Increased IL-1beta expression and myofibroblast recruitment in subacromial bursa is associated with rotator cuff lesions with shoulder stiffness.

We evaluated whether proinflammatory cytokine expression and myofibroblast recruitment in subacromial bursa was linked to rotator cuff lesions with shoulder stiffness. We analyzed expressions of IL-1beta, IL-6, and TNF-alpha in subacromial bursa and joint fluid collected from 14 patients with cuff tears with stiffness as a study group (Group I) and 14 patients with rotator cuff tears without shoulder stiffness as a control group (Group II) using real-time RT-PCR, immunohistochemistry, and ELISA. Myofibroblast apoptosis in subacromial bursa was analyzed using terminal deoxynucleotidyl transfera

Failed surgical management of partial thickness rotator cuff tears.

Some authors have recommended that rotator cuff tears <50% of tendon thickness be debrided and those involving >50% of the tendon be treated with miniopen repair. We hypothesize that if indications for selecting between simple debridement and tendon repair were appropriate, then both groups should have comparable outcomes. Thirty-nine patients with partial rotator cuff tears met inclusion criteria and were available for retrospective analysis. Twenty-six percent of patients who underwent debridement and 12.5% of patients who had mini-open repair had unsatisfactory results according to Neer's c

Perioperative transition experience of patients undergoing shoulder arthroscopy day surgery: a qualitative exploratory study using patient journey mapping.

BACKGROUND: With the advancement of orthopedic enhanced recovery after surgery (ERAS), the frequency of orthopedic procedures performed as day surgeries has increased. Improvements in shoulder arthroscopy techniques and updates in ERAS protocols have enabled arthroscopic rotator cuff repair (ARCR) to be conducted as a day surgery. However, the reduced hospital stay introduces significant challenges for patients. Limited research exists on how day surgery patients manage the transition between home and the surgical setting, although these experiences and coping strategies have a substantial imp

[Association of increased greater tubercle angle and critical shoulder angle with rotator cuff tears].

OBJECTIVE: The greater tuberosity angle (GTA) and critical shoulder angle (CSA) are commonly referred to as radiographic markers which were used to described morphology of the greater tuberosity and acromion respectively. At present, most international studies focus on the correlation between the above two parameters and rotator cuff tears (RCTs), and their diagnostic value and risk assessment. This study attempts to find out the trend of GTA and CSA changes and risk threshold of RCTs, as well as the protective factors and risk factors. METHODS: In this study, 130 individuals from May 2019 to

Editorial Commentary: Biomechanical Data and Early Clinical Results Demonstrate an Emerging Strategy for Combining Superior Capsule Reconstruction and Lower Trapezius Transfer in the Management of Massive Irreparable Posterosuperior Rotator Cuff Tear.

Numerous biomechanical studies demonstrate that superior capsule reconstruction (SCR) improves glenohumeral superior translation and subacromial contact pressures in cadaveric models of massive irreparable rotator cuff tears (MIRCT) at time zero. These findings provide a logical basis for clinical observations of significant improvements in pain, range of motion, and patient-reported outcome measures at short-term follow-up. However, the efficacy of SCR at longer-term follow up is controversial, and a recent survey of AANA members demonstrates that there is decreasing use of SCR for MIRCTs as

Subacromial Balloon Spacer Does Not Reduce the Retear Rate for Massive Rotator Cuff Tears: A Comparative Study.

PURPOSE: To determine whether a subacromial spacer decreases the recurrent rotator cuff tear rate in arthroscopically managed massive rotator cuff tears (MRCTs) with 1 year of follow-up. METHODS: We selected all patients who met the following criteria: (1) an MRCT excluding Collin type A, (2) Goutallier stage equal or less than 2, and (3) complete arthroscopic repair of the MRCT. Patients were allocated into 2 groups: A (without subacromial spacer) or B (with subacromial spacer) for a prospective evaluation 1 year after surgery. The primary outcome was the retear rate, determined with magneti

Comparison of Survivorship and Failure Modes Between Anatomic and Reverse Total Shoulder Arthroplasty Across Multiple Government Joint Registries for a Single Platform Shoulder System.

INTRODUCTION: We analyzed two different government joint registries for survivorship associated with one platform shoulder system and compared reasons for revision and trends in usage of anatomic total shoulder arthroplasty (aTSA) and reverse total shoulder arthroplasty (rTSA) over a period of more than 10 years to elucidate reasons for any changes in market trends. METHODS: A review of the United Kingdom (UK) and Aus- tralian national joint registries was performed for a single platform shoulder prosthesis (Equinoxe; Exactech, Inc, Gainesville, Florida, USA) from 2011 to 2022 to investigate

Recurrence in traumatic anterior shoulder dislocations increases the prevalence of Hill-Sachs and Bankart lesions: a systematic review and meta-analysis.

PURPOSE: The extent of shoulder instability and the indication for surgery may be determined by the prevalence or size of associated lesions. However, a varying prevalence is reported and the actual values are therefore unclear. In addition, it is unclear whether these lesions are present after the first dislocation and whether or not these lesions increase in size after recurrence. The aim of this systematic review was (1) to determine the prevalence of lesions associated with traumatic anterior shoulder dislocations, (2) to determine if the prevalence is higher following recurrent dislocatio

The association between critical shoulder angle and revision following anatomic total shoulder arthroplasty: a matched case-control study.

