Proximal humerus fractures treated with single or dual-plate constructs: a comparison of clinical outcomes.

BACKGROUND: Proximal humerus fractures (PHFs) are common injuries in the adult population. Open reduction and internal fixation (ORIF) still represents the most common treatment for reconstructable injuries. Despite the continued prevalence of ORIF, studies have demonstrated fixation failure ranging from 11% to 18% and a reoperation rate of up to 27%. Dual plating is an emerging technique that has been noted to enhance fracture fixation. There is only one comparative study examining the outcomes of isolated locked plating vs. dual plating. Therefore, our objective was to compare the effect of

Optimal blocking screw placement for retrograde IM nail fixation of distal femur fracture: a standardized biomechanical study of "osteoporotic" synthetic bone.

PURPOSE: Intramedullary (IM) nail use for metaphyseal fracture management is problematic in long bones like the femur because the larger medullary cavity allows increased fracture motion and potentially increased implant failure and malunion/nonunion risk; Achieving effective distal femur fracture fixation is even more difficult in osteoporotic bone. Blocking screws to control IM nail movement are known to be effective for tibia fracture management. Few studies have evaluated IM nail and blocking screw use efficacy for distal femur fracture fixation in osteoporotic bone. METHODS: This study u

The survivorship and factors affecting the outcome of the use of spring plates in multifragmentary posterior wall acetabular fractures at a single academic institution.

BACKGROUND: Multifragmentary posterior wall fractures are not uncommon after posterior hip fracture-dislocation. They can be elementary but are commonly associated with transverse acetabular fractures. Specific technical challenges are encountered when managing these fracture patterns. The loss of the posterior landmarks due to the associated comminution may render achieving an anatomical reduction and stable fixation difficult. The application of spring plates can provide a good buttress with multiple anchor points and creates a new cortex for the bridging plate. PURPOSE OF THE STUDY: -To as

Displaced Midshaft Clavicle Fracture Union Can Be Accurately Predicted with a Delayed Assessment at 6 Weeks Following Injury: A Prospective Cohort Study.

BACKGROUND: It is unclear if clinical recovery following a midshaft clavicle fracture can accurately predict fracture-healing. The additional information that can be assessed at 6 weeks after injury may have superior predictive value compared with information available at the time of the injury. METHODS: A prospective study of all patients (≥16 years of age) who sustained a fully displaced midshaft clavicle fracture was performed. We assessed patient demographic characteristics, injury factors, functional scores, and radiographic predictors with a standardized protocol at 6 weeks. Conditional

Tension-Band Wire Fixation of Olecranon Fractures.

Olecranon fractures account for approximately 20% of fractures of the proximal part of the forearm1. Clinicians may consider nonoperative management for elderly low-demand patients2, whereas operative fixation is recommended for active patients with a displaced fracture. Tension-band wire (TBW) fixation is commonly employed for simple isolated stable displaced fractures of the olecranon (type IIA according to the Mayo classification)3-5. In contrast, plate fixation is thought to provide superior outcomes for unstable comminuted olecranon fractures. Biomechanical principles of the TBW construct

WITHDRAWN: Surgical interventions for treating distal radial fractures in adults.

BACKGROUND: Fracture of the distal radius is a common clinical problem, particularly in older white women with osteoporosis. OBJECTIVES: To determine when, and if so what type of, surgical intervention is the most appropriate treatment for fractures of the distal radius in adults. SEARCH STRATEGY: We searched the Cochrane Bone, Joint and Muscle Trauma Group specialised register (November 2002), the Cochrane Central Register of Controlled Trials (The Cochrane Library, Issue 1, 2003), MEDLINE (1966 to February 2003), EMBASE (1988 to 2003 Week 8), CINAHL (1982 to February 2003), the National Re

Nailing of three- and four-part fractures of the humeral head -- long-term results.

BACKGROUND AND PURPOSE: Long term outcome of the treatment of displaced complex fractures of humeral head is rare in the literature especially in greater cohorts. Main purpose of our study was the assessment of long term results of intramedullary nailing of 3-4 part fractures. PATIENTS AND METHODS: 137 patients with 137 three or four-part fractures of the humeral head treated by intramedullary nailing were reassessed after an average follow-up time of 57 (27-93) months. The whole cohort consisted of 85 three-part fractures, 38 four-part fractures and 14 fracture-dislocations. Radiographic and

Staged Emergency Caesarean Section and Cementless Total Hip Arthroplasty Within a Single Anaesthetic Episode for Pipkin Type III Femoral Head Fracture-Dislocation in a Gravid Patient: A Case Report.

Pipkin type III femoral head fracture-dislocation in a gravid patient is an exceptionally rare clinical entity. We report the case of a woman in her early 40s who presented at 36 weeks of gestation following a high-energy motor vehicle collision. Clinical examination and imaging confirmed a left posterior hip dislocation; computed tomography revealed a severely comminuted Pipkin type III fracture that precluded open reduction and internal fixation. A multidisciplinary team formulated a staged single-anaesthetic operative strategy; emergency caesarean section was performed immediately before ce

The origins of limb lengthening and reconstruction surgery date back to 1521 when the first intervention ever reported in history was performed on St. Ignatius of Loyola.

