Surgical management of urinary incontinence in children with anatomical bladder-outlet anomalies.

BACKGROUND: Pediatric urinary incontinence secondary to anatomical anomalies of the bladder outlet is relatively rare, and there is no consensus on its surgical treatment. We reviewed the clinical courses and surgical management of children with this pathology. METHODS: From 1991 to 2001, we performed bladder neck reconstruction on seven children (three boys and four girls). The mean age at the first intervention was 7 years (range 5-12 years). The underlying diagnosis was bladder exstrophy in two children, pseudoureterocele-type ectopic ureter in three, bladder neck incompetence from unknown

Cutaneous and laryngeal squamous cell carcinoma in mixed epidermolysis bullosa, kindler syndrome.

Kindler syndrome is a rare autosomal recessive genodermatosis characterized by trauma-induced acral blisters in infancy and childhood, photosensitivity, and progressive poikiloderma. Other clinical features include chronic erosive gingivitis, dysphagia, esophageal and urethral strictures, ectropion, and an increased risk of mucocutaneous squamous cell carcinoma. We describe a patient with Kindler syndrome associated with squamous cell carcinoma of the skin and larynx. He had squamous cell carcinoma on his left knee with simultaneous unresectable laryngeal carcinoma at the age of 43 years. The …

Modifications of the radial forearm flap phalloplasty for female-to-male gender reassignment.

The radial forearm flap remains the preferred technique for phalloplasty. From 1999 to 2009, 19 patients with primary female transsexualism … acceptable aesthetic appearance. We further describe technical modifications to reduce the rate of urethral strictures and fistulas. None of the patients regretted undergoing gender transformation. Patients are satisfied with the surgical result and generally prepared to accept its potential costs, in view of the significant psychological and legal benefits.

Augmentation vaginoplasty of colonic neovagina stricture using oral mucosa graft.

BACKGROUND: Vaginal reconstruction is performed for a variety of congenital and acquired anomalies, and several techniques have been described. Conventional neovaginal reconstructions typically involve skin grafts or vascularized intestinal segments. Oral mucosa has been used successfully in urethral reconstruction, and several long-term studies have demonstrated its surgical versatility and durability. CASE REPORT: Here we present the successful use of an oral mucosal graft in the surgical treatment of a strictured, colonic neovagina in a 19-year-old, 46XX female with cloacal exstrophy. CON

Long-term outcome of metaidoioplasty in 70 female-to-male transsexuals.

In 1996, metaidoioplasty was introduced as an alternative to phalloplasty in female-to-male transsexuals. To assess the long-term outcome in 70 consecutive patients (mean follow-up 8 years), we established the characteristics of postoperative events and additional surgical procedures. Metaidoioplasty and primary or secondary scrotoplasty was uneventful in 8 patients. In the other patients, postoperative events included immediate postoperative complications (n = 23), urethral fistulas (n = 26) or strictures (n = 25), or loss (n = 22) or dislocation (n = 34) of testicular prostheses. An average

Self-expanding stents for malignant dysphagia.

Self-expanding metallic stents have been employed successfully for vascular, urethral, and biliary strictures. In a prospective study we examined the efficacy of the 16 mm Wallstent for palliation of malignant dysphagia. Eight patients with severe dysphagia due to advanced primary (four) or secondary (four) oesophageal malignant disease were recruited and nine Wallstents were inserted (one patient required two). Dysphagia was reduced in all but one patient, who died after oesophageal perforation; a second patient had a self-limiting bout of haematemesis. Two patients required subsequent treatm

Flexible cystourethroscopy in the follow-up of posturethroplasty patients and characterisation of recurrences.

BACKGROUND: Urethral strictures can be difficult to diagnose at an early stage because the urinary flow rate does not diminish until the urethral calibre is ≤3mm. In the past, posturethral surgery follow-up has relied upon flow rates and contrast imaging. OBJECTIVE: To evaluate the role of flexible urethroscopy in the follow-up of patients undergoing urethroplasty. DESIGN, SETTING, AND PARTICIPANTS: Prospective flexible urethroscopy follow-up of 144 male patients who underwent urethroplasty by a single surgeon over a 10-yr period at a tertiary referral centre. INTERVENTION: Flexible urethro…

Nurse-led postoperative telemonitoring program following outpatient urethroplasty: a pilot study.

INTRODUCTION: Urethral stricture disease remains a challenging condition with significant morbidity and quality-of-life impairment. Urethroplasty represents the gold-standard treatment, yet postoperative follow-up is demanding, requiring multiple in-person visits at referral centers, often far from patients’ homes. To optimize postoperative care, we developed a structured nurse-led postoperative telemonitoring program (TMP) designed to replace in-person follow-up visits. This study aimed to describe the TMP development and to assess feasibility, safety, and patient acceptability. MATERIALS AND…

Reoperative Anorectal Procedures in Patients with Anorectal Malformations - Is Bladder Function Affected?

BACKGROUND: Anorectal malformations (ARM) are associated with neurogenic bladder. The traditional surgical ARM repair is a posterior sagittal anorectoplasty (PSARP), which is believed to have a minimal effect on bladder dynamics. However, little is known about the effects of reoperative PSARP (rPSARP) on bladder function. We hypothesized that a high rate of bladder dysfunction existed in this cohort. METHODS: We performed a retrospective review of ARM patients undergoing rPSARP at a single institution from 2008 to 2015. Only patients with Urology follow-up were included in our analysis. Data

Extended Ureteral Stricture Corrected With Appendiceal Replacement in a Kidney Transplant Recipient.

OBJECTIVE: To present our technique of ureteral replacement with an appendicovesicostomy for the treatment of extended ureteral stricture after kidney transplantation. Ureteral stricture represents a urologic complication that may cause progressive function impairment and graft loss. Symptomatic ureteral stenosis is generally treated with a percutaneous nephrostomy and ureteral stenting. This approach may be initially effective, but often does not offer a long-term solution. METHODS: A 48-year-old Caucasian man underwent kidney transplantation from a heart-beating deceased donor. The graft wa