HYPOTHESIS: The concept of the critical shoulder angle (CSA) was introduced in 2013, with studies showing that larger CSA is associated with rotator cuff tears (RCTs) and smaller CSA with glenohumeral osteoarthritis. We hypothesized outcomes following total shoulder arthroplasty (TSA) would differ depending on CSA. METHODS: We conducted a matched case-control study using Kaiser Permanente's Shoulder Arthroplasty Registry to identify patients who underwent primary elective anatomic TSA for the diagnosis of osteoarthritis from 2009-2018. Seventy-eight adult patients who underwent revision follo

Isolated Radial Nerve Palsy as a Complication After Anterior Dislocation of the Glenohumeral Joint: A Case Report and Clinical Review.

Shoulder dislocation is the most common feature in emergencies, while the anterior dislocation of the glenohumeral joint is the most frequent and requires reduction. Accompanied nerve injury is common with an incidence of 21%, while radial nerve palsy is very rare. We describe the case of a 56-year-old man who presented with an anterior dislocation of the left shoulder due to a fall on an outstretched hand with wrist drop 8 hours after injury. Neurological examination revealed loss of sensation along the radial border of the forearm. Closed reduction with Kocher procedure was performed. Magnet

Tolerance and effect of sodium thiosulfate in calcific tendinitis of the rotator cuff.

BACKGROUND: Calcific tendinitis of the rotator cuff is one of the most common causes of shoulder pain. Ultrasound-guided percutaneous lavage of calcific tendinopathy is performed when conservative treatments have failed. Sodium thiosulfate (STS) has recently been used with success in the treatment of tumoral calcinosis. The goal of this phase II study was to assess the tolerance and the feasibility of STS lavage of calcific tendinopathy. METHODS: We included patient with type hard calcifications. Patients were treated with puncture and lavage followed by injection of STS in the calcification.

MicroRNA-29a Mitigates Subacromial Bursa Fibrosis in Rotator Cuff Lesion with Shoulder Stiffness.

Rotator cuff lesion with shoulder stiffness is a major cause of shoulder pain and motionlessness. Subacromial bursa fibrosis is a prominent pathological feature of the shoulder disorder. MicroRNA-29a (miR-29a) regulates fibrosis in various tissues; however, the miR-29a action to subacromial bursa fibrosis remains elusive. Here, we reveal that subacromial synovium in patients with rotator cuff tear with shoulder stiffness showed severe fibrosis, hypertrophy, and hyperangiogenesis histopathology along with significant increases in fibrotic matrices collagen (COL) 1A1, 3A1, and 4A1 and inflammato

Transtendinous repair of partial articular sided supraspinatus tears is associated with higher rates of stiffness and significantly inferior early functional scores than tear completion and repair: A systematic review.

INTRODUCTION: Transtendon repair (TTR) and tear completion and repair (TCR) are common repair techniques for partial thickness rotator cuff tears (PTRCTs). Previous systematic reviews have not demonstrated any advantage of either but have not specifically addressed early recovery. AIM: To compare the outcomes of these two techniques in treating PTRCTs with respect to post-operative stiffness, delay in functional recovery and re-tear rates. MATERIAL AND METHODS: A systematic review of the Medline and EMBASE database was performed in accordance with the PRISMA guidelines. Both cases series and

One-stage surgical treatment for concomitant rotator cuff tears with shoulder stiffness has comparable results with isolated rotator cuff tears: a systematic review.

BACKGROUND: Addressing preoperative shoulder stiffness before rotator cuff repair (RCR) is advocated, but the effectiveness of this approach is debatable. We hypothesized that 1-stage treatment of concomitant rotator cuff tear (RCT) with shoulder stiffness has comparable results with isolated RCT. METHODS: Following the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, the databases including MEDLINE, Embase, Cochrane Library, and Scopus were searched using the keywords of "shoulder stiffness" OR "adhesive capsulitis" OR "frozen shoulder" AND "rotator cuf

[Smoking Status and a Pulmonary Function Test in Patients with Rotator Cuff Tears].

Although chronic obstructive pulmonary disease (COPD) affects one in several smokers, only a few patients are correctly diagnosed compared to the estimated number of patients. Several recent reports indicate that the development of rotator cuff tears is related to smoking. In this study, we investigated smoking status in patients with rotator cuff tears and evaluated the possibility of undiagnosed COPD by a pulmonary function test. The subjects were 150 consecutive patients over 40 years old, who had been diagnosed with rotator cuff tears and had been examined by a pulmonary function test befo

Rotator cuff tear and sarcopenia: are these related?

BACKGROUND: Sarcopenia is the loss of muscle mass and consequent loss of muscle function with aging. Its prevalence among the general population is 12% to 30% in those aged >60 years. We evaluated (1) the difference in the prevalence of sarcopenia between patients with rotator cuff tear and controls and (2) the sarcopenia severity according to the size of the rotator cuff tear. METHODS: Group 1 included 48 consecutive patients with chronic symptomatic full-thickness rotator cuff tears (mean age, 60.1 ± 6.5 years; range, 46-76 years), and group 2 included 48 age- and sex-matched patients. Chronic Irreducible Anterior Dislocation of the Shoulder without Significant Functional Deficit.

Shoulder dislocation is frequently encountered by orthopedists, and closed manipulation is often sufficient to treat the injury in an acute setting. Although most dislocations are diagnosed and managed promptly, there are rare cases that are missed or neglected, leading to a chronically dislocated state of the joint. They are usually irreducible and cause considerable pain and functional disability in most affected patients, prompting the need to find a surgical method to reverse the worsening conditions caused by the dislocated joint. However, there are cases of even greater rarity in which c
