The trauma of birth.

The trauma of birth is an international concern for all childbearing women globally. Since changes in 1994 to the Diagnostic Statistical Manual that included childbirth as a potentially traumatic event, several clusters of researchers, particularly representing the Scandinavian countries, the United Kingdom, and Australia, have emerged. Their research findings appear in numerous publications; yet, what is known from these studies is based on a variety of methodological designs and differing measurement tools making it difficult to draw many firm conclusions (Ayers, 2004 ; Ayers, Joseph, Mc-Ken

Severe Hypercalcemia in a Postpartum Patient.

BACKGROUND: There are multiple clinical manifestations of hypercalcemia and several causes of hypercalcemia. Hypercalcemia caused by milk-alkali syndrome is increasing in frequency. CASE REPORT: A 26-year-old woman presented after having undergone caesarian section. She complained of severe myalgias, arthralgias, an inability to ambulate, nausea, vomiting, abdominal pain, and marked depression. Each of these symptoms has a broad differential diagnosis, but when considered together the theme "stones, bones, moans, and groans," seen in patients with hypercalcemia, is evident. This patient was f

Stressful Life Event Experiences of Pregnant Women in the United States: A Latent Class Analysis.

OBJECTIVES: Nearly 65% to 70% of pregnant women in the United States experience one or more stressful life events (SLEs), which can lead to adverse maternal and/or fetal outcomes. This study aimed to identify groups of women with similar patterns of antenatal SLE experiences, and to examine their sociodemographic correlates. METHODS: Data from the 2009 to 2011 Pregnancy Risk Assessment Monitoring System were used and latent class analysis performed (N = 115,704) to identify unobserved class membership. The relative likelihood of membership in each latent class was explored using multinomial l

Not identifying with postnatal depression: a qualitative study of women's postnatal symptoms of distress and need for support.

INTRODUCTION: Diagnoses and symptoms of postnatal mental health problems are associated with adverse outcomes for women and their families. Current classification and assessment of postnatal mental health problems may not adequately address the range or combination of emotional distress experienced by mothers. This study aims to explore symptoms of mental health problems reported by new mothers and their experiences of being assessed for these. METHODS: In-depth interviews with 17 women in southeast England with a baby under 1-year old who experienced a postnatal mental health problem. Data w

Working memory in pregnant women: Relation to estrogen and antepartum depression.

This article is part of a Special Issue "Estradiol and cognition". Subjective changes in concentration and memory are commonly reported by women during the second or third trimesters of pregnancy, but the nature of the problem is poorly understood. We hypothesized that these self-reports might reflect difficulties in working memory (WM). It was further hypothesized that antepartum depression (depression arising during pregnancy) may play an etiological role, either on its own or due to secondary changes in endocrine function or sleep. Using WM tasks that emphasized executive control processes

Under the shadow of maternity: birth, death and puerperal insanity in Victorian Britain.

Death and fear of death in cases of puerperal insanity can be linked to a much broader set of anxieties surrounding childbirth in Victorian Britain. Compared with other forms of mental affliction, puerperal insanity was known for its good prognosis, with many women recovering over the course of several months. Even so, a significant number of deaths were associated with the disorder, and a large proportion of sufferers struggled with urges to destroy their infants and themselves. The disorder evoked powerful delusions concerning death, with patients expressing intimations of mortality and long

Long-term maternal depression after expectant management of early preterm birth.

OBJECTIVE: This study was undertaken to determine factors that affect depression after delivery of infants at the limits of viability. We hypothesized that expectantly managed mothers would have lower risk of depression several years after delivery. STUDY DESIGN: Charts were reviewed for deliveries in the calendar years 2003 through 2005 with a diagnosis of preterm labor, premature rupture of membranes or cervical insufficiency admitted at a gestation between 20 and 25(6/7) weeks. Surveys were sent that included the Edinburgh Postnatal Depression Scale and questions regarding care around the

The treatment of post-partum thyroid disease.

INTRODUCTION: Bile cast nephropathy (BCN) is a rare and often overlooked cause of acute kidney injury (AKI), usually seen in the context of severe hyperbilirubinemia. It results from the deposition of bile pigments and bile casts in renal tubules, specifically distal and collector tubules, leading to obstruction and direct tubular toxicity. CASE DESCRIPTION: We describe the case of 49-year-old male who presented with severe jaundice and oliguric AKI. With the gradual resolution of liver dysfunction, the patient's kidney function also improved. However, during the recovery phase, the patient d

Mother-infant interaction in post-partum women with schizophrenia and affective disorders.

BACKGROUND: Psychiatric mother and baby units are increasingly asked to assess parenting in people with severe mental illness, particularly schizophrenia, but little research evidence exists on which to base assessments. METHOD: Mother-infant interaction was assessed in 26 women who had recovered from the acute phase of severe post-partum mental disorder, a validated rating scale based on direct observation was used. RESULTS: Women with schizophrenia showed greater interaction deficits than those with affective disorders, being more remote, insensitive, intrusive and self-absorbed. The 4-mon

Participation in alternative care: relationship to anxiety, depression, and hostility.

