Corticosteroids for periorbital and orbital cellulitis.

BACKGROUND: Periorbital and orbital cellulitis are infections of the tissue anterior and posterior to the orbital septum, respectively, and can be difficult to differentiate clinically. Periorbital cellulitis can also progress to become orbital cellulitis. Orbital cellulitis has a relatively high incidence in children and adults, and potentially serious consequences including vision loss, meningitis, and death. Complications occur in part due to inflammatory swelling from the infection creating a compartment syndrome within the bony orbit, leading to elevated ocular pressure and compression of

Surgical interventions for vertical strabismus in superior oblique palsy.

BACKGROUND: Superior oblique palsy is a common cause of vertical strabismus in adults and children. Patients may be symptomatic from binocular vertical diplopia or compensatory head tilt required to maintain single vision. Most patients who are symptomatic elect to undergo strabismus surgery, but the optimal surgical treatment for vertical strabismus in people with superior oblique palsy is unknown. OBJECTIVES: To assess the relative effects of surgical treatments compared with another surgical intervention, non-surgical intervention, or observation for vertical strabismus in people with supe

Intravitreal steroids versus observation for macular edema secondary to central retinal vein occlusion.

BACKGROUND: Central retinal vein occlusion (CRVO) is a common retinal vascular abnormality associated with conditions such as hypertension, diabetes, glaucoma, and a wide variety of hematologic disorders. Macular edema (ME) represents an important vision-threatening complication of CRVO. Intravitreal steroids (IVS), such as triamcinolone acetonide, have been utilized to treat macular edema stemming from a variety of etiologies and may be a treatment option for CRVO-ME. OBJECTIVES: To explore the effectiveness and safety of intravitreal steroids in the treatment of CRVO-ME. SEARCH METHODS: We

Aflibercept for neovascular age-related macular degeneration.

BACKGROUND: Central vision loss caused by age-related macular degeneration (AMD) is the leading cause of blindness among the elderly in developed countries. Neovascular AMD is characterized by choroidal neovascularization (CNV). Growth of new blood vessels in patients with neovascular AMD is driven by a complex process that involves a signal protein called vascular endothelial growth factor A (VEGF-A). Anti-VEGF drugs that block this protein include ranibizumab, bevacizumab, and aflibercept. OBJECTIVES: To assess and compare the effectiveness and safety of intravitreal injections of afliberce

Exploring Vitamin D Signaling-Associated Biomarkers and Their Diagnostic Value in Diabetic Retinopathy: A Combined Transcriptomic and Single-Cell Analysis With Experimental Validation.

BACKGROUND: Diabetic retinopathy (DR) can significantly impair vision and lead to blindness. Vitamin D (VD) has been shown to enhance the production of anti-inflammatory factors, alleviating the effects of hyperglycemia. However, downstream genes and molecular networks associated with VD signaling in DR remain unidentified. This study aimed to employ a systems biology approach to nominate high-priority candidate genes and cellular contexts as a hypothesis-generating effort to facilitate future functional studies on the role of VD in DR. METHODS: DR-related datasets were obtained from public d

Lactoferrin Alleviates Lipopolysaccharide-Induced Infantile Intestinal Immune Barrier Damage by Regulating an ELAVL1-Related Signaling Pathway.

As the most important intestinal mucosal barrier of the main body, the innate immune barrier in intestinal tract plays especially pivotal roles in the overall health conditions of infants and young children; therefore, how to strengthen the innate immune barrier is pivotal. A variety of bioactivities of lactoferrin (LF) has been widely proved, including alleviating enteritis and inhibiting colon cancer; however, the effects of LF on intestinal immune barrier in infants and young children are still unclear, and the specific mechanism on how LF inhibits infantile enteritis by regulating immune s

Medicinal plants as a potential source of Phosphodiesterase-5 inhibitors: A review.

ETHNOPHARMACOLOGICAL RELEVANCE: The prevalence and distress caused by erectile dysfunction (ED) to both male and female partners are increasing at a steady rate. ED has now become the most treated sexual disorder for men among young and old age groups due to varying physical and psychological factors. The treatment with synthetic Phosphodiesterase-5 (PDE5) inhibitors are cost-effective but due to adverse effects such as priapism, loss of vision, heart attack and syncope, the daily life patterns of these patients are distressed and hence the need for alternative medicaments or sources are of ut

Rituximab for thyroid-associated ophthalmopathy.

