Comparison of image quality between photon-counting detector CT and energy-integrating detector coronary CT angiography in heart transplant patients.

This study aims to compare the image quality between photon-counting detector computed tomography (PCD-CT) and energy-integrating CT (EID-CT) for coronary artery visualization post-heart transplant (HTx) patients, using a paired comparison within one patient. Consecutive HTx patients who underwent both 3rd generation dual source EID-CT and ultra-high-resolution PCD-CT scans as part of routine clinical care, with a maximum interval of two years between scans were included. Image quality was assessed subjectively using a 5-point Likert scale and quantitatively with the signal-to-noise ratio (SNR

Prognostic Value of AI-Based Quantitative Coronary CTA vs Human Reader-Based Visual Assessment: Results From the CONFIRM2 Registry.

BACKGROUND: The severity and extent of whole heart coronary plaque volume and stenosis can be reliably measured by artificial intelligence-guided quantitative coronary computed tomography angiography (AI-QCT). Limited data are available on the potential incremental prognostic value compared with currently recommended qualitative coronary computed tomography angiography (CTA) reads and the coronary artery calcium score (CACS). OBJECTIVES: The aim of this study was to evaluate the prognostic value of AI-QCT compared with human coronary CTA reads, including the CAD-RADS (Coronary Artery Disease-

Diagnostic performance of deep learning to exclude coronary stenosis on CT angiography in TAVI patients.

We evaluated the diagnostic performance of a deep-learning model (DLM) (CorEx®, Spimed-AI, Paris, France) designed to automatically detect > 50% coronary stenosis on coronary computed tomography angiography (CCTA) images. We studied inter-observer variability as an additional aim. CCTA images obtained before transcatheter aortic valve implantation (TAVI) were assessed by two radiologists and the DLM, and the results were compared to those of invasive coronary angiography (ICA) used as the reference standard. 165 consecutive patients underwent both CCTA and ICA as part of their TAVI work-up. We

CT Three-Dimensional (3D) Modeling Maintains Fluoroscopy Time and Contrast Volume to a Minimum in the Endovascular Treatment of Great Vessel Injury from Iatrogenic Trauma.

CT angiography with multislice detector has become the preferred method for assessment of hemodynamically stable patients suspected of great vessel injury from iatrogenic and blunt trauma. The CT images obtained can be transformed into a three-dimensional (3D) model using the software within minutes. This allows the clinician to evaluate the injury and the injury's proximity to other vital structures for operative planning. The 3D modeling provides geometric calibration of the c-arm or image intensifier in regard to optimal cranial/caudal and oblique angles to evaluate and treat the vessel inj

Multidimensional MDCT angiography in the context of transcatheter aortic valve implantation.

OBJECTIVE: Transcatheter aortic valve implantation has emerged as a viable treatment alternative for patients with severe aortic stenosis who are not surgical candidates. Multidimensional (3D and 4D) MDCT angiography plays a critical role in the safety, success, and outcome of an institutional transcatheter aortic valve implantation program. CONCLUSION: Given the increasing therapeutic role of this innovative technique, an understanding of essential imaging concepts in its context is critical for appropriate image acquisition and interpretation.

Multislice computed tomography in the diagnosis of superior canal dehiscence: how much error, and how to minimize it?

HYPOTHESIS: Multi-slice computed tomography (MSCT) overestimates the size of superior semicircular canal dehiscences (SSCDs) and also can misinterpret thin bone over the superior semicircular canal as dehiscent. A threshold of the radiodensity of the bone over the superior semicircular canal may exist that could optimize prediction of an actual SSCD. BACKGROUND: The gold standard for diagnosis of SSCD is MSCT, but there is a higher prevalence of SSCD based on MSCT compared with histologic studies. Overestimation of SSCD can lead to inappropriate diagnosis and treatment. METHODS: We correlate

The primary percutaneous coronary intervention for acute anterior myocardial infarction in a middle-aged male patient with bilateral coronary artery to pulmonary artery fistulas.

A 38-year-old man admitted to emergency department with 2 h of typical substernal chest pain, shortness of breath and nausea. The ECG revealed sinus rhythm with a 3 mm ST elevation in precordial leads V1-V6. The coronary angiography revealed acute total occlusion in left anterior descending artery (LAD) with normal circumflex and right coronary artery (RCA) along with bilateral fistulas arising from the proximal LAD and ostial RCA draining into the main pulmonary artery. Therefore, primary percutaneous coronary intervention and bare metal stent implantation was performed to culprit LAD lesion.

