Diastasis with low distal fibula fractures: an anatomic rationale.

Tibiofibular diastasis occurs infrequently in displaced ankle fractures that include a low fibula fracture. This is because the interosseous ligament, the major ligament resisting diastasis, lies above the level of the fracture. The anatomic study of the syndesmotic complex of the ankle reported here showed that the interosseous ligament has a variable attachment on the fibula, differing between specimens in its distance above the synovial reflection or joint line. Although most specimens had attachments approximately 1 cm or greater above the joint line, one of 18 specimens attached just abov

Biomechanical evaluation of novel construct for stabilizing subcapital femoral neck fractures: a synthetic bone model.

BACKGROUND: High complication rates continue to be reported for femoral neck fractures, where moderate to severe shortening has been associated with pain and inferior functional outcomes. The objective was to evaluate the biomechanical performance of a novel femoral neck fracture fixation device by comparing to a clinically relevant construct using commercially available cannulated screws. METHODS: Twelve Sawbones femurs were equally divided into two groups where Group I was instrumented with a new fixation system consisting of two partially threaded cannulated screws and a cross-screw and Gro

Risk Factors of Diaphyseal Femoral Nonunions: A Systematic Review and Meta-analysis.

BACKGROUND: Intramedullary nailing of diaphyseal femur fractures often results in reliable union, with union rates ranging from 96% to 100%. However, femoral nonunions (FNUs) can be a debilitating clinical condition for patients. The purpose of this review was to identify and characterize the predictors of nonunion after femoral midshaft fractures. METHODS: A systematic search was conducted using key terms "femoral shaft non-union," "femoral diaphyseal non-union," "femoral shaft fracture," "femoral diaphysis fracture," and "femoral midshaft fracture" published before June 2024. Human studies

[Surgical Treatment of Calcaneus Fractures with C-Nail].

PURPOSE OF THE STUDY The calcaneus bone is the largest tarsal bone of a complex shape, the restoration of which after fracturing, often caused by a high-energy injury, is critical. The top priority in treating these fractures is to correctly asses the condition of the surrounding soft tissues that may be further excessively traumatized by inappropriate timing, surgical approach or technique. Even when adhering to all the rules and guidelines, complications in surgical wound healing have been described in up to 16-33% cases when the extended lateral approach was used. Therefore, the development

Effect of Distal Fragment Length on Construct Stability in an Extra-articular Distal Tibial Fracture Model Fixed With Locked Intramedullary Nailing: A Biomechanical Study.

Introduction  Fractures of the distal tibia are complex injuries with high complication rates, which include delayed union, non-union, and wound complications like dehiscence and infection. The two commonly employed definite internal fixation modalities include locked intramedullary (IM) nailing and plating. There is controversy regarding the superiority of the fixation construct, although nailing is proven to be more biological and devoid of soft tissue complications. There is also no consensus regarding the minimum distance of the fracture from the tibial plafond that is amenable to nailing

Effect of Radial Head Implant Shape on Radiocapitellar Joint Congruency.

PURPOSE: Radial head arthroplasty is indicated in displaced fractures in which comminution precludes successful internal fixation. Many types of radial head implants have been developed varying in material, methods of fixation, and degrees of modularity and geometry. The purpose of this study was to investigate the effect of radial head implant shape on radiocapitellar joint congruency. METHODS: Joint congruency was quantified in 7 cadaveric specimens employing a registration and inter-surface distance algorithm and 3-dimensional models obtained using computed tomography. Forearm rotation was

"Does advanced imaging improve the reliability of 2018 AO/OTA classification for trochanteric femoral fractures?".

BACKGROUND: The influence of advanced imaging modalities on the reliability of 2018 AO/OTA classification and detection of features contributing to fracture instability have not been adequately studied in the literature. METHODS: This prospective study was conducted to assess the reliability of 2018 AO/OTA classification for trochanteric femoral fractures, and features of instability in 50 patients using radiographs, multiplanar reformats (MPR), and 3D-reconstruction with fracture segmentation (3DR-FS) by 3 observers on 2 occasions at an interval of 4 weeks. RESULTS: Mean inter-observer reli

A 3D Printed Anatomically Pre-Contoured Plate for the Treatment of Y-T Humeral Condylar Fractures: A Feline Cadaveric Study.

