The CaV1.2 L-type calcium channel regulates bone homeostasis in the middle and inner ear.

Bone remodeling of the auditory ossicles and the otic capsule is highly restricted and tightly controlled by the osteoprotegerin (OPG)/receptor activator of nuclear factor kappa-Β ligand (RANKL)/receptor activator of nuclear factor kappa-Β (RANK) system. In these bony structures, a pathological decrease in OPG expression stimulates osteoclast differentiation and excessive resorption followed by accrual of sclerotic bone, ultimately resulting in the development of otosclerosis, a leading cause of deafness in adults. Understanding the signaling pathways involved in maintaining OPG expression in

[Phenotypic characterization of a DFNA6 family with low-frequency hearing loss].

BACKGROUND: Hereditary hearing impairment is a heterogeneous sensory defect with approximately two-thirds of all cases being nonsyndromic. Only two loci (DFNA1 and DFNA6/14/38) are associated with low frequency sensorineural nonsyndromic hearing impairment. DFNA6 was mapped to chromosome 4p16. Recessive mutations in the WFS1 gene are responsible for Wolfram syndrome; missense mutations inherited as an autosomal dominant result in low frequency sensorineural hearing impairment (LFSNHI). PATIENTS AND METHODS: In this study we analyzed the phenotype of a large Hungarian family with LFSNHI and li

Mechanistic Pathways Linking Type 2 Diabetes Mellitus, Hypertension, and Osteoarthritis to Falls in Older Adults: A Scoping Review.

Background: Falls in older adults are a major public health and rehabilitation concern and may reflect interacting chronic disease-related mechanisms rather than diagnostic labels alone. Type 2 diabetes mellitus (T2DM), hypertension (HTN), and osteoarthritis (OA) are common in later life and may influence fall vulnerability through sensory, vascular, musculoskeletal, cognitive, medication-related, and functional pathways. This scoping review aimed to map mechanism-oriented evidence linking T2DM, HTN, and OA to falls and fall-related outcomes in older adults. Methods: A scoping review was condu

Ototoxicity Associated with Antineoplastic Agents in the Pediatric Population: An Evidence-Based Review of Auditory Monitoring Strategies and Contemporary Diagnostic Frameworks-Narrative Review.

Ototoxicity represents a clinically significant complication of anticancer therapy in pediatric patients. Cytotoxic agents used in oncology, particularly platinum-based chemotherapy, may induce damage to the auditory and vestibular systems, resulting in hearing loss, tinnitus, and balance disturbances. Even mild hearing impairment during childhood may negatively affect speech perception, language development, communication abilities, and subsequent educational and psychosocial functioning. This narrative review aims to synthesize current evidence on treatment-related ototoxicity in children, w

Differential diagnosis between Pendred and pseudo-Pendred syndromes: clinical, radiologic, and molecular studies.

The disease gene for Pendred syndrome has been recently characterized and named PDS. It codes for a transmembrane protein called pendrin, which is highly expressed at the apical surface of the thyroid cell and functions as a transporter of chloride and iodide. Pendrin is also expressed at the inner ear level, where it appears to be involved in the maintenance of the endolymph homeostasis in the membranous labyrinth, and in the kidney, where it mediates chloride-formate exchange and bicarbonate secretion. Mutations in the PDS gene and the consequent impaired function of pendrin leads to the cla

Cochlear implantation in children with anomalous cochleovestibular anatomy.

OBJECTIVES/HYPOTHESIS: To evaluate outcomes after cochlear implantation in children with anomalous cochleovestibular anatomy, a review of radiological classification, surgical implantation, and outcome of 103 children with such anomalies was performed. The hypothesis was that children with anomalous cochleovestibular anatomy would have poorer outcomes and therefore be poorer candidates as a result of their diminished ability to interpolate and use auditory information delivered through a cochlear implant. STUDY DESIGN: A series of studies was carried out to review the cochleovestibular anomal

OculoMotor Assessment Tool Test Procedure and Normative Data.

SIGNIFICANCE: This study establishes normative data and a testing procedure for the oculomotor assessment tool. The oculomotor assessment tool standardizes visual targets for the Vestibular/OculoMotor Screening assessment and provides additional metrics that may aid in the differentiation between those with normal and those with abnormal oculomotor function potentially caused by a concussion. PURPOSE: This study aimed to assess the oculomotor endurance of healthy participants with no self-reported history of concussions using the oculomotor assessment tool. METHODS: Healthy participants (n =

Data-driven vestibular fMRI in PPPD and residual dizziness after BPPV: a narrative methodological review of AI-based phenotyping and longitudinal modeling.