The functional single ventricle: how imaging guides treatment.

Functional single ventricle (FSV) encompasses a spectrum of severe congenital heart disease. Patients with FSV are living longer than decades prior resulting in more frequent imaging both for surgical planning and functional evaluation. At each stage of surgical intervention, imaging plays a critical role in detecting postoperative complications and preprocedural planning. This article describes the unique imaging findings, including complications, that are most important to the referring physician or surgeon at each surgical stage of FSV management. A description of lesions that embody the di

Coronary artery vasculitis: assessment with cardiac multi-detector computed tomography.

Coronary artery vasculitis is rare and comprises an array of inflammatory diseases. It often results in severe and life-threatening complications, including coronary artery aneurysm, coronary artery stenosis, intraluminal thrombosis, and microcirculation abnormalities. These may occur at a young age and are often silent in the early phases. Invasive coronary angiography is the gold standard for diagnosing coronary artery disease (CAD); however, multi-detector computed tomography (MDCT) is now widely regarded as a powerful non-invasive tool for the detection of CAD. It is important for clinicia

Effects of two different anesthetic protocols on 64-MDCT coronary angiography in dogs.

Heart rate is a major factor influencing diagnostic image quality in computed tomographic coronary artery angiography (MDCT-CA), with an ideal heart rate of 60-65 beats/min in humans. The purpose of this prospective study was to compare effects of two different clinically applicable anesthetic protocols on cardiovascular parameters and 64-MDCT-CA quality in 10 healthy dogs. Scan protocols and bolus volumes were standardized. Image evaluations were performed in random order by a board-certified veterinary radiologist who was unaware of anesthetic protocols used. Heart rate during image acquisit

Prediction of bone strength by μCT and MDCT-based finite-element-models: how much spatial resolution is needed?

OBJECTIVES: Finite-element-models (FEM) are a promising technology to predict bone strength and fracture risk. Usually, the highest spatial resolution technically available is used, but this requires excessive computation time and memory in numerical simulations of large volumes. Thus, FEM were compared at decreasing resolutions with respect to local strain distribution and prediction of failure load to (1) validate MDCT-based FEM and to (2) optimize spatial resolution to save computation time. MATERIALS AND METHODS: 20 cylindrical trabecular bone specimens (diameter 12 mm, length 15-20mm) we

Cone-beam computed tomography-based grading system for oropharyngeal airway narrowing: a novel diagnostic framework for multidisciplinary clinical use.

This technical report presents a novel CBCT-Based Grading System for Oropharyngeal Airway Narrowing, designed to provide clinicians with a standardised, objective method to assess oropharyngeal airway narrowing using Cone-Beam Computed Tomography (CBCT). The grading system is developed based on the least surface area on axial section measurements/minimal cross-sectional area (MCA) on CBCT. It classifies oropharyngeal narrowing into five distinct grades (Grade 0 to 4). Each grade also has subcategories that correspond to specific anatomical regions-distal to the soft palate (P), distal to the b

Small but high risk: a 10-year single-hospital study on sharp gastrointestinal foreign bodies-the case of the jujube pit.

BACKGROUND: Guidelines from Europe and the United States concur that sharp foreign bodies (FBs) whose length exceeds 2.5 cm seldom traverse the pylorus and are associated with serious complications. To determine whether jujube pits conform to this rule, we investigated the clinical course of patients with jujube pit impaction, characterized the patient populations at greatest risk, and compared the diagnostic performance of barium esophagography (BE) and multidetector computed tomography (MDCT). METHODS: A single-center retrospective study was conducted on cases of jujube pit impaction treate…

Sex Differences in Plaque Composition and Morphology Among Symptomatic Patients With Mild-to-Moderate Carotid Artery Stenosis.

BACKGROUND AND PURPOSE: Incidence of ischemic stroke differs between men and women, with substantially higher rates in men. The underlying mechanism of this difference remains poorly understood but may be because of differences in carotid atherosclerosis. Using an in-depth imaging-based approach, we investigated differences between carotid plaque composition and morphology in male and female patients with stroke, taking into account differences in total plaque burden. Additionally, we investigated all possible within-artery combinations of plaque characteristics to explore differences between
