Electrical storm reversible by isoproterenol infusion in a striking case of early repolarization.

A 40-year-old woman was referred to intensive care unit after recurrent ventricular fibrillation. She was free of cardiac medical history or medications. The resting ECG displayed an extended early repolarization in the inferior leads and all the precordial leads. Incessant ventricular fibrillations justified a treatment by intravenous amiodarone associated with general anaesthesia and mechanical ventilation without success on ventricular fibrillation. Because of a low heart rate intravenous isoproterenol infusion was initiated. Isoproterenol infusion was associated with heart rate acceleratio

Neurocognitive function in an extended Afrikaner-ancestry family with affective illness.

OBJECTIVE: To characterize the neuropsychological profile of an extended family with unipolar depression (UPD) and other forms of affective illness. METHOD: We administered a battery of neuropsychological tasks measuring various aspects of executive function and visual and verbal memory to 49 individuals in 1 extended family. Six participants had 1 lifetime episode of major depression (MDE-S), 15 were diagnosed with recurrent major depression (MDE-R), 11 had another DSM-IV diagnosis and 17 subjects were unaffected. RESULTS: After controlling for multiple confounding factors, including mood a

[Trichloroethylene intoxication presenting with temporal seizures].

INTRODUCTION: Trichloroethylene (TCE) is extensively used as a degreasing agent in the metal industry and as a solvent for organic compounds. Its neurotoxicity (with respect to both the peripheral and central nervous systems) is now widely acknowledged. OBSERVATION: Here, we report the case of a 32-year-old male drug addict presenting temporal seizures after "huffing" TCE (i.e. voluntary inhalation). The patient also deve loped a "psycho-organic syndrome" which included cognitive dysfunction (with memory disorders in particular) and personality changes. These disorders had not been noticed by

A novel mutation (Thr116Ile) in the presenilin 1 gene in a patient with early-onset Alzheimer's disease.

We report a novel presenilin 1 (PSN1) mutation (Thr116Ile) in a woman with early onset Alzheimer's disease (AD). This mutation was not found in 100 healthy controls, indicating that this is not a common polymorphism. The patient presented with forgetfulness at age 45, followed over the next 3 years by a worsening of the memory loss and frequent episodes of confusion and spatial disorientation. Neuroimaging studies were consistent with AD. The analysis of the family's pedigree showed that the proband was apparently the only member affected. Because the early death of several close relatives (i.

Cocaine's history, especially the American experience.

The history of cocaine in America can be traced to the late 19th century. After the discovery of its physiological and psychological effects, cocaine figured in consumables as diverse as hay fever remedies, local anaesthetics and soft drinks. The development of its different usages as well as eventual control of its use through restrictive legislation followed a different pattern in America from that in Europe. In the United States, national laws to control drugs faced constitutional obstacles until the era of World War I. Initially acclaimed as an ideal tonic, within two decades of its introd

Transient global amnesia: a case-control study.

The clinical findings and laboratory results from 18 patients with clear-cut transient global amnesia (TGA) were compared with the results from 90 nonischemic neurological control patients using case-control analysis. A prior episode of cerebral ischemia, generally in the posterior circulation, was the most significant risk factor for TGA. Other risk factors for cerebrovascular disease were more prominent in the TGA group, but this association became less significant when cerebral ischemia was controlled for. Migraine, epilepsy, and psychiatric disorders were not significantly associated with

Controlled and Real-Life Investigation of Optical Tracking Sensors in Smart Glasses for Monitoring Eating Behavior Using Deep Learning: Cross-Sectional Study.

BACKGROUND: The increasing prevalence of obesity necessitates innovative approaches to better understand this health crisis, particularly given its strong connection to chronic diseases such as diabetes, cancer, and cardiovascular conditions. Monitoring dietary behavior is crucial for designing effective interventions that help decrease obesity prevalence and promote healthy lifestyles. However, traditional dietary tracking methods are limited by participant burden and recall bias. Exploring microlevel eating activities, such as meal duration and chewing frequency, in addition to eating episod

VORTIOXETINE - THE NEW ANTIDEPRESSANT AGENT WITH PROCOGNITIVE PROPERTIES.

Depression symptoms resulting from cognitive function impairment are emphasized by both DSM-5 and ICD-10 diagnostic criteria for major depressive disorder and depressive episodes. Nonetheless, the role of cognitive dysfunctions seem to remain underestimated in case of depressive disorders, thus they are rarely perceived as therapeutic target. Vortioxetine is a relatively new, multi-functional agent. With its unique properties and strong affinity towards serotonin transporter (5-HTT), vortioxetine is a modulator and stimulator of serotonergic transmission. Vortioxetine is an antidepressant drug

Immunological characterisation of an unmasking TB-IRIS case.

BACKGROUND: Tuberculosis-associated immune reconstitution inflammatory syndrome (TB-IRIS) is an early complication of combination antiretroviral therapy (cART). Two forms are recognised: (i) paradoxical - recurrent or new TB symptoms develop after cART initiation in patients receiving TB treatment prior to cART; and (ii) unmasking TB-IRIS - active TB presents within 3 months of cART in patients not receiving TB treatment at cART initiation. The latter has heightened clinical manifestations and a marked inflammatory presentation. AIM: To gain insight into the immune pathogenesis of a case of u

Divergent cortical-subcortical intrinsic neural timescales in first-episode drug-naïve or minimally treated schizophrenia.

