Radiographic classification of Os odontoideum and its clinical significance.

STUDY DESIGN: This study analyzed the relationship between the configuration of the atlantoaxial joint and the clinical characteristics of p … n and had more severe myelopathy than that of patients in the other groups. According to Crandall's classification of myelopathy, all patients showed the transverse lesion syndrome in the cone and blunt-tooth types. Four of six patients with the round type of atlantoaxial configuration had the Brown-Séquard syndrome. With the round type, it appeared that the lat …

Cervical cord neurapraxia: classification, pathomechanics, morbidity, and management guidelines.

One hundred ten cases of the transient neurological phenomenon, cervical cord neurapraxia (CCN), are presented. The authors established a classification system for CCN, developed a new computerized measurement technique for magnetic resonance (MR) imaging, investigated the relationship of the cervical cord to the canal, and analyzed clinical, x-ray, and MR data. One hundred nine males and one female were included in the study; the average age of the participants was 21 years (range 13-33 years). All episodes occurred during sports participation; 87% occurred while the patient was playing footb…

[Anejaculation: its etiology and pathogenesis, classification and clinical aspects].

43 patients suffering from anejaculation were treated in 1983-1993 in the department of andrology of the N. A. Semashko Medical Institute urological clinic. Most commonly anejaculation is caused by prostatic and bladder neck surgery (25.6%), diabetes mellitus (18.6%) and presents as retrograde ejaculation, impaired sperm emission into the urethra, aspermatism (51.2, 27.9, 20.9% of patients, respectively). The leading pathogenetic factors are peripheral neuropathy, surgical injury to the bladder internal sphincter, psychosexual disorders. The authors propose classification and algorithm of the …

[Cranio-cerebral dermoid and epidermoid cysts. Classification and pathogenesis].

The pathogenesis of cranial dermoids and epidermoids is still controversial, owing to the multiple etiologies and locations of these lesions. We reviewed 25 cases, classified as follows: extradural lesion of the calvarium; of the occipital squama; dysraphic occipital lesions; and strictly intradural lesions. In the latter group, all lesions but one were in a paramedian, prenevraxial situation, and could be classified according to their situation relative to the tentorium cerebelli. In our series, strictly intradural dermoids are more often in a rostral situation, and epidermoids in a more caud

The host tissue reaction to Madurella mycetomatis: new classification.

In this prospective study, three types of tissue reaction to mycetoma grains are described. Type I reaction is characterized by the adherence of neutrophils to the surface of the grain leading to its disintegration. In Type II reaction, the fragmented grain and the dead neutrophils are cleared by macrophages and multinucleated giant cells, while in Type III reaction there is a discrete well developed epithelioid granuloma with Langhans's giant cells. The three types of reaction are usually seen in different combinations in the same lesion.

DNA sequence analysis using hierarchical ART-based Classification Networks.

Adaptive resonance theory (ART) describes a class of artificial neural network architectures that act as classification tools which self-organize, work in realtime, and require no retraining to classify novel sequences. We have adapted ART networks to provide support to scientists attempting to categorize tandem repeat DNA fragments from Onchocerca volvulus. In this approach, sequences of DNA fragments are presented to multiple ART-based networks which are linked together into two (or more) tiers; the first provides coarse sequence classification while the subsequent tiers refine the classific…

[Intractable vasculitis: general consideration-concept and classification: pathological aspects].

Vasculitis may be defined as an inflammatory process, primarily affecting the vascular wall which possesses an architecture, such as organ or tissue, and accordingly may not include the capillary. Since the walls are destroyed, secondary circulatory disturbances may be associated and produce vasculitic symptoms, which are nonspecific in most instances. Although the pathogenesis of most vasculitides is well understood, the lack of knowledge of the etiology makes it difficult to establish a satisfactory classification of vasculitis. The inflammatory lesions tend to be distributed along the cours…

Necrotizing fasciitis: a classification of necrotizing soft tissue infections.

Necrotizing fasciitis is a rare, often fatal soft tissue infection. It still remains a confusing entity because of the nomenclature and multiple subtypes described in the past. An interesting case study of a patient with necrotizing fasciitis secondary to nonclostridial gas gangrene is presented. A comprehensive review of necrotizing fasciitis, its disease process and treatment modalities will be discussed.

