HIV infection among homeless adults and runaway youth, United States, 1989-1992. Field Services Branch.

OBJECTIVES: Homeless persons have an increased risk of HIV infection because of a high prevalence of HIV-related risk behaviors. These include drug use, sexual contact with persons at risk for HIV infection, and the exchange of sex for drugs. The objectives of this investigation were to describe HIV seroprevalence rates in homeless adults and runaway youth. METHODS: In 1989, the Centers for Disease Control and Prevention began collaboration with state and local health departments to conduct HIV seroprevalence surveys in homeless populations. Unlinked HIV seroprevalence surveys were conducted

The spectrum of HIV related skin diseases in an Irish population.

A prospective study was carried out between January 1988 and January 1990 in St. James's hospital, Dublin, which is the chief referral centre for the HIV population in Ireland. Patients were routinely referred from the genitourinary medicine department to the dermatology clinic for a full dermatological assessment which was carried out by one physician (LB). The patients were not preselected on the basis of a skin complaint. The study aimed to detect the prevalence of skin disease amongst this HIV group of patients and to compare them with an age and sex matched group of normal controls. There

Politics and Pediatrics: State Political Policies' Impact on Pediatrics Residency Application and Future Practice Decisions.

OBJECTIVE: Understanding whether political policies influence residents' training and career decision-making is critical to address pediatric workforce concerns. METHODS: We surveyed pediatric residents on factors influencing residency application/ranking decisions (interns only) and future practice plans (all). Respondents reporting "state political policies" as an influence identified which specific topics affected them, the degree and direction of influence, and reasons for this influence. Responses were analyzed using standard descriptive statistics and compared by respondent demographics

Intersectional stigma among LGBTQIA+ adolescents with chronic primary musculoskeletal pain and its association with functioning and depressive symptoms.

OBJECTIVE: Many adolescents with chronic primary musculoskeletal pain (CPMP) feel stigmatized by their pain condition. Studies have not previously evaluated whether having another stigmatized identity (i.e., intersectional stigma) contributes to poorer physical and psychosocial outcomes in this population. The current study evaluated the hypothesis that adolescents with CPMP identifying as having a minoritized sexual orientation and/or gender identity (lesbian, gay, bisexual, transgender, queer, intersex, and asexual [LGBTQIA+]) experience greater pain-related stigma, perceived injustice, func

Potential Risks and Benefits of Low-Barrier Access and Monitoring for HIV Preexposure Prophylaxis: A Modeling Study.

BACKGROUND: PrEP prevents HIV acquisition, but its population-level impact is limited by access barriers. Low-barrier delivery of daily oral tenofovir disoproxil fumarate/emtricitabine (TDF/FTC), such as over-the-counter (OTC) availability, could improve access by eliminating clinic visits, though reduced monitoring may increase risks including drug resistance, renal toxicity, and hepatitis B virus (HBV) reactivation. We estimated the benefits and risks of OTC PrEP. METHODS: Using a stochastic network-based model of HIV/STI transmission among gay, bisexual, and other men who have sex with men

Group size modulates kinship dynamics and selection on social traits.

An individual's relatedness to its group may change with age due to demographic processes, and such kinship dynamics can shape age-linked social behaviors. Existing models, however, have focused almost exclusively on dispersal and mating, leaving the consequences of group size local variation unexplored. Here, we extend these models to incorporate such variation within a single, genetically connected meta-population. We then contrast predicted kinship dynamics and age-linked selection for helping/harming in coexisting smaller and larger groups in three typical social mammal systems characteriz

Palliative Care Awareness and Misconceptions By Sexual Orientation: HINTS 2018 and 2024.

INTRODUCTION: Sexual minority individuals with serious illness experience persistent inequities in end-of-life care compared with their heterosexual counterparts, including reduced access to timely, high-value palliative care (PC). This study examined differences in PC awareness and misconceptions between U.S. sexual minority (lesbian, gay, and bisexual) and heterosexual adults. METHODS: We analyzed nationally representative data from the 2018 Health Information National Trends Survey (HINTS) Cycle 2 and the 2024 HINTS 7. Sequential multivariable robust Poisson regression models were used to

The Down Low Among Latino Men Who Have Sex With Men: A Review and Theoretical Analysis.

