Maslen et al. (2013) have provided us with a comprehensive overview of the current legislation regulating non-clinical cognitive enhancement devices (CEDs) in the European Union and have proposed a specific model whereby CEDs would be regulated in the same way as medical devices. An alternative model would be to require manufacturers to quantify risks only. Irrespective of the purported 'benefits' of a product, this would allow the consumer freedom of choice to use the product at their will and allow the periodic review of worthwhile indications and unexpected adverse events. Although this dep

Surrogate markers have enormous potential for contributing to the diagnosis, prognosis, and therapeutic evaluation of acute brain damage, but extensive prior study of individual candidates has not yielded a biomarker in widespread clinical practice. We hypothesize that a panel of neuron-enriched proteins measurable in cerebrospinal fluid (CSF) and blood should vastly improve clinical evaluation and therapeutic management of acute brain injuries. Previously, we developed such a panel based initially on the study of protein release from degenerating cultured neurons, and subsequently on rodent m

The metabolic processes within the brain are reflected in the cerebrospinal fluid (CSF). It is in close contact with the nervous system, which is both target and source of multiple steroids. The aim of our study was to develop and validate robust, sensitive LC-MS/MS methods with and without derivatization step for the analysis of unconjugated steroids from all major steroid classes in CSF. The validation of the method without derivatization was performed for ten C19- steroids (dehydroepiandrosterone (DHEA), 7α-hydroxyDHEA, 7β-hydroxyDHEA, 7-ketoDHEA, testosterone, epitestosterone, dihydrotesto

Colloid cysts of the third ventricle are rare, benign intracranial lesions that can produce a broad spectrum of symptoms that depend on both the degree and rate of cerebrospinal fluid (CSF) obstruction. When the presentation is insidious, these lesions may at first mimic idiopathic intracranial hypertension (IIH), particularly in young women who may first develop headaches and papilledema without any focal neurological deficits on exam. We describe the case of a 24-year-old woman with several months of non-positional headaches accompanied by intermittent nausea, photophobia, and brief episodes

Headache is a frequent symptom in sickle cell disease (SCD) that usually is attributable to anemia or cerebrovascular disease. We report 3 pediatric patients with SCD (1 patient with SCD-SC and 2 patients with SCD-SS) who presented with headache and were diagnosed with pseudotumor cerebri (PC). All 3 patients had elevated opening pressures during a lumbar puncture with normal cerebrospinal fluid studies. Magnetic resonance imaging revealed no evidence of hydrocephalus or arteriopathy in all 3 cases. Magnetic resonance venograms performed in 2 of the patients at diagnosis revealed no evidence o

A rare case of an abscess in the cavum septi pellucidi (CSP) is described and previously reported cases are reviewed. A 60-year-old male was admitted to the hospital because a diagnosis of cerebellar hemisphere infarction was made on CT scan. Seven years earlier, the patient had undergone a craniotomy for aneurysm clipping, and a ventriculo-peritoneal shunt was installed for normal pressure hydrocephalus 14 days after the aneurysmal rupture. On his second hospitalization CT scan also demonstrated CSP but this was not associated with ventriculomegaly. He was placed on a rehabilitation regimen a

In many instances patients who have undergone neurosurgery are given antibiotics. Some of these patients, however, run the risk of developing pseudomembranous colitis. In our department over the past three years, 239 patients, whose hospitalization period exceeded two weeks, were given antibiotics. Of this total number, 6 patients (2.5%) contacted pseudomembranous colitis and a clinical study of these 6 cases was conducted from a neurosurgical viewpoint. This study concentrated on diarrhea, the primary symptom, and the 6 patients consisted of 1 male and 5 females whose ages ranged from 61 to 7

BACKGROUND: Sporadic cerebral small vessel disease (cSVD) is the most common cause of vascular dementia. cSVD features a wide range of neuroimaging markers, including white matter hyperintensity (WMH), lacunes, perivascular spaces, microbleeds, and diffusion magnetic resonance imaging (MRI)-based markers of white matter (WM) injury, and is accompanied by progressive cognitive decline. However, few studies have systematically investigated how and when various imaging and cognitive markers change along the complete cSVD progression trajectory.
AIMS: To establish changes in imaging and cognitive

