypothesized that self concept as a provider may be particularly salient to cessation of use during pregnancy. To isolate psychological processes specific to pregnancy from those associated with the transition to parenthood, we examined birth mothers who made adoption placements participating in the Early Growth and Development Study.

Associations between maternal prenatal depression and neonatal behavior and brain function - Evidence from the functional near-infrared spectroscopy.

BACKGROUND: Maternal prenatal depression is a significant public health issue associated with mental disorders of offspring. This study aimed to determine if maternal prenatal depressive symptoms are associated with changes in neonatal behaviors and brain function at the resting state. METHODS: A total of 204 pregnant women were recruited during the third trimester and were evaluated by Edinburgh Postpartum Depression Scale (EPDS). The mother-infant pairs were divided into the depressed group (n = 75) and control group (n = 129) based on the EPDS, using a cut-off value of 10. Cortisol levels

Gender preference and perinatal depression in Turkey: A cohort study.

BACKGROUND: Child gender preference is important in some cultures and has been found to modify risk for antenatal and postnatal depression. We investigated discrepancies in the child gender preference between participating women and other key family members and the extent to which these predicted perinatal depression. METHODS: In a large cohort study of perinatal depression in urban and rural Turkey, participants had been asked about child gender preferences: their own, and those of their husband, parents, and parents in-law. Of 730 participants recruited in their third trimester (94.6% parti

Does antenatal screening for psychosocial risk factors predict postnatal depression? A follow-up study of 154 women in Adelaide, South Australia.

OBJECTIVE: This study investigated the effectiveness of antenatal screening for psychosocial risk factors in predicting postnatal depression, in women living in a socioeconomically deprived area. METHOD: 154 women completed the Antenatal Psychosocial Questionnaire (APQ) and the Edinburgh Postnatal Depression Scale (EPDS). Logistic regression analysis was used to identify antenatal psychosocial risk factors predictive of EPDS caseness, with an EPDS score of 10 or more being taken to indicate depression. RESULTS: Antenatal depression was more common than postnatal depression. 44 women (30%) me

The relationship between duration of labour, time of delivery, and puerperal psychosis.

BACKGROUND: Puerperal psychosis is the most serious psychiatric disorder after childbirth. Despite the ongoing debate regarding its diagnostic status, there is increasing evidence that it is related to bipolar or schizoaffective disorder. Although a well-recognized precipitant of mania, the role of sleep loss has not been systematically studied in the onset of puerperal psychosis. The aim of this study was to test the hypothesis that sleep disruption resulting from longer labour or nighttime delivery would be associated with the onset of puerperal psychosis. METHOD: Data on duration of labour

Intimate partner abuse before and during pregnancy as risk factors for postpartum mental health problems.

BACKGROUND: Although research has established the profound effects that intimate partner abuse can have on postpartum mental health, little is known regarding how this association may change as a function of the timing and type of abuse. This study examined associations of psychological, physical and sexual abuse experienced as adults before and during pregnancy with symptoms of postpartum mental health problems in a non-clinical sample of women. METHODS: English-speaking mothers aged 18 years and older in the metropolitan area of a large, Western Canadian city were recruited to participate i

[Fathers and peripartum mental illness: the neglected parent?].

Fathers also play an important role during pregnancy and the postpartum period, both for the partner and for the child. With changes in society and increasing early involvement in the care of infants, the father-child relationship has become increasingly more important in recent years. There is growing evidence that fathers can also suffer from mental illnesses during their partner's pregnancy and especially after the birth of a child. As the transition to the role of a father is a major change in a man's life, the birth of a child can be a life event that contributes to a first time mental il

Reproduction in water buffalo: comparative aspects and implications for management.

The domestic buffalo occupies an important niche in many ecologically disadvantaged agricultural systems, providing milk, meat and draught power. Although buffalo can adapt to harsh environments and live on low quality forage, their reproductive efficiency is often compromised by such conditions. Climatic stress depresses ovarian cyclicity, oestrous expression and conception rates. Poor nutrition, usually related to seasonal fluctuations in availability and quality of feed, delays puberty and increases the duration of postpartum anoestrus. Management factors such as the system of grazing (free

Transdiagnostic internet cognitive behavioural therapy for anxiety and depressive symptoms in postnatal women: protocol of a randomized controlled trial.

BACKGROUND: Nearly 20% of women will be confronted with anxiety or depressive disorders during the perinatal period and this may lead to adverse outcomes for both mother and child. Cognitive behavioural therapy (CBT) is the psychological intervention with the most empirical support for the clinical management of anxiety and depressive disorders. Anxiety and depression frequently occur in women during the perinatal period, and there is growing evidence that internet-delivered CBT (iCBT) could be an acceptable and effective intervention. THIS WAY UP, an Australian digital mental health service,

Case-finding with the anxiety sub-scale of the Edinburgh Postnatal Depression Scale in an observational cohort: Sensitivity, specificity, and cost-effectiveness.

BACKGROUND: The Edinburgh Postnatal Depression Scale (EPDS) is effective and cost-effective for identifying postnatal depression. Postnatal anxiety is also common and can be identified by three questions on the EPDS (anxiety sub-scale). We aimed to compare EPDS score alone with EPDS score and sub-scale score together (EPDS+) to identify common mental illness (depression or anxiety) in postnatal women. METHODS: The sensitivity and specificity of the EPDS and sub-scale were explored. We developed a decision tree to compare costs and health outcomes associated with case-finding for postnatal dep

Emotional distress in mothers of early-preterm infants, late-preterm infants, and full-term infants in Malawi.

BACKGROUND: Mothers of preterm infants, early or late, report more distress than mothers of full-term infants. Malawi has the highest preterm birth rate in the world, but nothing is known about the relation of preterm birth to maternal mental health. PURPOSE: To compare emotional distress among mothers of early-preterm, late-preterm, and full-term infants. METHODS: We recruited 28 mothers of early-preterm, 29 mothers of late-preterm, and 28 mothers of full-term infants. Emotional distress was assessed 24-72 hr following birth.
