Development of new treatments for congenital nystagmus.

The use of ocular motor data as the basis for the development of both nonsurgical and surgical therapies for congenital nystagmus (CN) has been underway since the mid-1960s. This paper presents three nonsurgical therapies (composite prisms, soft contact lenses, and afferent stimulation) and a new surgical therapy (four-muscle tenotomy) hypothesized from analysis of ocular motor data. The expanded nystagmus acuity function test was developed to both predict and measure the effectiveness of CN therapies and for intersubject comparisons. Base-out prisms may be used to damp CN during distance fixa

Diagnosis and treatment of patients with patellofemoral pain.

The patient-athlete with patellofemoral pain requires precise physical examination based on a thorough history. The nature of injury and specific physical findings, including detailed examination of the retinacular structure around the patella, will most accurately pinpoint the specific source of anterior knee pain or instability. Radiographs should include a standard 30 degrees to 45 degrees axial view of the patellae and a precise lateral radiograph. Nonoperative treatment is effective in most patients. Prone quadriceps muscle stretches, balanced strengthening, proprioceptive training, hip e

Long-lasting potentiation of a direct central connection between identified motor neurons in the locust.

The plasticity of the direct central connection between the fast extensor and the posterior fast flexor tibiae motor neurons in the locust (Schistocerca gregaria) metathoracic ganglion was studied. An action potential in the fast extensor results in a monosynaptic excitatory postsynaptic potential (EPSP) in the flexor motor neuron. Antidromic stimulation of the fast extensor at 100 Hz for 3.5 s resulted in a long-lasting potentiation of the EPSP amplitude. The potentiation was not dependent on feedback caused by movement of the tibia, and was associated with an increase in the input resistance

[Alloesthesia without impairment of consciousness after right putaminal small hemorrhage].

Alloesthesia is a condition in which a sensory stimulus, given on one side of the body, is perceived to be at the corresponding area on the opposite side. In our previous study (Kawamura, Hirayama et al., 1987), we suggested that it may be useful for localization because this phenomenon was observed most frequently in patients with a right putaminal hemorrhage of medium or large size, an average of 42 ml on CT scans, presenting a slight disturbance of consciousness and, in about half of the patients, anosognosia. We also suggested that since alloesthesia is produced not only in cerebral but al

[Function-oriented neurophysiologic diagnosis: long loop reflexes in spinal and cerebral lesions].

Neurophysiological methods which allow to identify objectively lesions of the efferent cortico-spinal pathways in humans are with the exception of transcutaneous electrical stimulation of the motor cortex nor available. Long latency EMG responses from leg muscles are mediated by a "transcortical loop" and offer the possibility to detect in combination with sensory evoked potentials lesions both of the afferent proprioceptive and the efferent motor pathways. Subjects stood on a platform which was rotated toe-up (50 degrees/s, 4 degrees) around the axis of the ankle joint. This lead to two EMG r

Structural equation model of knee pain in individuals with knee osteoarthritis-associated knee pain, based on functional capacity, kinesiophobia, muscle thickness, and joint position sense.

BACKGROUND: There are several clinical parameters that contribute to knee osteoarthritis (KOA)-associated knee pain and/or its progression such as kinesiophobia, functional capacity, muscle thickness, and joint position sense. The relationship between knee pain and those aforementioned clinical parameters is needed for clear understanding. Therefore, this study constructed the structural equation model of knee pain with the variables of kinesiophobia, functional capacity, muscle thickness, and joint position sense in individuals with KOA-associated knee pain. METHODS: We examined 200 individu

Eye Movements as Indicators of Vestibular Dysfunction.

Virtual reality technologies are in wide use in sport psychology. An advantage of this kind of technology is the possibility to assess sportspeople's readiness to perform complex movements. This study is aimed at developing a method for the evaluation of vestibular function disturbances in young skaters. Such disturbances may occur while skaters are performing rotation movements. To achieve this goal, we induced a vection illusion, accompanied by virtual environment rotation in a CAVE virtual reality system. Vestibular disturbances were tested for two groups-professional skaters and people who

Effects of surgical repair on active shoulder position sense and 3-dimensional reaching accuracy in a patient with anterior shoulder instability.

The purpose of this report was to investigate whether a subject with anterior shoulder instability exhibited better active shoulder position sense and 3-dimensional (3-D) reaching accuracy after a surgical repair. The 19-year-old male subject underwent an open Bankart repair procedure for his left shoulder followed by a standardized post-operative rehabilitation program. Shoulder position sense was examined with traditional passive matching and active positioning protocols. Reaching accuracy in space with the unrestricted arm motion was also examined. The subject was tested 5 months prior to t

Postural stability during visual stimulation and the contribution from the vestibular apparatus.

CONCLUSION: When combined with vestibular dysfunction, visual flow can exacerbate reductions in postural stability. This effect may be one of the mechanisms underlying visual vertigo, which can be evaluated using frequency analysis of body sway elicited by optokinetic stimulation (OKS). OBJECTIVE: To clarify the interaction between the postural responses to visual flow and to input from the vestibular apparatus as a mediator of visual vertigo. METHODS: Horizontal and vertical OKS with a stable fixation target were presented to 14 healthy subjects and 38 peripheral vestibular patients standin

Center of mass control and multi-segment coordination in children during quiet stance.

