BACKGROUND: The importance of the radial head to elbow function and stability is increasingly apparent. Although preservation of the native radial head is preferred, severely comminuted fractures may necessitate resection or arthroplasty. Silastic radial head arthroplasty has been condemned on the basis of several sporadic reports of silicone synovitis. However, problems of "overstuffing," cartilage wear, and motion loss are becoming apparent with metal prostheses, indicating this also is not an ideal solution. Thus, the choices remain controversial.

Although the majority of head injuries in children and adults involve dynamic loading conditions, some patients suffer static loading. Static loading occurs when forces are applied slowly to the head, and it produces a much different pattern of injuries. Crush injuries are usually described in the context of industrial accidents, but in our experience, these injuries are not rare in children. We report a series of seven crush injuries in young children admitted during a period of 29 months and describe our experience in the evaluation and treatment of this complex entity. Patient ages ranged f

OBJECTIVE: The aim of surgical stabilization of proximal humerus fractures is to restore the anatomical relations between the greater and lesser tubercle, to reconstruct the joint and preserve the vascular supply of the fragments.
INDICATIONS: Approximately 80% of proximal humeral fractures can be treated conservatively. Surgical treatment is indicated based on the fracture pattern, patient-related factors and the risk of avascular head necrosis. Two-part fractures with a metaphyseal comminution zone and 3/4-part fractures can benefit from near to anatomic reconstruction depending on the pati

PURPOSE: The use of technology to aid in assessment, planning, and management of complex craniomaxillofacial injuries is increasingly common. Preoperative computed tomography (CT) evaluation is considered the standard of care, and intraoperative imaging is becoming increasingly accessible. Limited data exist regarding the implication of intraoperative CT on decision making in the management of all sites of facial fractures. The purposes of this study were to characterize the use of an intraoperative CT scanner for craniomaxillofacial surgery within our institution, to quantify the effect of in

BACKGROUND: Rigid locked plating constructs can suppress fracture healing by inhibiting interfragmentary motion required to stimulate natural bone healing by callus formation. Dynamic fixation with active locking plates reduces construct stiffness, enables controlled interfragmentary motion, and has been shown to induce faster and stronger bone healing in vivo compared to rigid locking plates. This prospective observational study represents the first clinical use of active locking plates. It documents our early clinical experience with active plates for stabilization of humeral shaft fractures

BACKGROUND: Mandibular angle fractures carry disproportionate postoperative morbidity, yet optimal incision selection remains controversial despite its clinical importance.
OBJECTIVE: The aim of the study was to evaluate whether incisional approach, particularly trocar-assisted transbuccal access, is independently associated with any postoperative complication, including postoperative paresthesia, after ORIF of mandibular angle fractures.
METHODS: A retrospective cohort of consecutive patients undergoing ORIF for mandibular angle fractures was analyzed with respect to incision approach, incl

Introduction Sub-Saharan Africa is disproportionately affected by injuries, and short-term morbidity and mortality are high in this context. Lower limb fractures (LLFs) are a common type of injury that can lead to severe disability with extensive negative social and economic consequences. However, little is known about the long-term outcomes of LLFs in sub-Saharan Africa. In this pilot study, we describe the feasibility of assessing epidemiological patterns of LLFs, the state of care, and long-term outcomes in rural Rwanda. Methods This study was conducted from July to December 2021. It con

INTRODUCTION: Humeral shaft fractures are common fractures of the diaphysis of the humerus. The aim of this study was to evaluate factors affecting the clinical outcomes of humeral nonunions surgically treated with open reduction and single- versus double-plate fixation with grafting.
MATERIALS AND METHODS: A total of 31 patients with nonunion treated with single- or double-plate screw fixation with bone grafting were retrospectively analysed. The patients were divided into two groups according to the treatment method as Group 1 (single-plate, n = 14) and Group 2 (double-plate, n = 17). Data

BACKGROUND: The outcome of open Lisfranc injuries has been reported infrequently. Should these injuries be managed as closed injuries and is their outcome different?
METHODS: We undertook a retrospective study of high-energy, open Lisfranc injuries treated between 1999 and 2005. The types of dislocation, the associated injuries to the same foot, the radiologic and functional outcome, and the complications were studied. There were 22 patients. Five patients died. One had amputation. Of the remaining 16 patients, 13 men were followed up at a mean of 56 months (range, 29-88 months). The average