PURPOSE: To explore the historical case of Saint Ignatius of Loyola's leg injury and subsequent surgical interventions as a potential early instance of limb lengthening and reconstruction surgery. METHODS: A detailed analysis of "A Pilgrim's Journey" (Ignatius of Loyola's autobiography) was conducted, focusing on orthopaedic descriptions of his injury and treatments. RESULTS: In 1521, Íñigo López de Loyola sustained a severe, comminuted open fracture of the tibia due to a cannonball wound during the siege of Pamplona. Initial attempts at reduction were unsuccessful, leading to a non-union wi

Open reduction and internal fixation compared to closed reduction and external fixation in distal radial fractures: a randomized study of 50 patients.

BACKGROUND AND PURPOSE: In unstable distal radial fractures that are impossible to reduce or to maintain in reduced position, the treatment of choice is operation. The type of operation and the choice of implant, however, is a matter of discussion. Our aim was to investigate whether open reduction and internal fixation would produce a better result than traditional external fixation. METHODS: 50 patients with an unstable or comminute distal radius fracture were randomized to either closed reduction and bridging external fixation, or open reduction and internal fixation using the TriMed system

Articular fractures of the distal humerus.

Distal humeral fractures represent 2% of all adult elbow fractures. Injury mechanisms include high-energy trauma with skin involvement, and low energy trauma in osteoporotic bone. Treatment goals are anatomical restoration in young, high-demand patients and quick recovery of activities of daily living in the elderly. Complete fractures are relatively easy to diagnose, but partial intra-articular fractures are not. The clinical diagnosis must take into account potential complications such as open injuries and ulnar nerve trauma. Standard X-rays with additional distraction series in the operatin

Sideswipe elbow fractures.

A retrospective review of all cases of sideswipe elbow fractures (SSEFs) treated at two community hospitals from 1982 to 1992 was conducted to determine the functional outcome of the operative treatment of SSEFs. All five injuries involved the left elbow, and they included open fractures of the olecranon, the radius and ulna, the ulna and humerus, the humerus, and traumatic amputation of the arm. Concomitant injuries included three radial nerve palsies and two injuries each to the median nerve, ulnar nerve, and brachial artery. Treatment included irrigation, debridement (repeated if necessary)

Dual Fixation in Complex Long Bone Fractures: A Case Series of 30 Patients.

INTRODUCTION: Complex long bone fractures characterized by comminution, segmental patterns, metaphyseal extension, or associated joint instability pose significant challenges to stable fixation. While intramedullary nailing remains the standard treatment for diaphyseal fractures, certain fracture configurations are prone to mechanical failure, delayed union, or non-union when treated with a single fixation modality. Dual fixation using a combination of intramedullary nailing and plate augmentation may provide superior biomechanical stability and improve healing outcomes. OBJECTIVES: To evalua

Loose-fit vs. press-fit stems and risk for surgical reintervention following radial head arthroplasty: a US-based cohort study of 1575 patients.

BACKGROUND: Radial head arthroplasty (RHA) is performed with increasing frequency for reconstruction of comminuted radial head fractures. Implants can be categorized by stem design, either loose fit or press fit. Currently, the RHA literature does not suggest one implant type is superior to another based on revision and reoperation rates, although most RHA outcome studies have small numbers of patients with few events to detect a difference. This study evaluated the association between stem design and risk of revision and reoperation after RHA. METHODS: A total of 1575 patients aged ≥18 years

[Defect of the femoral condyle in the knee after patellectomy. Long-term results of fresh massive osteochondral allografting].

The presence of a massive full-thickness osteochondral defect in the knee of young and active individuals is an unsolved problem in orthopedic surgery, especially in post-traumatic large bone defects. Fresh massive osteochondral allografts have been used for many years but mostly in oncology but not in post-traumatic cases. This case report describes a 20-year-old right leg-dominant woman, who, at age 19, sustained open Gustilo-Anderson type III comminuted fractures of the left patella and lateral femoral condyle in a motorbike accident. Initial treatment included immediate débridement and pat

A Comparison of Vitamin D Levels and Hip Fracture Severity in Elderly Patients With and Without Type 2 Diabetes Mellitus: A Retrospective Clinical Study.

INTRODUCTION: Evidence shows that poor glycemic control and diabetes are strongly associated with poor bone quality and fragility fractures. This study was conducted to record vitamin D (VD) levels and assess hip fracture severity in elderly hip-fractured patients with and without type 2 diabetes mellitus (T2DM). METHODS: We examined 114 patients over 65 years old with low-energy hip fractures, classified as extracapsular or intracapsular. Severe fractures were defined by Garden's classification for subcapital fractures and the AO/Orthopaedic Trauma Association classification for intertrochan

Stabilizing the Unstable: Cone Hemiarthroplasty in Geriatric Intertrochanteric Fractures.

BACKGROUND: Unstable intertrochanteric fractures in the elderly pose a significant treatment challenge due to poor bone quality, comminution, and associated comorbidities. Bipolar hemiarthroplasty offers the advantage of early mobilization and reduced fixation-related complications. This study evaluates short-term outcomes of bipolar hemiarthroplasty using an uncemented titanium fluted, tapered cone femoral prosthesis in such fractures. METHODS: A retrospective analysis was conducted on 43 consecutive elderly patients treated with uncemented bipolar hemiarthroplasty using a tapered, fluted co
