Vasopressin-induced hyponatremia in an adult normotensive trauma patient: A case report.

BACKGROUND: Arginine vasopressin is a neuropeptide produced in the hypothalamus and released by the posterior pituitary gland. In addition to maintaining plasma osmolarity, under hypovolemic or hypotensive conditions, it helps maintain plasma volume through renal water reabsorption and increases systemic vascular tone. Its synthetic analogues are widely used in the intensive care unit as a continuous infusion, in addition to hospital floors as an intravenous or intranasal dose. A limited number of cases of hyponatremia in patients with septic or hemorrhagic shock have been reported previously

Optical ventricular cardioversion by local optogenetic targeting and LED implantation in a cardiomyopathic rat model.

AIMS: Ventricular tachyarrhythmias (VTs) are common in the pathologically remodelled heart. These arrhythmias can be lethal, necessitating acute treatment like electrical cardioversion to restore normal rhythm. Recently, it has been proposed that cardioversion may also be realized via optically controlled generation of bioelectricity by the arrhythmic heart itself through optogenetics and therefore without the need of traumatizing high-voltage shocks. However, crucial mechanistic and translational aspects of this strategy have remained largely unaddressed. Therefore, we investigated optogeneti

Resuscitative endovascular balloon occlusion of the aorta (REBOA) may be superior to resuscitative thoracotomy (RT) in patients with traumatic brain injury (TBI).

BACKGROUND: The effects of aortic occlusion (AO) on brain injury are not well defined. We examined the impact of AO by resuscitative endovascular balloon occlusion of the aorta (REBOA) and resuscitative thoracotomy (RT) on outcomes in the setting of traumatic brain injury (TBI). METHODS: Patients sustaining TBI who underwent RT or REBOA in zone 1 (thoracic aorta) from September 2013 to December 2018 were identified. The indication for REBOA or RT was hemodynamic collapse due to hemorrhage below the diaphragm. Primary outcomes included mortality and systemic complications. RESULTS: 282 patien

The potential role of the HCN1 ion channel and BDNF-mTOR signaling pathways and synaptic transmission in the alleviation of PTSD.

The function of the hyperpolarization-activated cyclic nucleotide-gated channel 1 (HCN1) and the expression of brain-derived neurotrophic factor (BDNF) may be involved in the pathogenesis of post-traumatic stress disorder (PTSD). This study aims to explore the role of the HCN1 channel, BDNF, and mTOR in the actions of PTSD and to examine whether synaptic transmission or plasticity is involved in the regulation of this disease. In the present study, rats were exposed to the single prolonged stress and electric foot shock (SPS&S) procedure, which can induce PTSD-like behaviors in rats. ZD7288 wa

Initial blood pressure is important for long-term outcome after traumatic spinal cord injury.

OBJECTIVE: Patients with traumatic spinal cord injury (TSCI) are at risk of developing neurogenic shock that causes hypotension and thereby secondary injury to the spinal cord due to ischemia. Hemodynamic treatment of patients with acute TSCI remains inadequately elucidated. Guidelines for management are divergent and based on limited evidence. To this end, the authors evaluated whether mean arterial blood pressure (MABP) during the prehospital and initial hospital phases of TSCI treatment is correlated with long-term neurological outcome. METHODS: The authors performed a retrospective cohort

Systematic reviews of scores and predictors to trigger activation of massive transfusion protocols.

BACKGROUND: The use of massive transfusion protocols (MTPs) in the resuscitation of hemorrhaging trauma patients ensures rapid delivery of blood products to improve outcomes, where the decision to trigger MTPs early is important. Scores and tools to predict the need for MTP activation have been developed for use to aid with clinical judgment. We performed a systematic review to assess (1) the scores and tools available to predict MTP in trauma patients, (2) their clinical value and diagnostic accuracies, and (3) additional predictors of MTP. METHODS: MEDLINE, EMBASE, and CENTRAL were searched

Prolonged bradycardia, asystole and outcome of high spinal cord injury patients: Risk factors and management.

BACKGROUND: High spinal cord injury (HSCI) is one of the devastating traumatic injuries. 80% of these patients are young male, and 93% will have major neurological disabilities. There is a paucity of literature about prolonged bradycardia in HSCI patients. The aim of this study was to know the prevalence, risk factors, precipitating factors for prolonged bradycardia in the HSCI patients. MATERIALS AND METHODS: All patients who were admitted to the Intensive Care Unit (ICU) of a tertiary hospital, with spinal cord injury above level of dorsal (D4) were enrolled in this study prospectively. [The concept of an emergency admission unit in a major community hospital].

The main task of a central interdisciplinary emergency admission unit (EAU) staffed and administered by the anesthesia department is to provide primary hospital treatment for emergency patients. The EAU serves also as a base for the emergency ambulance and functions as a 24-hour resuscitation unit for the whole hospital. The placement of the unit in the underground floor of the hospital makes it easy accessible for ambulances but excludes walk-in patients. The shock room has a floor area of 152 m2 and is optimally equipped, while two smaller emergency rooms provide reserve accommodation. On th

Blunt cerebrovascular injuries: does treatment always matter?