Being on the "down low" (DL) describes men who identify as straight and secretly engage in same-sex encounters while maintaining heterosexual public identities. While the DL phenomenon has been discussed primarily in the context of African American men, Latino men in the United States experience similar patterns of sexual concealment shaped by cultural forces not experienced similarly by other groups. Relatively little is still known about how machismo, familismo, and religious values affect the sexual decisions and health outcomes of Latino men who have sex with men (MSM), and most existing r

Training through trauma: A multivariate analysis of burnout and associated factors among physician trainees in Guatemala.

OBJECTIVE: Burnout is a pervasive occupational hazard in medicine, beginning early in training and linked to depression, suicide, medical errors, and attrition. While extensively studied in high-income countries, little is known from Latin America. Guatemala faces scarce mental health resources, social inequities, and limited residency regulation. This study aimed to characterize burnout, psychological distress, and moral injury among Guatemalan physicians in postgraduate medical training ("residents"). METHOD: We conducted a cross-sectional survey of residents in Guatemala (September-October

Disparities in Poly-Tobacco Use Among United States Adults in the All of Us Research Program.

INTRODUCTION: Although overall tobacco use has declined in the United States (U.S.), poly-tobacco use remains a concern due to increased risk of toxicant exposure and addiction potential, with disparities across sociodemographic groups. This study examined the prevalence of tobacco use patterns among different sociodemographic groups in U.S. adults. METHODS: We analyzed data from 480,608 U.S. adults (aged ≥18) in the All of Us Research Program. Tobacco use was classified as single-tobacco product (any 1 product), dual-tobacco (any 2 products), or poly-tobacco (any 3+ products) use. Sociodemog

Disparities in healthcare access among sexual and gender minority individuals with generalized anxiety disorder in the US.

PURPOSE: We aimed to quantify and compare the prevalence of healthcare access barriers among individuals diagnosed with generalized anxiety disorder (GAD), focusing on differences between sexual and gender minority (SGM) and non-SGM adults. We also examined whether intersecting identities further compounded these barriers to inform equitable healthcare policy. METHODS: We analyzed data (2017-2022) from the All of Us Program, a nationwide database of self-reported surveys, electronic health records, and demographic information. Our sample included 8,841 adults diagnosed with GAD. Participants

Partners, Pride, and Prevention: Scaling Mpox Vaccination Access Across Minnesota.

Mpox is a rare but potentially serious vaccine-preventable disease. The 2022 United States outbreak disproportionately impacted gay, bisexual, and other men who have sex with men, people living with HIV, and people of transgender experience. Early vaccination efforts revealed substantial racial and geographic inequities, with lower uptake among Black and Hispanic cisgender men, transgender women, and residents of rural areas. To address these challenges, Fairview's Minnesota Immunization Networking Initiative (MINI), a 20-year-old mobile health collaborative, partnered with state and local pub

Indicator condition-guided HIV testing in patients with mononucleosis-like illness and community-acquired pneumonia in an Emergency Department setting: A prospective study from Poland (2015-2018).

BACKGROUND: Despite advances in HIV care, late diagnosis remains a major public health challenge in Europe, with nearly half of HIV diagnoses in the EU/EEA occurring at a late stage. In Poland, a substantial proportion of people living with HIV remain undiagnosed. Indicator condition-guided testing has been proposed as an effective strategy to reduce missed opportunities for earlier diagnosis. METHODS: A prospective cohort study was conducted at the Emergency Department of the Hospital for Infectious Diseases in Warsaw, Poland, between May 2015 and March 2018, as part of the European OptTEST

Daily HIV pre-exposure prophylaxis enhances monocyte activation and reprogrammes immune cell metabolism.

INTRODUCTION: Pre-exposure prophylaxis (PrEP) with tenofovir/emtricitabine (TDF/FTC) is highly effective for HIV prevention. While antiretroviral therapy (ART) is linked to chronic inflammation in people living with HIV, its direct effects on immune phenotype, function, and metabolism in HIV-negative individuals remain unclear. This study aimed to investigate how daily TDF/FTC pre-exposure prophylaxis modulates immune activation, functional responses, and metabolic programming in innate and adaptive immune cells in HIV-negative individuals. METHODS: Gay, bisexual, and other men who have sex w

Operationalizing Health Equity in Public Emergency and Primary Care in Urban Brazil.

Health systems globally continue to struggle to translate health equity commitments into consistent day-to-day practice at the point of care. Although equity strategies are often articulated at national or system levels, frontline health care services frequently lack the governance structures, partnerships, operational tools, and learning environments needed to prioritize populations experiencing inequities amid competing clinical and organizational demands. This challenge is particularly visible in high-volume, resource-constrained settings. Early findings suggest that equity work becomes mor
