Identification of co-diagnostic effect genes for aortic dissection and metabolic syndrome by multiple machine learning algorithms.

Aortic dissection (AD) is a life-threatening condition in which the inner layer of the aorta tears. It has been reported that metabolic syndrome (MS) has a close linkage with aortic dissection. However, the inter-relational mechanisms between them were still unclear. This article explored the hub gene signatures and potential molecular mechanisms in AD and MS. We obtained five bulk RNA-seq datasets of AD, one single cell RNA-seq (scRNA-seq) dataset of ascending thoracic aortic aneurysm (ATAA), and one bulk RNA-seq dataset of MS from the gene expression omnibus (GEO) database. Identification of

Progress in the treatment of aneurysms of the distal aortic arch: approach through median sternotomy.

AIM: A review of past and current operative procedures for the treatment of aneurysms of the distal aortic arch is presented in conjunction with a series of 43 patients. In this study, distal aortic arch aneurysm refers to an aneurysm involving at least the origin of the left subclavian artery, but not extending beyond the left common carotid artery. We excluded dissection aneurysm and extended aneurysm to the descending thoracic aorta from this study. METHODS: Between January, 1985, and March, 2000, 43 consecutive patients (37 males, 6 females; mean age 67.5 years) underwent repair of aneury

Epicardial neural ganglionated plexus of ovine heart: anatomic basis for experimental cardiac electrophysiology and nerve protective cardiac surgery.

BACKGROUND: Sheep are routinely used in experimental cardiac electrophysiology and surgery. OBJECTIVE: The purpose of this study was to (1) ascertain the topography and architecture of the ovine epicardial neural plexus (ENP), (2) determine the relationships of ENP with vagal and sympathetic cardiac nerves and ganglia, and (3) evaluate gross anatomic differences and similarities of ENP in humans, sheep, and other species. METHODS: Ovine ENP and extrinsic sympathetic and vagal nerves were stained histochemically for acetylcholinesterase in whole heart and/or thorax-dissected preparations from

A large French family with TGFBR2 pathogenic variant: illustration of variability.

AIMS: To report aortic events in a large family carrying a variant in the TGFBR2 gene. METHODS: Since 1990 up to 2024, we have conducted a longitudinal clinical study of a large single family comprising 63 members across four generations who carry the same TGFBR2 pathogenic variant. We assessed the incidence of aortic events and prophylactic surgery, as well as life expectancy. RESULTS: Over a follow-up, 21 patients died (33% of the population), of whom ten were related to a dissection (48%). Eight patients underwent prophylactic aortic root surgery (13%). Over the generations, there is an i

Risk of Aortic Dissection in the Moderately Dilated Ascending Aorta.

BACKGROUND: Recent studies have demonstrated that many patients with acute type A aortic dissection (AD) have aortic diameters of <55 mm at presentation, prompting discussion of lowering the prophylactic surgical guidelines. However, risk of dissection at these smaller diameters is poorly defined. OBJECTIVES: The purpose of this study is to understand the risk of AD in moderately dilated ascending aortas using a large echocardiographic data set. METHODS: Using an institutional echocardiography database, we identified 4,654 nonsyndromic adults (age: 68.6 ± 13.1 years; 1,003 women) with maxima

Effects of statin therapy on ascending aorta aneurysms growth: A propensity-matched analysis.

BACKGROUND: Pleiotropic effects of statins have been advocated for remodeling of the vascular wall. The aim of the present study was to investigate whether statin therapy influences the growth rate of ascending aorta (AA) diameter. METHODS: A total of 1348 patients was referred to our outpatient clinic for initial AA ectasia from September 2005 to December 2011. A propensity score was built to perfectly match (1:1) patients administered (Group A) or not (Group B) with statin therapy. Clinical and echocardiographic follow-up was 100% completed at 3 years after the first visit. Treatment groups

A best evidence topic in cardiac surgery was written according to a structured protocol. The question addressed was 'Which cannulation (ascending aortic cannulation or peripheral arterial cannulation) is better for acute type A aortic dissection surgery?' Altogether 393 papers were found using the reported search, of which 14 represented the best evidence to answer the clinical question. The authors, journal, date and country of publication, patient group studied, study type, relevant outcomes and results of these papers are tabulated. Femoral artery cannulation has the highest rate of mortali

Does recurrent laryngeal nerve anatomy condition the choice of the side for approaching the anterior cervical spine?