Neuroimaging studies of vertigo and chronic dizziness have identified abnormalities across vestibular, visual, somatosensory, and cognitive-emotional networks, but conventional group-level analyses are limited in capturing individual heterogeneity and longitudinal change. This narrative methodological review integrates current evidence and proposes a framework combining functional magnetic resonance imaging (fMRI), artificial intelligence (AI), and advanced statistical modeling, anchored in two priority research scenarios: PPPD phenotyping and longitudinal characterization of residual dizzines

Cognitive-motor interference during the suppression head impulse paradigm.

ObjectiveThe vestibulo-ocular reflex (VOR) is critical for stabilizing vision during head motion and for daily multitasking activities. The suppression head impulse paradigm (SHIMP) complements the head impulse paradigm (HIMP) and is considered more sensitive for residual vestibular function. This study examined the impact of cognitive and motor dual-tasking on SHIMP in healthy adults.MethodsTwenty-two participants performed single-task HIMP and SHIMP alongside two cognitive dual-task SHIMP (serial subtraction, map navigation) and one motor dual-task SHIMP (counter clicking). Outcomes included

An Overview of Noninvasive Brain Stimulation: Basic Principles and Clinical Applications.

The brain has the innate ability to undergo neuronal plasticity, which refers to changes in its structure and functions in response to continued changes in the environment. Although these concepts are well established in animal slice preparation models, their application to a large number of human subjects could only be achieved using noninvasive brain stimulation (NIBS) techniques. In this review, we discuss the mechanisms of plasticity induction using NIBS techniques including transcranial magnetic stimulation (TMS), transcranial direct current stimulation (tDCS), transcranial alternating cu

Neurofibromatosis Type 2.

Neurofibromatosis type 2 (NF2) is a rare autosomal dominant disorder (incidence 1:33 000-40 000) characterized by formation of central nervous system tumors, due to mutation in the NF2 gene on chromosome 22q12. Vestibular schwannomas are the hallmark lesion, affecting 95% of individuals and typically occur bilaterally. Schwannomas commonly occur on other nerves intracranially and in the spinal compartment, along with meningiomas, ependymomas, and gliomas. Although histologically benign, tumors are associated with significant morbidity due to multiple problems including hearing and vision loss,

Goitrous congenital hypothyroidism and hearing impairment associated with mutations in the TPO and SLC26A4/PDS genes.

CONTEXT: Pendred syndrome (PS) and thyroid peroxidase (TPO) deficiency are autosomal-recessive disorders that result in thyroid dyshormonogenesis. They share congenital hypothyroidism, goiter, and an iodide organification defect as common features. Whereas the hallmark of PS is sensorineural deafness, other forms of congenital hypothyroidism may also lead to hearing impairment. Therefore, a definite diagnosis may be difficult and require molecular genetic analyses. CASE REPORT: The propositus presented at birth with primary hypothyroidism and goiter. He also had congenital bilateral moderate

Visual and visual-proprioceptive training improve gait via sensory reweighting after chronic stroke: a randomized controlled trial.

BACKGROUND: Post-stroke gait recovery depends on multisensory integration, but vision- and gaze-stability training are rarely emphasized. We tested whether 12-week visual oculomotor training (VOT) or visual–proprioceptive integration (VPI) programs improve gait after chronic stroke. METHODS: In this single-center, assessor- and analyst-blinded randomized controlled trial, 45 participants were allocated to a control group (CG), VOT, or VPI. All groups received standardized gait training plus three 30-min group-specific sessions weekly for 12 weeks. Static and dynamic visual acuity were assessed

Enlarged vestibular aqueduct: Audiological and genetical features in children and adolescents.

BACKGROUND: Enlarged Vestibular Aqueduct (EVA) is one of the most common congenital malformations associated with sensorineural or mixed hearing loss. The association between hearing loss and EVA is described in syndromic (i.e. Pendred Syndrome, BOR, Waardenburg) and non-syndromic disorders, as isolate or familiar mutations of the SLC26A4 gene. The audiological phenotype of the EVA syndrome is heterogeneous, the type and entity of hearing loss may vary and vertigo episodes might also be present. OBJECTIVE: The aim of this retrospective study was to describe the clinical and genetic features o
