Stability of markers used for real-time tumor tracking after percutaneous intrapulmonary placement.

PURPOSE: To determine the stability of markers used for real-time tumor tracking after percutaneous intrapulmonary placement. METHODS AND MATERIALS: A total of 42 patients with 44 lesions, 111 markers, and ≥2 repeat computed tomography (CT) scans were studied. The tumor on the repeat CT scans was registered with the tumor on the planning CT scan. Next, the three-dimensional marker coordinates were determined on the planning CT scan and repeat CT scans. Marker stability was analyzed by the displacement of the markers and the displacement of the center of mass (COM) of the marker configurations

The deep breathing test: median-based expiration-inspiration difference is the measure of choice.

Heart rate variability (HRV) has become an important parameter for the assessment of autonomic function in many areas of medicine. In particular, respiratory sinus arrhythmia measured during the deep breathing test (DBT) is often used. Results are usually expressed in common time-domain parameters. A "most preferred measure" has not yet been identified. We investigated the sensitivity of the DBT to the following anomalies: in-test variance; shifts of mean heart rate; premature ventricular contractions; and breathing rate deviations. Frequency and magnitude of the anomalies were determined in a

Impact of obstructive sleep apnea on lung volumes and mechanical properties of the respiratory system in overweight and obese individuals.

BACKGROUND: Even through narrowing of the upper-airway plays an important role in the generation of obstructive sleep apnea (OSA), the perip … y. These results might be attributed to the abnormally increased lung elasticity recoil pressure on exhalation, due to increase in lung elasticity and decreased lung volume in obese OSA.

Control of gill ventilation and air-breathing in the bowfin amia calva.

The purpose of this study was to investigate the roles of branchial and gas bladder reflex pathways in the control of gill ventilation and air-breathing in the bowfin Amia calva. We have previously determined that bowfin use two distinct air-breathing mechanisms to ventilate the gas bladder: type I air breaths are characterized by exhalation followed by inhalation, are stimulated by aquatic or aerial hypoxia and appear to regulate O2 gas exchange; type II air breaths are characterized by inhalation alone and possibly regulate gas bladder volume and buoyancy. In the present study, we test the h…

Predictive and Diagnostic Value of Nasal Nitric Oxide in Eosinophilic Chronic Rhinosinusitis with Nasal Polyps.

BACKGROUND: A hallmark of eosinophilic chronic rhinosinusitis with nasal polyps (eCRSwNP) is mucosal eosinophil-predominant inflammation. Nasal nitric oxide (nNO) is a known biomarker of eosinophilic inflammation in the upper airway. However, the utility of nNO measurement in the upper airway remains controversial. The present study aimed to compare the use of other clinical parameters with nNO to prediagnose patients with eCRSwNP from Central China. METHODS: From June 2019 to December 2019, 70 patients with CRSwNP undergoing endoscopic sinus surgery and 30 healthy subjects were enrolled. nNO

Remote Ischemic Preconditioning Decreases Oxidative Lung Damage After Pulmonary Lobectomy: A Single-Center Randomized, Double-Blind, Controlled Trial.

BACKGROUND: During lobectomy in patients with lung cancer, the operated lung is often collapsed and hypoperfused. Ischemia/reperfusion injury may then occur when the lung is re-expanded. We hypothesized that remote ischemic preconditioning (RIPC) would decrease oxidative lung damage and improve gas exchange in the postoperative period. METHODS: We conducted a single-center, randomized, double-blind trial in patients with nonsmall cell lung cancer undergoing elective lung lobectomy. Fifty-three patients were randomized to receive limb RIPC immediately after anesthesia induction (3 cycles: 5 mi

Diaphragm and genioglossus corticomotor excitability in patients with obstructive sleep apnea and control subjects.

Corticomotor excitability of peripheral muscles appears to be altered in patients with obstructive sleep apnea. However, there is no evidence of such alteration for upper airway/respiratory muscles that are involved in the pathophysiology of this disease. The aim of this study was to compare the effects of hypercapnic stimulation on diaphragm and genioglossus corticomotor excitability in awake healthy subjects versus patients with obstructive sleep apnea. Corticomotor excitability was assessed by transcranial magnetic stimulation in 12 untreated apneic men (48 ± 10 years; body mass index = 28.