This study aimed to apply an uncontrolled manifold (UCM) approach to investigate how children utilize the variability of multiple body segment movement to facilitate the center of mass (COM) control during quiet stance. Three groups of participants were included in this study: younger children (YC, mean age 6.3 years), older children (OC, mean age 10.3 years), and young adults (YA, mean age 20.5 years). Participants stood on a force platform with their hands on the iliac crests for 40 s in each trial. Two visual conditions were examined including eyes-open and eyes-closed and three trials were

Vestibular and pulse-related modulation of skin sympathetic nerve activity during sinusoidal galvanic vestibular stimulation in human subjects.

We have previously shown that sinusoidal galvanic vestibular stimulation (sGVS), a means of a selectively modulating vestibular afferent input without affecting other inputs, can cause partial entrainment of muscle sympathetic nerve activity (MSNA). Given that motion sickness causes sweating and pallor, we tested the hypothesis that sGVS also entrains skin sympathetic nerve activity (SSNA), but that the optimal frequencies are closer to those associated with slow postural changes (0.2 Hz). SSNA was recorded via tungsten microelectrodes inserted into the common peroneal nerve in 11 awake-seated

Rat model of intracerebellar hemorrhage.

Approximately 15% of all strokes are due to intracerebral hemorrhage (ICH) and of these, 5-10% occur in the cerebellum. The resultant mortality is around 20-30%. However, there is no well-established animal model to address this important clinical problem. We induced intracerebellar hemorrhage in rats using stereotaxic collagenase injection through a burr-hole into right cerebellum. Dosage-dependent effect of collagenase (0.2, 0.4, and 0.6 U) was tested in male and female rats. Brain edema formation was assessed by brain water content and hemorrhagic volume measured by hemoglobin assay. Wire s

Leukocyte trafficking and pain behavioral responses to a hydrogen sulfide donor in acute monoarthritis.

Hydrogen sulfide (H(2)S) is an endogenous gaseous mediator with the ability to modulate tissue inflammation and pain. The aim of this study was to determine the effect of an H(2)S donor (Na(2)S) on leukocyte-endothelium interactions, blood flow, and pain sensation in acutely inflamed knee joints. Acute arthritis was induced in urethane anesthetized C57bl/6 mice by intra-articular injection of kaolin/carrageenan (24-h recovery), and the effect of local administration of Na(2)S on leukocyte trafficking was measured by intravital microscopy. Synovial blood flow was measured in inflamed knees by l

Oculomotor cerebellum.

The anatomical, physiological, and behavioral evidence for the involvement of three regions of the cerebellum in oculomotor behavior is reviewed here: (1) the oculomotor vermis and paravermis of lobules V, IV, and VII; (2) the uvula and nodulus; (3) flocculus and ventral paraflocculus. No region of the cerebellum controls eye movements exclusively, but each receives sensory information relevant for the control of multiple systems. An analysis of the microcircuitry suggests how sagittal climbing fiber zones bring visual information to the oculomotor vermis; convey vestibular information to the

Vertigo due to whiplash injury: a neurotological approach.

A series of examinations were carried out to elucidate the characteristics of vertigo due to whiplash injury. In patients with whiplash injury, there are over-excitation of the cervical and/or the lumbar proprioceptors on one hand, and dysfunction of the central nervous system, such as the hypothalamus, the brain stem and the cerebellum, on the other hand. These two etiological factors induce disequilibrium by a trigger-and-target relationship in which the above proprioceptors act as a trigger and the central nervous system acts as a target. This postulate is applicable to the explanation of a

Unraveling the Left-Right Judgment Task in Chronic Low Back Pain: Insights Through Behavioral, Electrophysiological, Motor Imagery, and Bodily Disruption Perspectives.

Bodily disruptions have been consistently demonstrated in individuals with chronic low back pain. The performance on the left-right judgment task has been purposed as an indirect measure of the cortical proprioceptive representation of the body. It has been suggested to be dependent on implicit motor imagery, although the available evidence is conflicting. Hence, the aim of this case-control observational study was to examine the performance (accuracy and reaction times) and event-related potentials while performing the left-right judgment task for back and hand images in individuals with chro

People with chronic ankle instability benefit from brace application in highly dynamic change of direction movements.

BACKGROUND: The application of ankle braces is an effective method for the prevention of recurrent ankle sprains. It has been proposed that the reduction of injury rates is based on the mechanical stiffness of the brace and on beneficial effects on proprioception and neuromuscular activation. Yet, how the neuromuscular system responds to the application of various types of ankle braces during highly dynamic injury-relevant movements is not well understood. Enhanced stability of the ankle joint seems especially important for people with chronic ankle instability. We therefore aimed to analyse t…

Posturographic changes associated with music listening.

OBJECTIVES: The aim of this study was to ascertain whether listening to music might cause changes in human stability and be useful in fall prevention and rehabilitation. The aim was also to find what percentage of subjects without neurologic signs or symptoms associated with falling had less than ideal stability. DESIGN: Computer dynamic posturography (CDP) provided stability scores in 266 subjects without a history of falls or vertigo. Subjects were randomized into several different music listening groups and one control group. The music listening groups were given a daily specific music lis
