Growth Hormone Dosing Estimations Based on Body Weight Versus Body Surface Area.

OBJECTIVE: Both body weight (BW)- and body surface area (BSA)-based dosing regimens have been recommended for growth hormone (rhGH) replacement. The aim was to compare the two regimens to determine if either resulted in inadequate treatment depending on anthropometric factors. METHODS: The retrospective study included children diagnosed with idiopathic isolated growth hormone deficiency. BW-based dosing in mcg/kg/day was converted to BSA in mg/m2/day to determine the equivalent amounts of the given rhGH. Those with a BW-to-BSA ratio of more than 1 were allocated to the “relatively over-dosed

Preventing hypothermia in pediatric neurosurgery in Africa-A randomized controlled non-inferiority trial of insulation versus active warming.

PURPOSE: The objective of this study was to compare the efficacy of a low-cost heat-preserving method in preventing intraoperative hypothermia with that of forced-air warming in a resource-limited setting. METHODS: In this randomized controlled non-inferiority trial, we recruited children younger than 12 years scheduled for cranial neurosurgery in a large East-African hospital. Patients were block-randomized by age to intraoperative warming measures using Hibler's method (intervention) or warm air (comparator). Hibler's group patients were circumferentially wrapped in transparent plastic shee

Acute Administration of Bioavailable Curcumin Alongside Ferrous Sulphate Supplements Does Not Impair Iron Absorption in Healthy Adults in a Randomised Trial.

Ferrous sulphate (FS) is a cost effective, readily available iron supplement for iron deficiency (ID). The pro-oxidant effect of oral ferrous iron is known to induce inflammation, causing gastric side-effects and resulting in poor compliance. Curcumin is a potent antioxidant and has also been shown to exhibit iron chelation in-vitro, although it is not established whether these effects are retained in-vivo. The aim of this study was therefore to assess the influence of a formulated bioavailable form of curcumin (HydroCurcTM; 500 mg) on acute iron absorption and status in a double blind, placeb

Oral melatonin as a new tool for neuroprotection in preterm newborns: study protocol for a randomized controlled trial.

BACKGROUND: Prevention of neurodevelopmental impairment due to preterm birth is a major health challenge. Despite advanced obstetric and neonatal care, to date there are few neuroprotective molecules available. Melatonin has been shown to have anti-oxidant/anti-inflammatory effects and to reduce brain damage, mainly after hypoxic ischemic encephalopathy. The planned study will be the first aiming to evaluate the capacity of melatonin to mitigate brain impairment due to premature birth. METHOD: In our planned prospective, multicenter, double-blind, randomized vs placebo study, we will recruit,

Effectiveness of dual-task functional power training for preventing falls in older people: study protocol for a cluster randomised controlled trial.

BACKGROUND: Falls are a major public health concern with at least one third of people aged 65 years and over falling at least once per year, and half of these will fall repeatedly, which can lead to injury, pain, loss of function and independence, reduced quality of life and even death. Although the causes of falls are varied and complex, the age-related loss in muscle power has emerged as a useful predictor of disability and falls in older people. In this population, the requirements to produce explosive and rapid movements often occurs whilst simultaneously performing other attention-demandi

Bypass versus angioplasty in severe ischaemia of the leg (BASIL): multicentre, randomised controlled trial.

BACKGROUND: The treatment of rest pain, ulceration, and gangrene of the leg (severe limb ischaemia) remains controversial. We instigated the BASIL trial to compare the outcome of bypass surgery and balloon angioplasty in such patients. METHODS: We randomly assigned 452 patients, who presented to 27 UK hospitals with severe limb ischaemia due to infra-inguinal disease, to receive a surgery-first (n=228) or an angioplasty-first (n=224) strategy. The primary endpoint was amputation (of trial leg) free survival. Analysis was by intention to treat. The BASIL trial is registered with the National R

Desflurane compared with propofol for postoperative sedation in the intensive care unit.

BACKGROUND: We hypothesized that emergence from sedation in postoperative patients in the intensive care unit would be faster and more predictable after sedation with desflurane than with propofol. METHODS: Sixty patients after major operations were allocated randomly to receive either desflurane or propofol. The target level of sedation was defined by a bispectral index(TM) (BIS(TM)) of 60. All patients were receiving mechanical ventilation of the lungs for 10.6 (SD 5.5) h depending on their clinical state. The study drugs were stopped abruptly in a calm atmosphere with the fresh gas flow se

Clinical potential of insulin therapy in critically ill patients.

Stress of critical illness is often accompanied by hyperglycaemia, whether or not the patient has a history of diabetes mellitus. This has been considered to be part of the adaptive metabolic response to stress. The level of hyperglycaemia in patients with acute myocardial infarction (MI) or stroke upon admission to the hospital has been related to the risk of adverse outcome. However, until recently, there was no evidence of a causal relationship and thus stress-induced hyperglycaemia was only treated with exogenous insulin when it exceeded 12 mmol/L (220 mg/dL). In patients with known diabet

[Clinical aspects, epidemiology and economic impact of foot epidermomycosis in an industrial milieu].