BACKGROUND: The neuropathological mechanisms of schizophrenia remain unclear. Intrinsic neural timescale (INT) reflects the temporal organization of neural activity. This study aimed to characterize INT alterations in patients with first-episode schizophrenia who were drug-naïve or minimally treated, while minimizing confounding effects of chronic illness and medication exposure. METHODS: Resting-state fMRI data were collected from 123 first-episode, drug-naïve or minimally treated schizophrenia patients and 102 matched healthy controls. INT values were derived from the autocorrelation functi

Why is vestibular migraine associated with many comorbidities?

Vestibular migraine (VM) is a usual trigger of episodic vertigo. Patients with VM often experience spinning, shaking, or unsteady sensations, which are usually also accompanied by photophobia, phonophobia, motor intolerance, and more. VM is often associated with a number of comorbidities. Recurrent episodes of VM can affect the patient's emotions, sleep, and cognitive functioning to varying degrees. Patients with VM may be accompanied by adverse moods such as anxiety, fear, and depression, which can gradually develop into anxiety disorders or depressive disorders. Sleep disorders are also a co

The temporal dynamics of dissociation: protocol for an ecological momentary assessment and laboratory study in a transdiagnostic sample.

BACKGROUND: Dissociation is a ubiquitous clinical phenomenon. Dissociative disorders (DD) are primarily characterized by dissociation, and dissociative states are also a criterion for borderline personality disorder (BPD) and the dissociative subtype of post-traumatic stress disorder (PTSD). Dissociative reactions (e.g., depersonalization/derealization or gaps in awareness/memory) across diagnostic categories are believed to be affect contingent and theorized to serve affect regulation functions. What is not clear, however, is how self-reported affect and physiological reactivity unfold within

Acute otitis media caused by Streptococcus pneumoniae serotype 19A ST320 clone: epidemiological and clinical characteristics.

BACKGROUND: Streptococcus pneumoniae serotype 19A ST320, a highly multiresistant and virulent clone, has emerged as a common pathogen causing acute otitis media (AOM) in children. METHODS: Patients aged 0-18 years with AOM who presented at Mackay Memorial Hospital, Taipei, Taiwan were prospectively enrolled between December 1, 2009, and November 30, 2012. For each patient, a specimen of middle-ear fluid was obtained and cultured. S. pneumoniae isolates were tested by serotyping, antibiotic-resistance profiling, and multilocus sequence typing. Demographic characteristics and clinical history o

Reduction in defibrillator shocks with an implantable device combining antitachycardia pacing and shock therapy.

Implantable defibrillators reduce the risk of sudden death in patients with malignant ventricular arrhythmias, but significant restriction in quality of life can occur as a result of frequent device activation. To determine if a device that provides both antitachycardia pacing and shock therapy can safely reduce the frequency of shocks after implantation, 46 consecutive patients undergoing initial implantation of a defibrillator were studied. In all patients, the implanted device provided antitachycardia pacing and shock therapy. Detected tachycardia characteristics and the results of therapy

Continuous bed-side monitoring of the ECG for detection of silent myocardial ischaemia.

In patients with unstable angina pectoris, transient episodes of ischaemia occur frequently and the prognostic implications of those predominantly silent episodes have been shown. The method of Holter tape recording is, however, not ideal for the care of unstable patients: (a) the results of a recording period are only available after this period is finished and subsequent evaluation has taken place; and (b) the sensitivity of monitoring may be increased by recording more than two leads. We have used a portable ECG computer, which monitors the 12-lead ECG every 20 s and records ECGs either whe

Micro-RNA profiles of pathology and resilience in posterior cingulate cortex of cognitively intact elders.

The posterior cingulate cortex (PCC) is a key hub of the default mode network underlying autobiographical memory retrieval, which falters early in the progression of Alzheimer's disease (AD). We recently performed RNA sequencing of post-mortem PCC tissue samples from 26 elderly Rush Religious Orders Study participants who came to autopsy with an ante-mortem diagnosis of no cognitive impairment but who collectively displayed a range of Braak I-IV neurofibrillary tangle stages. Notably, cognitively unimpaired subjects displaying high Braak stages may represent cognitive resilience to AD patholog

Activities of daily living measured by the Harvard Automated Phone Task track with cognitive decline over time in non-demented elderly.

BACKGROUND: Impairment in activities of daily living is a major burden to both patients and caregivers. Mild impairment in instrumental activities of daily living is often seen at the stage of mild cognitive impairment. The field of Alzheimer's disease is moving toward earlier diagnosis and intervention and more sensitive and ecologically valid assessments of instrumental or complex activities of daily living are needed. The Harvard Automated Phone Task, a novel performance-based activities of daily living instrument, has the potential to fill this gap. OBJECTIVE: To further validate the Harv

The Feasibility of At-Home iPad Cognitive Testing For Use in Clinical Trials.

BACKGROUND: Technological advances now make it feasible to administer cognitive assessments at-home on mobile and touch-screen devices such as an iPad or tablet computer. Validation of these techniques is necessary to assess their utility in clinical trials. OBJECTIVES: We used a Computerized Cognitive Composite for Preclinical Alzheimer's Disease (C3-PAD) developed for iPad 1) to determine the feasibility of performing the C3-PAD at home by older individuals without the presence of a trained psychometrician; 2) to explore the reliability of in-clinic compared to at-home C3-PAD performance an