Women's psychological mood and its relationship to satisfaction with participatory perinatal care was examined using a two-group evaluation design of convenience samples (N = 99). Women entered the perinatal experience with similar degrees of anxiety, depression and hostility, but contrary to prediction, the type of perinatal experience, conventional or alternative, was not associated with their psychological mood. However, women in both groups reported less anxiety 2 to 3 days and 4 weeks after delivery than they reported having in general prior to delivery. Women who were more satisfied with

Gaussian process emulation to improve efficiency of computationally intensive multidisease models: a practical tutorial with adaptable R code.

BACKGROUND: The rapidly growing burden of non-communicable diseases (NCDs) among people living with HIV in sub-Saharan Africa (SSA) has expanded the number of multidisease models predicting future care needs and health system priorities. Usefulness of these models depends on their ability to replicate real-life data and be readily understood and applied by public health decision-makers; yet existing simulation models of HIV comorbidities are computationally expensive and require large numbers of parameters and long run times, which hinders their utility in resource-constrained settings. Predictors and consequences of long-term pregnancy-related pelvic girdle pain: a longitudinal follow-up study.

BACKGROUND: Pelvic girdle pain (PGP) is a multifactorial condition, which can be mentally and physically compromising both during and after pregnancy. However, long-term pregnancy-related PGP has been poorly investigated. This longitudinal follow-up study uniquely aimed to describe prevalence and predictors of PGP and its consequences on women's health and function up to 11 years after pregnancy. METHODS/DESIGN: A postal questionnaire was sent to 530 women who participated in 1 of 3 randomized controlled studies for PGP in pregnancy. Women who reported experiencing lumbopelvic pain were offer

Real-World Data Insights into Antidepressant Prescription and Adherence During Pregnancy in Catalonia (Spain).

BACKGROUND: Affective disorders, particularly depression, are common among women of childbearing age, and pregnancy often exacerbates symptoms. Antidepressants are often required for treatment, but adherence during pregnancy is variable. Although some studies suggest potential risks to the foetus, many cannot rule out confounding by indication. In this context, understanding real-world patterns of antidepressant prescription and adherence during pregnancy is essential to inform clinical practice and ensure adequate mental healthcare. OBJECTIVE: The aim of the present study was to characterise

Safety and Pharmacogenetics of Oxycodone in Post-Cesarean Analgesia and Breastfeeding Dyads: A Proactive Approach to Precision Medicine.

Background: The aim of the study is (1) to assess safety of opioids in nursing mothers after cesarean delivery and in breastfed infants and (2) to evaluate the role of CYP2D6 genetics in maternal and infant clinical outcomes after cesarean delivery. Methods: A total of 210 mother-infant dyads were enrolled after cesarean delivery. Oxycodone 5 mg orally was administered every 4-6 h as needed as part of a standardized opioid-sparing ERAS protocol. Primary outcomes were opioid-related adverse effects, including maternal respiratory depression (RD) and postoperative nausea and vomiting (PONV) and

Deficiency of the paternally-expressed imprinted Peg3 gene in mice has sexually dimorphic consequences for offspring communication and social behaviour.

INTRODUCTION: Imprinted genes are expressed from one parental allele as a consequence of epigenetic processes initiated in the germline. Consequently, their ability to influence phenotype depends on their parent-of-origin. Recent research suggests that the sex of the individual expressing the imprinted gene is also important. We have previously reported that genetically wildtype (WT) dams carrying and caring for pups mutant for PEG3 exhibit anxiety-like behaviours and their mutant pups show a reduction in ultrasonic vocalisation when separated from their mothers. Sex-specificity was not examin

Interventions for preventing or reducing domestic violence against pregnant women.

BACKGROUND: Domestic violence during pregnancy is a major public health concern. This preventable risk factor threatens both the mother and baby. Routine perinatal care visits offer opportunities for healthcare professionals to screen and refer abused women for effective interventions. It is, however, not clear which interventions best serve mothers during pregnancy and postpartum to ensure their safety. OBJECTIVES: To examine the effectiveness and safety of interventions in preventing or reducing domestic violence against pregnant women. SEARCH METHODS: We searched the Cochrane Pregnancy an

Evaluation of the birth plan implementation: a parallel convergent mixed study.

BACKGROUND: Pregnancy, birth, and motherhood are among the most important events of every woman's life. Training and participation of mothers in the decision-making process of delivery play an essential role in physical as well as psychosocial preparation of the mother. The healthcare system can improve and enhance the level of care by involving the patient in their self-care process. The aim of the present study is to assess the implementation of the birth plan for the first time in Iran in Tabriz city. METHODS/DESIGN: The present study uses a mixed-method with a parallel convergence approac

Mental health of mothers and fathers of twins conceived via assisted reproduction treatment: a 1-year prospective study.

BACKGROUND: Although twin deliveries after assisted reproduction treatment (ART) are common, the mental health of the parents has scarcely been addressed. Therefore, we evaluated the psychological well-being of ART and spontaneously conceiving parents of twins and singletons. Furthermore, the impact of parity and children's health-related factors on mental health was evaluated. METHODS: We conducted a prospective longitudinal questionnaire study among ART parents of 91 pairs of twins and of 367 singletons and on control parents of 20 pairs of twins and of 379 singletons in the 2nd trimester o