Fungal infections of the feet are very common in some professions. This has been particularly studied in coal mine workers up to 50 p. 100 of whom were found to have dermatophytosis of the feet. The purpose of this study was to determine the clinical, epidemiological and evolutive characteristics of interdigital and plantar intertrigo of the feet among people working in a coking plant, a potash mine and a motorcar factory. We have also evaluated the cost of treatments in relation to their immediate effectiveness and their preventive effects on relapses. Initially, 208 workers entered the study

The effect of the web-based Barkley behavioral training model on the caregiving burden and resilience of mothers of children with attention-deficit/hyperactivity disorder: a randomized controlled trial.

BACKGROUND: Mothers of children with attention-deficit/hyperactivity disorder (ADHD) often face considerable challenges in communicating with their children, which increases their caregiving burden. Resilience, as an important psychological resource, can help mitigate the negative effects of these challenges. Barkley's behavioral parent training, especially when delivered web-based with easy access and flexible scheduling, may provide valuable support for these mothers. This study examined the impact of a web-based behavioral training program based on Barkley's model on caregiving burden and r

Multicentre, controlled, randomised and single-blind, adaptive phase IV protocol to evaluate effectiveness and cost-effectiveness of a pre-emptive genotyping strategy to optimise tacrolimus dosage in a pretransplant chronic kidney disease population cohort: iPHARMGx TRANSPGx nested clinical trial.

INTRODUCTION: Genetic variations impact drug response, driving the need for personalised medicine through pre-emptive pharmacogenetic testing. However, the adoption of pre-emptive pharmacogenetic testing for commonly prescribed drugs, such as tacrolimus, outside of tertiary hospitals is limited due to a lack of pharmacoeconomic evidence to support widespread implementation by healthcare policymakers. The iPHARMGx Consortium addresses this by developing the TRANSPGx clinical trial to assess the hypothesis that widespread adoption of a pre-emptive genotyping scheme in populations susceptible to

Core outcomes for the evaluation of new healthcare programmes - a modified Delphi study.

BACKGROUND: New healthcare programmes that focus on the encounter between patients and healthcare services, such as those advocated for in different forms on "centredness", are being pushed to the forefront of the healthcare agenda to, amongst other goals, combat rising costs. However, lack of consensus regarding which outcomes should be evaluated to cover the needs of all stakeholders creates barriers to prioritising between competing alternatives. The aim of this research was to develop a core outcome set (COS) for the evaluation of new healthcare programmes adopting centredness within the e

Global, regional, and National burdens of cervical spinal cord injuries from 1990 to 2021, findings from global burden of disease study 2021.

BACKGROUND: Spinal cord injuries (SCI) are major contributors to global disability, with high incidence rates, substantial medical costs, and profound long-term impacts. Cervical spinal cord injuries (cSCI), in particular, lead to severe outcomes, making them a critical public health challenge. While the rising global burden of SCI, including cSCI, is well-known, comprehensive and updated epidemiological studies specifically focused on cSCI, especially those that project future trends and analyze regional disparities, remain limited. This study aims to provide novel insights into the evolving

A Comparative Study of Radiological, Functional, and Clinical Outcomes of Three-Pin Versus Five-Pin Techniques for Distal Radius Fracture Fixation.

Introduction Distal radius fractures are among the most frequently encountered injuries in orthopedic practice, and their optimal management remains a topic of considerable debate. Percutaneous pinning with Kirschner wires (K-wires) is a widely accepted treatment modality, valued for its minimal invasiveness and cost-effectiveness. However, the ideal number and configuration of pins required to balance fracture stability with potential complications is unclear. This study aims to compare the radiological, functional, and clinical outcomes of three-pin versus five-pin fixation techniques for di

Unravelling the benefits of thermal waters enhancing oral health: a pilot study.

BACKGROUND: Oral health represents a public health problem due to its remarkable social impact and medical costs. Crenotherapy with sulfur water is shown to be a complementary, less toxic, and traumatizing therapy, but the number of studies that evaluate the effect of natural mineral waters effect on oral health are scarce. The aim of this pilot study is to evaluate the impact of thermal water therapy on the oral health of the participants, assessing parameters such as plaque index, gingival bleeding index and periodontal probing depth as well on the perception of symptoms of oral mucosa disea

MultiSCRIPT-Cycle 1-a pragmatic trial embedded within the Swiss Multiple Sclerosis Cohort (SMSC) on neurofilament light chain monitoring to inform personalized treatment decisions in multiple sclerosis: a study protocol for a randomized clinical trial.

BACKGROUND: Treatment decisions for persons with relapsing-remitting multiple sclerosis (RRMS) rely on clinical and radiological disease activity, the benefit-harm profile of drug therapy, and preferences of patients and physicians. However, there is limited evidence to support evidence-based personalized decision-making on how to adapt disease-modifying therapy treatments targeting no evidence of disease activity, while achieving better patient-relevant outcomes, fewer adverse events, and improved care. Serum neurofilament light chain (sNfL) is a sensitive measure of disease activity that cap
