Machine learning-causal inference based on multi-omics data reveals the association of altered gut bacteria and bile acid metabolism with neonatal jaundice.

Early identification of neonatal jaundice (NJ) appears to be essential to avoid bilirubin encephalopathy and neurological sequelae. The interaction between gut microbiota and metabolites plays an important role in early life. It is unclear whether the composition of the gut microbiota and metabolites can be used as an early indicator of NJ or to aid clinical decision-making. This study involved a total of 196 neonates and conducted two rounds of "discovery-validation" research on the gut microbiome-metabolome. It utilized methods of machine learning, causal inference, and clinical prediction m

A multi-center, multi-organ, multi-omic prediction model for treatment-induced severe oral mucositis in nasopharyngeal carcinoma.

PURPOSE: Oral mucositis (OM) is one of the most prevalent and crippling treatment-related toxicities experienced by nasopharyngeal carcinoma (NPC) patients receiving radiotherapy (RT), posing a tremendous adverse impact on quality of life. This multi-center study aimed to develop and externally validate a multi-omic prediction model for severe OM. METHODS: Four hundred and sixty-four histologically confirmed NPC patients were retrospectively recruited from two public hospitals in Hong Kong. Model development was conducted on one institution (n = 363), and the other was reserved for external v

Current perspectives and future horizons in predicting response to nutrition therapy in children with Crohn's disease - a narrative review.

Exclusive enteral nutrition (EEN) and partial enteral nutrition (PEN) alongside Crohn's Disease Exclusion Diet (CDED) are evidence-based treatment strategies for children with Crohn's disease. However, the remission rates induced through nutritional therapy vary depending on the research design, treatment population, and outcome definition. The growing body of evidence highlights the necessity of a transitioning from "one-treatment-suits-all" strategy towards "tailor-made" precision-based therapeutic approach in children with Crohn's disease to obtain the optimal treatment response. Precise pe

Single-cell atlas of AML reveals age-related gene regulatory networks in t(8;21) AML.

Acute myeloid leukemia (AML) is characterized by cellular and genetic heterogeneity, which correlates with clinical course. Although single-cell RNA sequencing (scRNA-seq) reflects this diversity to some extent, the low sample numbers in individual studies limit the analytic potential when comparing specific patient groups. We performed large-scale integration of published scRNA-seq datasets to create a unique single-cell transcriptomic atlas for AML (AML scAtlas), totaling 748,679 cells, from 159 AML patients and 51 healthy donors from 20 different studies. This is the largest single-cell dat

A Community Standard Multispecies Cell Atlas of the Basal Ganglia.

The NIH BRAIN Initiative Cell Atlas Network (BICAN) aims to generate a standardized, integrated cell atlas of the human, macaque, marmoset, and mouse brain that serves as a foundational community reference for the classification and study of brain cell types. Here we present the first major component of this effort: a cross-species, multimodal atlas of the basal ganglia, a group of subcortical nuclei central to motor control and implicated in a broad range of neurological disorders. Grounded in large-scale single-cell transcriptomic classification and integrated with epigenomic and spatial gen

Mechanisms and Risk Factors of Cognitive Impairment in COPD.

TOPIC IMPORTANCE: Cognitive impairment (CI) is an underrecognized extrapulmonary complication of COPD. Emerging evidence suggests that CI in COPD is both highly prevalent and clinically significant, with detrimental effects on disease management, medication adherence, exacerbation risk, and quality of life. The underlying biological mechanisms contributing to CI in COPD remain poorly defined. REVIEW FINDINGS: Several plausible mechanisms have been implicated. For example, chronic hypoxemia has been associated with cerebral metabolic dysfunction and structural brain changes. Sleep-disordered b

Conserved Metastatic Cell States and Spatial Immune Microenvironment Remodeling Define Pancreatic Cancer Liver Metastases.

BACKGROUND: Pancreatic ductal adenocarcinoma (PDAC) mortality is driven largely by liver metastatic disease; however, the malignant cell states and tumor microenvironmental features of PDAC liver metastases remain obscure. METHODS: We developed a transplant model system of matched pancreatic and liver tumors to study PDAC metastatic progression. Using this model, we identified murine PDAC cell lines with distinct liver metastatic capacities and performed multiomic profiling of matched primary pancreatic and metastatic liver tumors. Refractory Celiac Disease: Nutritional Failure, Immune Dysregulation, and Lymphomagenesis.

Refractory celiac disease (RCeD) is a rare but severe complication of celiac disease characterized by persistent or recurrent malabsorptive symptoms and villous atrophy despite a strict gluten-free diet, after exclusion of ongoing gluten exposure, alternative enteropathies, and overt lymphoma. RCeD comprises two biologically distinct entities. RCeD-I is associated with phenotypically normal, polyclonal intraepithelial lymphocytes and generally reflects persistent gluten-independent mucosal inflammation with a relatively favorable prognosis. RCeD-II is defined by expansion of aberrant clonal in

Integrating Next-Generation Sequencing into von Willebrand Disease Diagnostics: Insights from the PCM-EVW-ES Multicenter Project.

Von Willebrand disease (VWD) is the most common inherited bleeding disorder, caused by quantitative or qualitative defects in von Willebrand factor (VWF). Diagnosis is challenging and requires integrating bleeding history, VWF antigen and activity measurements, FVIII assays, and specialized phenotyping. Genetic testing is increasingly recognized as a key component. Here, we review current concepts in VWD diagnostics and highlight the Spanish Clinical and Molecular Profile of von Willebrand Disease (PCM-EVW-ES) project as a model for genomics-enabled precision medicine. PCM-EVW-ES is a multicen

Innate antiviral and immune functions associated with the HIV reservoir decay after anti-PD-1 therapy.

Antiretroviral therapy (ART) suppresses HIV but does not eliminate the latent viral reservoir, which persists in programmed cell death protein 1 (PD-1)-expressing CD4+ T cells. Anti-PD-1 therapies have reduced the HIV reservoir in people living with HIV (PLWH) and cancer; however, the individuals who benefit and the mechanisms driving reservoir reduction remain unclear. We performed a prespecified exploratory, longitudinal multiomic profiling of 30 PLWH (29 males and one female) with cancer in the phase 1 CITN-12 clinical trial, in which pembrolizumab was evaluated for safety and preliminary a

SCRMP-mediated SUMOylation of CSNK2B facilitates DNA damage repair to promote cisplatin resistance in small cell lung cancer.

Small cell lung cancer (SCLC) represents the most lethal subtype of lung carcinoma. Platinum-based agents, such as cisplatin and carboplatin, remain the cornerstone of first-line therapy for patients with SCLC, whether administered as chemotherapy alone or in combination with immunotherapy. However, patients frequently develop platinum resistance within a short time frame, leading to therapeutic failure. The molecular mechanisms underlying platinum resistance in SCLC require further investigation, given that effective intervention strategies remain elusive. Through multiomic analysis integrati

Differentiation-dependent EBF1 activity determines CD22 transcription and leukemia sensitivity to inotuzumab ozogamicin.

Inotuzumab ozogamicin (InO) is an antibody-calicheamicin conjugate with high efficacy in lymphoid malignancies. It targets the B-cell surface protein CD22, which is expressed in most B-cell acute lymphoblastic leukemia (B-ALL) cases, albeit with variable intensity. However, factors governing CD22 expression and thus leukemia sensitivity to InO remain incompletely understood. Using multiomic characterization of 196 human B-ALL samples, coupled with ex vivo InO sensitivity profiling, we showed that early leukemia differentiation arrest at the pre-pro-B stage is associated with resistance to InO.