OBJECTIVE: This study aimed to radiologically assess the relationship between the tilt angle of the bipolar radial head prostheses and radiocapitellar instability.
METHODS: In this radiological study, 28 consecutive patients (13 females and 15 males: mean age=47 years and age range=23-77 years) who underwent cemented bipolar radial head arthroplasty (Judet CRF II) because of comminuted radial head fracture with elbow instability were retrospectively reviewed.
RESULTS: There was excellent intra- and interobserver reliability for the measurements of the tilt angle and the radiocapitellar dista

This retrospective study evaluated the correlation of the angulation on initial and healed radiographs of displaced surgical humerus neck fractures treated nonoperatively, with emphasis on the initial axillary view. Thirty-six patients with displaced isolated surgical humerus neck fractures were treated nonoperatively to union. Fracture angulation of the injury and healed views were measured. Angulation on the initial axillary view was a poor predictor of angulation at union. Initial axillary radiographs of surgical neck fractures treated nonoperatively overestimated final angulation, with the

AIMS: This study compares functional outcomes and patient experience between cast immobilization and early mobilization in a removable boot after ankle fracture fixation, with early weightbearing encouraged in both groups.
METHODS: This pragmatic multicentre randomized controlled trial with qualitative component and economic evaluation was conducted across eight UK NHS hospitals. Adults with acute ankle fractures were randomized to receive a plaster cast or removable support boot two weeks post-surgery. The primary outcome was ankle function measured by the Olerud and Molander Ankle Symptom S

UNLABELLED: Non-bridging external fixation has recently been introduced as an alternative to volar angular stable plating for the fixation of unstable intra-articular distal radial fractures. The purpose of this study was to biomechanically compare a new non-bridging external fixator construct to volar angular stable plate fixation in a dorsally comminuted intra-articular fracture model of the distal radius.
MATERIALS AND METHODS: Five pairs of fresh frozen human cadaveric radii were randomly supplied with either a non-bridging external fixator or a stainless steel volar locking plate. A thre

The "induced membrane" technique described by Masquelet has been used successfully for many years for posttraumatic bone defect reconstruction, non-unions and osteomyelitis. The main advantages are the two-step surgical procedure that in case of primary infection allows repeated debridement if necessary, in case of internal fixation early weight bearing with decreased malalignment risk and it has a short learning curve. A theoretical application of this procedure is the management of acute severe traumatic bone loss of the limbs despite the lack of this experience in literature. We report on a

Fractures of the coronoid process are uncommon and can easily be missed. The purpose of this study was to classify the fracture patterns and explore the treatment options. This retrospective study included 39 patients with fractures of the mandibular coronoid process. Treatment protocols were developed based on the time of fracture, degree of mouth opening, location of the coronoid fracture, types of fracture, and other concomitant fractures. All patients were followed up for 12-60 months. Sixteen patients underwent conservative management and four of these patients developed progressive trism

Bone graft is required in clinical orthopedic situations that involve a skeletal defect. Whether the defect is the result of trauma or surgery, autogenous iliac bone graft is the gold standard of therapy. The author used autogenous bone marrow/cancellous allograft as a bone graft substitute in 19 consecutive patients with a bony defect (synthetic bone substitutes were not available commercially when the series began). In this prospective study, both clinical and radiographic outcomes were followed. Of the patients, 100% were clinically healed at the site of grafting; 95% achieved radiographic

Background Distal femur fractures are complex injuries requiring surgical intervention, particularly in cases involving bone loss, comminution, or osteoporosis. While autografts remain the preferred option for bone grafting due to their osteogenic, osteoinductive, and osteoconductive properties, their use is often restricted due to donor-site morbidity, limited availability, and increased surgical time. Allografts provide an alternative, offering structural support without additional surgical site morbidity. This study evaluates the radiological and functional outcomes of allograft application

OBJECTIVE: To compare the biomechanical characteristics of the less invasive stabilization system (LISS) and the dynamic condylar screw (DCS) in the fixation of subtrochanteric fractures of the femur so as to provide theoretical basis for choosing internal fixator in clinical application.
METHODS: Twelve cadaveric human femurs (35-50 years old) were selected with similar proximal femur, excluding deformities, fractures, and other lesions. The twelve femur specimens were randomly divided into 2 groups, 6 specimens per group. An 1 cm gap of osteotomy model was made in the proximal femur up to 1