AIM: The anterior cervical presternocleidomastoid approach is a safe and standardized procedure. Nevertheless, in all the largest series presented in literature, a certain risk of injury to the recurrent laryngeal nerves (RLNs) is reported and the choice of the side of the approach seems to influence such risk. Our aim was to study the surgical anatomy of the RLNs and to understand whether the side of the approach can be a risk factor for their surgical damage. METHODS: We performed an anatomical dissection of 6 fresh cadavers, studying the origin and the course of both the RLNs, to assess th

[Total arch replacement with reconstruction of non-coronary sinus for acute type A aortic dissection after previous coronary artery bypass grafting; report of a case].

A 73-year-old man, who underwent coronary artery bypass grafting (CABG) 2 years previously, had suffered from acute type A aortic dissection with concomitant mild to moderate aortic regurgitation. A median resternotomy was performed. Arterial and venous cannulae were inserted into the femoral artery and vein to institute a cardiopulmonary bypass. The intimal tear was existed just above the sino-tubular junction. As the non-coronary sinus was involved in the dissection severely, it was resected in a scallop-shaped configuration. The aortic graft was tailored accordingly and anastomosis was perf

[Preservation of the aortic valve in acute dissection of the ascending aorta].

The aim of this study was to evaluate the status of the native aortic valve in patients operated for acute dissection of the ascending. aorta (ADAA). Between November 1972 and November 1991, 93 patients were operated for ADAA. There were 76 men and 17 women (average age 54 +/- 12 years). The aortic valve was reserved in 80 cases (86%). In 13 patients (14%) aortic valve replacement was associated with replacement of the ascending aorta. The early mortality was 29% (27/93). The global actuarial survival rates at 5, 10 and 15 years were 60.2 +/- 5.2%, 49.7 +/- 6.1% and 26.9 +/- 9.9% respectively.

Lactate promotes macrophage-derived foam cell formation and atherosclerosis.

BACKGROUND AND AIMS: Diabetes accelerates atherosclerosis progression by disrupting multiple metabolic pathways. However, the role and mechanisms of lactate-a glycolytic byproduct significantly elevated in diabetic patients-in atherosclerosis remain poorly understood. We hypothesized that lactate promotes atherosclerosis by regulating macrophage foam cell formation through histone lactylation. METHODS: We evaluated the association between lactate levels, lipid metabolism disorders, and atherosclerotic progression using clinical samples and mouse models. The thoracic aortas of atherosclerotic

The Aortic Root: Natural History After Root-Sparing Ascending Replacement in Nonsyndromic Aneurysmal Patients.

BACKGROUND: Leaving native aortic tissue in situ in root-sparing ascending aortic replacement raises concern regarding potential later need for root reoperation or for the potential occurrence of localized dissections or rupture in the residual root. The purpose of this study was to evaluate the natural growth of the aortic root after root-sparing aortic replacement. METHODS: In all, 102 consecutive patients (mean age 61.8 ± 12.5 years; 60% male) who had undergone root-sparing aortic replacement had sufficient retrievable information regarding their aortic root diameter at postoperative basel

Progress in treatment of thoracoabdominal and abdominal aortic aneurysms involving celiac, superior mesenteric, and renal arteries.

This is a report of surgical treatment of thoracoabdominal aortic aneurysms and aneurysms of the abdominal aorta from which the visceral vessels arise during the 18 year period from April 5, 1960, to April 20, 1978. The extent of aneurysm is divided into five groups. Group I (10 patients) involved most of the thoracic and abdominal aorta down to celiac axis. Group II (22 patients) involved most of the thoracic and abdominal aorta distal to left subclavian artery. Group III (20 patients) were those with lesser involvement of the thoracic aorta and most of the abdominal aorta. Group IV (18 patie
