Pathologic cervical burst fracture presenting with airway compromise.

Mechanical airway compromise following cervical spine injury or fracture is a rare but known entity. It most commonly is the result of the development of a retropharyngeal hematoma or prevertebral soft tissue edema that obstructs the airway, leading to respiratory distress and emergent need for airway management and possible surgical intervention. We present a novel case of airway compromise following a C3 burst fracture without associated retropharyngeal hematoma or prevertebral soft tissue edema. Surgical management is discussed, and a review of relevant literature is provided. Pathological

Functional outcome and complications following two types of dorsal plating for unstable fractures of the distal part of the radius.

BACKGROUND: There is a paucity of data in the literature documenting the functional outcomes for patients who have been managed with a dorsal plate because of a distal radial fracture. The purpose of the present study was to determine the functional outcome and complications following dorsal plating for dorsally displaced, unstable fractures of the distal part of the radius. METHODS: The records of all patients who had been managed at our institution with dorsal plating because of a comminuted, dorsally displaced fracture of the distal part of the radius were reviewed. Patients with less than

Treatment of chronic tibial osteomyelitis, segmental bone loss, and soft tissue defect by bone transplant.

New methods of treatment have greatly improved the success of limb salvage in severe lower extremity trauma. We report the use of a microvascular free tissue transfer and distraction osteogenesis in the reconstruction of soft tissue and bone defects complicated by infection. Soft tissue coverage and bony stabilization by external fixation resulted in eradication of the infection. The external fixator also allowed bone transport and regeneration to overcome a bone defect of approximately 6 cm. Bone transportation can be accomplished in a distal to proximal direction if the situation necessitate

Is the use of arch bars or interdental wire fixation necessary for successful outcomes in the open reduction and internal fixation of mandibular angle fractures?

PURPOSE: The purpose of this retrospective cohort study was to analyze the complications associated with a series of mandibular angle fractures treated by open reduction and internal fixation and to determine if the method of intraoperative maxillomandibular fixation (MMF) affected patient outcome. PATIENTS AND METHODS: The records of 162 consecutive patients with isolated mandibular fractures that were treated by the senior author (R.B.B.) with open reduction and internal fixation were retrospectively reviewed and a number of clinical variables were recorded. Of these, all patients with frac

Orbital Fracture Repair Materials: Research Hotspots, Clinical Applications, and Emerging Technologies.

Orbital fractures, particularly of the medial and inferior walls, often require precise reconstruction to restore orbital volume and prevent enophthalmos, diplopia, and facial deformity. The selection of repair material, ranging from titanium meshes and porous polyethylene to resorbable polymers and tissue-engineered scaffolds, is therefore central to surgical success, yet a clinically oriented synthesis of the available options remains lacking. This study combines a bibliometric mapping with a focused review of repair materials to chart how the field has evolved and to clarify the relationshi

Limb salvage using reverse sural cross-leg flap in complex lower limb injury: A case series.

BACKGROUND: Complex lower limb injuries involving soft tissue defects, fractures, and vascular compromise present significant reconstructive challenges. Free tissue transfer is often the gold standard for reconstruction; however, reverse sural cross-leg flaps provide a reliable alternative in resource-limited settings or when the recipient's vessels are compromised. This case series highlights the utility of this technique in challenging scenarios, including comminuted fractures, vascular injuries, and extensive soft tissue defects. METHODS: Six male patients with complex lower limb injuries

Reconstruction of complex foot injury using anterior thigh free flap with vastus lateralis muscle pedicle and tri-cortical Iliac crest bone graft: a case report.

INTRODUCTION AND IMPORTANCE: Complex foot injuries from high-velocity trauma pose significant challenges due to combined bony and soft tissue damage. Early and comprehensive reconstruction using an orthoplastic approach is critical for limb salvage and achieving functional recovery. PRESENTATION OF CASE: A 22-year-old male sustained a severe crush injury to the right foot following a motor vehicle accident. The injury included degloving, extensive soft tissue loss, and multiple fractures: a comminuted first metatarsal shaft fracture, a medial cuneiform bone defect, and minimally displaced fou

General versus Brachial Plexus Block Anesthesia in Pain Management after Internal Fixation in Patients with Distal Radius Fracture: A Randomized Controlled Trial.

Distal radius fractures (DRFs) are very common injuries associated with aging, and the number of fractures is increasing with the increase in the elderly population. General anesthesia or brachial plexus block (BPB) is required for fracture fixation, and acute postoperative pain control is necessary after operation. Early pain control can improve patient satisfaction and functional outcomes. In this study, we report the clinical differences in postoperative pain, according to the method of anesthesia (general anesthesia versus brachial plexus block). Volar plating was used to treat 72 patients

Atraumatic Knee Marrow Edema-Like Signal Preceding a Low-Energy Bicondylar Tibial Plateau Fracture in a Patient with Crohn Disease: A Case Report.

Bone marrow edema-like signal is a nonspecific magnetic resonance imaging finding whose clinical significance depends on its cause, distribution, associated structural abnormalities, symptoms, and skeletal risk. A 44-year-old construction engineer with Crohn disease and previous systemic corticosteroid exposure developed several weeks of atraumatic, load-related right-knee pain. Available prefracture radiographs showed no visible acute fracture. Noncontrast MRI on 28 July 2025 was reported as showing a medial femoral condyle marrow contusion without a fracture line or subchondral collapse; ho

Does Grafting Matter in Surgically Treated Calcaneal Fractures? A Retrospective Analysis.

OBJECTIVES: The role of bone grafting in the surgical treatment of displaced intra-articular calcaneal fractures (DIACFs) remains controversial. Although bone grafts are commonly used to restore joint congruity and support anatomical reduction, recent evidence favors minimally invasive approaches that may eliminate the need for routine grafting. This study aimed to evaluate the impact of bone graft use on radiological parameters and functional outcomes in Sanders Type II, III, and IV calcaneal fractures treated surgically. METHODS: This retrospective cohort study included 115 patients who und

Floating knee injuries - An analytical study using a conglomerated comprehensive classification.

PURPOSE: Floating knee injuries are complex high velocity injuries with a simultaneous breach of skeletal integrity both above and below the knee joint. Intraarticular fractures, severe comminution, soft tissue insult, associated other skeletal and systemic injuries have been the most common factors influencing the outcome. With our previous experience we framed a conglomerated comprehensive classification, which includes important prognosticating factors that influences the outcome. Aim of this study is to analyse the efficacy of this classification system for floating knee injury by studying

Prosthetic radial head components and proximal radial morphology: a mismatch.

A morphometric study of the proximal radius was performed with magnetic resonance imaging scans to measure the anatomic dimensions of the radial head and neck. These dimensions were then compared with the manufacturer's size specifications of commercially available titanium prosthetic radial head components to determine whether these designs adequately match the morphologic characteristics of the proximal radius. A cadaveric correlation was performed to validate the accuracy and reliability of measurements obtained from the magnetic resonance scans. The narrow intramedullary dimensions of the

A clinically applicable reporting system for the diagnosis of facial fractures.

A clinically applicable, comprehensive reporting system for the diagnosis of facial fractures was developed with three guiding principles: (1) preservation of classical anatomical terminology and nomenclature, (2) facilitation of uniform interpretation of radiographs between radiologists and (3) non-redundant diagnostic descriptions of complex fractures, in a manner that correlates with treatment modality. Twenty-two fracture types (17 simple fracture types and 5 complex fracture types) are included in the system. Each patient's fracture pattern is described by listing the component fractures