(1) Background: Anatomically pre-contoured plates usually require only minimal or even no intraoperative contouring. For complex cases, such plates also assist the surgeon as an anatomical template during fracture reduction. In this study, we present our experience of using a 3D printing technology for the treatment of bicondylar humeral fractures in feline cadavers. (2) Methods: Surgeries were performed on 15 pairs of front limbs amputated at the scapula. The limbs were obtained from 15 adult cats without obvious pathology of the skeleton. After flexion of the elbow and subperiosteal elevatio

A biomechanical comparison of trochanteric nail proximal screw configurations in a subtrochanteric fracture model.

OBJECTIVES: Historically, because of the magnitude of muscle forces exerted locally, as well as the commonly associated comminution, subtrochanteric fractures have been difficult to treat. Tencer et al found intramedullary nail fixation to be superior to lateral plate constructs in axial compression and combined bending. In addition, reconstruction-type intramedullary nails of more recent design have been shown to provide strength and stiffness superior to that supplied by the earlier antegrade intramedullary implants. A relatively new reconstruction nail, the DePuy VersaNail Troch Entry Nail

Total elbow arthroplasty as primary treatment for distal humeral fractures in elderly patients.

We retrospectively reviewed the results of primary total elbow arthroplasty for the treatment of an acute fracture of the distal aspect of the humerus in twenty consecutive patients (twenty-one elbows) who had a mean age of seventy-two years (range, forty-eight to ninety-two years) at the time of the injury. The patients were managed between November 1982 and October 1992. The presence of rheumatoid arthritis in nine patients (ten elbows) influenced the choice of treatment. The mean interval between the injury and the total elbow arthroplasty was seven days (range, one to twenty-five days). Th

Fracture mapping of coronal shear fractures of the distal humerus: morphological analysis and fixation efficacy.

BACKGROUND: Coronal shear fractures of the distal humerus exhibit complex morphology, presenting substantial challenges in clinical management. Fracture mapping technology provides a visual tool for elucidating fracture patterns. This study aimed to: (1) generate three-dimensional (3D) fracture line maps and heatmaps to identify fracture-prone regions and characterize morphological features; and (2) assess the suitability of pre-contoured locking plating systems for screw placement. METHODS: A retrospective analysis was conducted on 135 coronal shear fractures of the distal humerus treated at

A randomised clinical study comparing palmar and dorsal fixed-angle plates for the internal fixation of AO C-type fractures of the distal radius in the elderly.

Current surgical treatments for distal radial fractures include dorsal and palmar plate fixation. We report results of a randomised study comparing these methods for AO C1-3 fractures. The emphasis was placed on the early postoperative functional recovery within the first 6 months as this interval is of decisive importance for elderly patients. Thirty patients with unilateral AO C1-3 fractures were enroled, 15 were treated with a palmar plate and 15 received a dorsal Pi-plate. Results were assessed 6 weeks, 3 months and 6 months postoperatively focusing on functional recovery. The palmar plate

Surgical treatment of fractures of the thoracic and lumbar spine via the transpedicular route.

Sixty-seven consecutive patients with burst or dislocation fractures of the thoracic or lumbar spine were submitted to early surgical reduction, via the transpedicular route, over a 5-year period. The first 22 patients received Harrington instrumentation, while transpedicular devices were applied in the last 44 cases, at either the thoracic or the lumbar level. One patient did not receive any spinal instrumentation. This surgical approach was found to be reliable in achieving a near-anatomical reconstruction of the fractured spinal segment. The rate of post-operative complications was low. Pla

Regional Anaesthesia for Tibial Fracture Surgery in the Third Trimester of Pregnancy.

PRESENTATION: A 27-year-old woman at 33 weeks' gestation presented with severe pain and swelling in her lower leg following a fall from a standing height at home. DIAGNOSIS: Imaging confirmed a severely comminuted tibial fracture. Given pregnancy-related concerns and historical debate regarding peripheral nerve blockade masking acute compartment syndrome (ACS), careful perioperative planning was undertaken with multidisciplinary input. TREATMENT: The patient elected for regional anaesthesia following shared decision-making. Intraoperative anaesthesia was achieved with a single-shot spinal an

[Is ultrasound equal to X-ray in pediatric fracture diagnosis?].

PURPOSE: Ultrasound is currently not established for the diagnosis of fractures. The aim of this study was to compare ultrasound and X-ray beyond their use solely for the identification of fractures, i. e., for the detection of fracture type and dislocation for pediatric fracture diagnosis. MATERIALS AND METHODS: Limb bones of dead young pigs served as a model for pediatric bones. The fractured bones were examined with ultrasound, X-ray, and CT, which served as the gold standard. RESULTS: 162 of 248 bones were fractured. 130 fractures were identified using ultrasound, and 148 using X-ray. Th