Premalignant lesions of the upper aerodigestive tract: pathologic classification.

Intraepithelial neoplasia of the upper aerodigestive tract (UADT), including both histologically defined dysplasia and carcinoma in situ (CIS), appears to fall into two broad groups similar to intraepithelial neoplasia of other squamous mucosae, keratinizing and non-keratinizing. Keratinizing dysplasia/CIS is common in the UADT and uncommon in other sites such as the cervix. In general, keratinizing epithelial proliferation results in thick epithelium, usually with prominent superficial keratin expression with a whitish or "leukoplakic" clinical appearance. Although most clinical leukoplakic c

The lateral pillar classification of Legg-Calvé-Perthes disease.

To determine the predictive value of a new classification system for Legg-Perthes, 93 hips in 86 patients with radiographic follow-up to maturity were reviewed. All patients were treated by bracing at the Texas Scottish Rite Hospital from 1970 to 1980. Hips were classified during the fragmentation stage of disease into three groups based on radiolucency in the lateral pillar of the femoral head. Final radiographs were reviewed at skeletal maturity, and the outcome was determined according to the Stulberg classification. Group A had a uniformly good outcome (100% Stulberg I and II results); Gro…

Exercise-Associated Collapse in Endurance Events: A Classification System.

In brief: Athletes who compete in endurance sports may sustain exercise-associated collapse (EAC) during or after an event. A classification system was devised for EAC that can be used by physicians who cover endurance events. Symptoms and signs of EAC include exhaustion, nausea, cramps, abnormally high or low core body temperature, muscle spasms, and inability to walk unassisted. The three classes of EAC are hyperthermic, normothermic, and hypothermic; each class is subclassified as mild, moderate, or severe. Treatment of warm runners includes applying ice bags wrapped with wet towels to the …

Clinical classification for undescended testes: experience in 1,010 orchidopexies.

A clinical classification for undescended testes based on location, size, and mobility of the testis in the inguinal canal was performed. The latter was artificially divided into three sections. The lower portion was considered position I; the mid part, position II; the upper part, position III; and the abdominal (nonpalpable) part, position IV). Normal-size testes were considered as being size A; testes reduced up to 30% in volume, size B; and gonads reduced more than 30% of the estimated normal, size C. According to these criteria, a case of chryptorchidism can then be classified as I, II, I…

Urinary tract dilatation in utero: classification and clinical applications.

A morphologic classification of in utero urinary tract dilatation is presented. Ninety-two hydronephrotic fetal kidneys diagnosed with ultrasound were graded according to the proposed classification. The findings suggest that grade I dilatation (anteroposterior diameter of the renal pelvis less than 10 mm) should be considered normal. Grades II and III constitute an intermediate hydronephrosis, requiring postnatal urologic surgery in nearly half the cases. Grade IV (moderate dilatation of the calyces, with easily identified residual renal cortex) and grade V (severe dilatation of the calyces w…

Classification of chronic psychoses including delusional disorders and schizophrenias.

A classification of chronic psychoses including nonparanoid schizophrenia, paranoid schizophrenia, paranoid state and paranoia (delusional disorder) is presented. This classification is dependent on a systematic increase in number of symptoms with each group. In particular, delusional disorder is examined with regard to family history. It is clear from the data which are presented that delusional disorder is more likely to be associated with a family history of such traits as suspiciousness, jealousy, secretiveness, and the presence of paranoid behavior or delusions. There is evidence that suc…

Radiologic classification of joint destruction in juvenile chronic arthritis.

A radiologic classification of the destruction of the large joints in juvenile chronic arthritis is described. Its principles are the same as previously published by the authors for evaluation of the knee joint. Osteoporosis, enlargement or other growth disturbance of the bone ends, cyst formation, erosions or deformation of the subchondral joint surface is expressed in terms of a score. In a retrospective study of 74 examinations of the large joints in 30 patients aged 6-18 years, the suggested classification was used. The radiologic score obtained had a good correlation to the clinical findi…