BACKGROUND: Blunt cerebrovascular injuries (BCVI) have become an increasingly recognized entity. Stroke as a result of these injuries can have devastating consequences. Optimal screening criteria, diagnostic imaging, and therapy for BCVIs have not been elucidated. Our institution began to apply liberal screening criteria using a whole-body scanning protocol with multidetector computed tomographic (WB-MDCT) scans to diagnose these injuries. The purpose of this study is to describe a single institution's large experience in patients with BCVI in an effort to provide insight into the diagnosis an

Renal outcomes of intensive care burn patients in an Asian tertiary centre.

INTRODUCTION: Acute kidney injury (AKI) is a frequent complication of severe burn injury and is associated with a high mortality rate of up to 80%. We aimed to establish the incidence, mortality rate, and factors related to mortality in adult patients with severe burn injury and AKI with renal replacement therapy (RRT) in Singapore. METHODS: We performed a retrospective cohort study of severely burned patients who were admitted to the Burns Intensive Care Unit (BICU) at the Singapore General Hospital (SGH) from January 2008 to December 2016. We compared patients with AKI with RRT who survived

Modelling post-traumatic stress disorder-major depressive disorder comorbidity in rats: impaired α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor signalling is consistent with synergistic behavioural and cognitive deficits.

Background: Post-traumatic stress disorder comorbid with major depressive disorder (PTSD + MDD) is a multifactorial condition with significant individual and societal impacts. Although neuroimaging studies have highlighted large-scale neural circuit dysfunctions, the underlying pathogenic mechanisms at the synaptic level remain poorly understood. The glutamatergic system, particularly the α-amino-3-hydroxy-5-methyl-4-isoxazolepropionic acid receptor (AMPAR), is hypothesized to play a pivotal role in stress-induced pathologies, yet its specific role in PTSD + MDD comorbidity has not been elucid

High-ratio plasma resuscitation decreases mortality in moderate traumatic brain injury.

BACKGROUND: Traumatic brain injury (TBI) is a leading cause of trauma-related mortality. While high ratios of fresh frozen plasma (FFP):red blood cells (RBC) improve survival in hemorrhagic shock, the role of FFP in polytrauma patients with TBI not requiring massive transfusion is unclear. We hypothesized that higher FFP ratios would be associated with improved early and late survival across TBI severities. METHODS: Adult trauma patients (age 18 or older) with suspected TBI (head AIS ≥1) who received ≥1 unit of blood product from 2020 to 2021 were identified in the American College of Surgeon

Red blood cell transfusion in neurocritical patients: an updated systematic review and meta-analysis.

BACKGROUND: Anemia is a prevalent complication in neurocritical patients and contributes to secondary brain damage by diminishing the oxygen supply to the brain. Although red blood cell transfusions may enhance cerebral oxygen delivery, whether this intervention improves patient outcomes remains unclear. METHODS: We searched the Cochrane Central Register of Controlled Trials (CENTRAL), Embase, and MEDLINE (PubMed) from inception to January 26, 2025. All randomized controlled trials (RCTs) assessing liberal versus restrictive RBC transfusion strategies in neurocritical patients were included i

The Use of Extracorporeal Membrane Oxygenation in Traumatic Brain Injury and Neurosurgical Patients: A Single-Center Analysis and Systematic Review.

BACKGROUND: This study evaluates the outcomes and management considerations associated with using extracorporeal membrane oxygenation (ECMO) in patients with traumatic brain injury (TBI), including those requiring neurosurgical intervention. It examines the feasibility of managing patients with TBI and neurosurgical patients on ECMO by synthesizing institutional data with the existing literature, emphasizing observed clinical contexts, anticoagulation strategies, and peri-procedural factors relevant to safe ECMO use. Our aim is to elucidate the potential for ECMO to be safely applied in patien

Predicting the Prehospital Blood Transfusion Need in a Large Metropolitan Center.

OBJECTIVES: Prehospital blood transfusion (PHBT) has been associated with reduced mortality for trauma patients in hemorrhagic shock. However, there are currently no standardized prehospital criteria to identify patients who may benefit from PHBT. We designed this study to assess the positive predictive value (PPV) of a set of PHBT criteria to identify patients requiring emergent transfusion within four hours of emergency department (ED) arrival. METHODS: We conducted an observational retrospective cohort study of all trauma patients ≥ 15 years-old who were transported by emergency medical se

Rib fixation still helps: Surgical stabilization of rib fractures in traumatic brain injury, a TQIP Study.

BACKGROUND: Surgical stabilization of rib fractures (SSRF) has been associated with lower odds of mortality and reduced pulmonary morbidity in patients with chest trauma, but its role in patients with concomitant traumatic brain injury (TBI) remains controversial. We hypothesized that TBI phenotype modifies the association between SSRF and clinical outcomes. METHODS: We conducted a retrospective study using the TQIP (2017-2023) database. Adults 18 years or above with head and thoracic AIS ≥3 and ≥3 rib fractures were included. TBI phenotypes (focal, diffuse, mixed, concussion-only) were class