Chronic eosinophilic pneumonia with persistent decreased diffusing capacity for carbon monoxide.

We report a 15-year-old girl presenting with dry cough, exertional dyspnoea, weight loss, fever and night sweats for over 1 month. Blood tests revealed hypereosinophilia, high IgE and antinuclear antibodies levels. Chest x-rays showed bilateral peripheral infiltrates mostly in the right upper lobe which was confirmed by a chest CT. Bronchoalveolar lavage showed hypercellularity with 28% of eosinophils. Idiopathic chronic eosinophilic pneumonia was confirmed after exclusion of other causes of eosinophilic pneumonia and systemic disease. The patient responded dramatically to oral corticosteroids

Sagittal reformations of volumetric inspiratory and expiratory high-resolution CT of the lung.

RATIONALE AND OBJECTIVES: The purpose of this study is to determine whether sagittal reformations of volumetric high-resolution computed tomography (CT) provide additional information in evaluating lung abnormalities compared to axial high-resolution CT images. MATERIALS AND METHODS: Twenty-two consecutive patients who underwent combined inspiratory and expiratory volumetric high-resolution CT from May to July 2003 were studied. Using end-inspiratory and end-expiratory sagittal reformatted images, respiratory motion artifacts were evaluated at upper, middle, and lower lung areas bilaterally.

Testing limits to airflow perturbation device (APD) measurements.

BACKGROUND: The Airflow Perturbation Device (APD) is a lightweight, portable device that can be used to measure total respiratory resistance as well as inhalation and exhalation resistances. There is a need to determine limits to the accuracy of APD measurements for different conditions likely to occur: leaks around the mouthpiece, use of an oronasal mask, and the addition of resistance in the respiratory system. Also, there is a need for resistance measurements in patients who are ventilated. METHOD: Ten subjects between the ages of 18 and 35 were tested for each station in the experiment. T…

Preoperative expiratory muscle training for swallowing function in patients with esophageal cancer undergoing esophagectomy: A randomized controlled phase II trial protocol.

Esophagectomy is a highly invasive and curative procedure for esophageal cancer. Although minimally invasive techniques reduce the incidence of pulmonary complications, postoperative dysphagia remains a common and clinically significant issue. Preoperative expiratory muscle training (EMT) may improve swallowing function by strengthening the relevant muscles; however, its effectiveness in patients with esophageal cancer has not been widely studied. This phase II randomized, controlled, double-blind trial has been designed to evaluate the effects of preoperative EMT on postoperative swallowing f

Sputum Eosinophils Predict Annual Remission of the Frequent Exacerbator Phenotype in COPD: A Prospective Cohort Study.

INTRODUCTION: The frequent-exacerbator (FE) phenotype guides COPD management, yet its year-to-year stability and the role of airway eosinophilia in FE remission are uncertain. METHODS: In a single-center prospective cohort (n = 289), patients were classified as FE (≥ 2 moderate or ≥ 1 severe exacerbation in the prior year; n = 88) or non-FE (n = 201) and followed 12 months. FE patients were labeled FE-persistent or FE-remitted at 1 year. Clinical data, spirometry, CT, fractional exhaled nitric oxide (FeNO), blood counts, and induced-sputum cytology were collected. Defective exercise-related expiratory muscle recruitment in patients with PHOX2B mutations: A clue to neural determinants of the congenital central hypoventilation syndrome.

INTRODUCTION AND OBJECTIVES: The human congenital central hypoventilation syndrome (CCHS) is caused by mutations in the PHOX2B (paired-like homeobox 2B) gene. Genetically engineered PHOX2B rodents exhibit defective development of the brainstem retrotrapezoid nucleus (RTN), a carbon dioxide sensitive structure that critically controls expiratory muscle recruitment. This has been linked to a blunted exercise ventilatory response. Whether this can be extrapolated to human CCHS is unknown and represents the objective of this study. MATERIALS AND METHODS: Thirteen adult CCHS patients and 13 health