BACKGROUND: Thyroid associated ophthalmopathy (TAO) is the most frequent extrathyroidal manifestation of Graves' disease, affecting up to 50% of patients, and has a great impact on quality of life. Rituximab is a human/murine chimeric monoclonal antibody that targets CD20, a transmembrane protein expressed on the surface of pre-B and mature B lymphocytes, but not on stem cells, pro-B lymphocytes or plasma cells. Preliminary work has shown that blocking the CD20 receptor on B-lymphocytes with rituximab affects the clinical course of TAO, by reducing inflammation and the degree of proptosis. OB

Interventions for disorders of eye movement in patients with stroke.

BACKGROUND: Eye movement disorders may affect over 70% of stroke patients. These eye movement disorders can result in difficulty maintaining the normal ocular position and difficulty moving the eyes appropriately. The resulting functional disabilities include a loss of depth perception, reduced hand-to-eye co-ordination, marked difficulties with near tasks and reading and reduced ability to scan the visual environment. They can also impact on the effectiveness of rehabilitation therapy. There are a wide variety of different treatment interventions proposed for eye movement disorders after stro

Performance of Artificial Intelligence-Based Models for Epiretinal Membrane Diagnosis: A Systematic Review and Meta-Analysis.

TOPIC: Epiretinal membrane (ERM) can impair central vision by forming a pre-retinal fibrous layer on the inner retina. Artificial intelligence (AI)-based tools may streamline ERM diagnosis, but their overall performance and factors affecting accuracy require evaluation. CLINICAL RELEVANCE: With an aging population, ERM prevalence is expected to rise, placing increased demands on clinical resources. Early detection via AI models could expedite diagnosis, reduce subjective errors, and guide timely surgical intervention. This systematic review and meta-analysis evaluates the pooled diagnostic pe

Comparison of Smart Display Versus Laptop Platforms for an eHealth Intervention to Improve Functional Health for Older Adults With Multiple Chronic Conditions: Protocol for a Randomized Clinical Trial.

BACKGROUND: Maintaining functional health, or the ability to live independently, is a primary goal of individuals as they age, but most older adults develop chronic conditions that threaten this goal. Physical activity is a key aspect of self-care that can improve functional health, and digital interventions offering guidance on appropriate exercise can help. However, older adults with multiple morbidities may be unable to use a laptop or smartphone-based eHealth because poor vision, dexterity, mobility, or other physical challenges make typing or touch navigation difficult. A smart display pl

Cortical brain potentials in response to lower limb proprioceptive stimuli in young adults with probable developmental coordination disorder.

BACKGROUND: Proprioceptive deficits have been shown to underlie motor problems in individuals with a probable developmental coordination disorder (pDCD). Behavioral studies have employed response times to passive limb movement to evaluate proprioceptive function in individuals with pDCD. However, the underlying neural mechanisms involved in the cortical processing of proprioceptive input and its corresponding motor response are unclear. To address this issue, this study aims to investigate neuropsychological and neurophysiological performances using event-related potentials (ERP) on propriocep

Botulinum toxin type A versus anticholinergics for cervical dystonia.

BACKGROUND: This is an update of a Cochrane Review first published in 2005. Cervical dystonia is the most common form of focal dystonia and is a highly disabling movement disorder, characterised by involuntary, usually painful, head posturing. Currently, botulinum toxin type A (BtA) is considered the first line therapy for this condition. Before BtA, anticholinergics were the most widely accepted treatment. OBJECTIVES: To compare the efficacy, safety, and tolerability of BtA versus anticholinergic drugs in adults with cervical dystonia. SEARCH METHODS: We searched the Cochrane Movement Disor

Complement inhibitors for age-related macular degeneration.

BACKGROUND: Given the relatively high prevalence of age-related macular degeneration (AMD) and the increased incidence of AMD as populations age, the results of trials of novel treatments are awaited with much anticipation. The complement cascade describes a series of proteolytic reactions occurring throughout the body that generate proteins with a variety of roles including the initiation and promotion of immune reactions against foreign materials or micro-organisms. The complement cascade is normally tightly regulated, but much evidence implicates complement overactivity in AMD and so it is

Music education for improving reading skills in children and adolescents with dyslexia.

BACKGROUND: Dyslexia (or developmental dyslexia or specific reading disability) is a specific learning disorder that has a neurobiological origin. It is marked by difficulties with accurate or fluent recognition of words and poor spelling in people who have average or above average intelligence and these difficulties cannot be attributed to another cause, for example, poor vision, hearing difficulty, or lack of socio-environmental opportunities, motivation, or adequate instruction. Studies have correlated reading skills with musical abilities. It has been hypothesized that musical training may