OBJECTIVE: To assess the utility, accessibility, and equivalence to supervised scales of online cognitive assessment in older individuals with cognitive impairment.
METHODS: Patients with Alzheimer's disease (AD, n = 31), idiopathic normal pressure hydrocephalus (iNPH, n = 26), and traumatic brain injury (TBI, n = 23) completed online cognitive tasks (Cognitron). We evaluated cognition relative to a large normative dataset (N ≈ 400,000), adjusting for device and demographics which can affect performance. Principal Component Analysis (PCA) was used to derive domain-specific and total composite

RATIONALE: Cobalamin C (cblC) deficiency is one of the most common congenital vitamin B12 metabolic abnormalities, and may cause severe neurologic symptoms, gastrointestinal and nephritic symptoms.
PATIENT CONCERNS: A 9-month-old boy presented with a 10-day history of progressive dyspnea and weak cough, accompanied by moaning, perioral cyanosis, and poor feeding. The parents also reported significant developmental regression and delay, characterized by an inability to raise his head, sit independently, or vocalize "dada" and "mama": milestones typically achieved by this age.

BACKGROUND AND OBJECTIVES: Laparoscopy is a minimally invasive alternative to laparotomy for ventriculoperitoneal shunt. However, recent evidence has renewed the debate over which technique is preferable. Therefore, we aimed to conduct a meta-analysis to compare these procedures.
METHODS: We searched PubMed, Embase, Web of Science, and Cochrane Library for studies comparing laparoscopy with laparotomy in patients undergoing ventriculoperitoneal shunt until August 2024. Outcomes included overall, distal, and proximal shunt failures; intraoperative complications; operation time; length of stay;

BACKGROUND: Cognition and frailty are sparsely studied in patients with idiopathic normal pressure hydrocephalus (iNPH). We aimed to describe the preoperative cognitive function compared with normative data and frailty profile in iNPH patients accepted for shunt surgery.
METHODS: All patients were diagnosed according to international guidelines and underwent a standardized cognitive and physical examination and a geriatric assessment prior to surgery. Z-scores for the cognitive tests were calculated based on age and education adjusted population norms.
RESULTS: The study cohort included 276

OBJECTIVE: The cingulate sulcus sign (CSS) has been observed in patients with idiopathic normal pressure hydrocephalus (iNPH), suggesting potential disruptions in cerebrospinal fluid circulation and compromised glymphatic system. Although there are similarities in the underlying mechanisms between cerebral small vessel disease (CSVD) and iNPH, the relationship between CSS and CSVD remains unclear. This study aimed to investigate the prevalence and potential mechanisms of CSS in patients with CSVD.
METHODS: Data from patients diagnosed with CSVD at Shengjing Hospital of China Medical Universit

Although the central canal is an integral component of the cerebral ventricular system, central canal dilation has not been examined adequately during the progression of subarachnoid hemorrhage-related hydrocephalus (SAH-H). Central canal dilation-associated ependymal cell desquamation or subependymal membrane rupture has been rarely reported. Herein, we try to describe possible mechanisms of central canal dilation "Hydromyelia," developing after SAH. A total of 25 New Zealand hybrid female rabbits were recruited. Five served as controls, and five received sham operations. In the remaining ani

BACKGROUND: Abnormal cardiac uptake of 123I-metaiodobenzylguanidine (123I-MIBG) is a diagnostic marker of Lewy body diseases (LBDs), e.g., Parkinson's disease (PD) and dementia with Lewy bodies (DLB). Planar imaging is generally used to assess cardiac sympathetic dysfunction in 123I-MIBG scintigraphy; however, its clinical utility requires further improvement. We hypothesized that the co-registration of single-photon emission tomography (SPECT) and computed tomography (CT) images would improve the diagnostic accuracy of 123I-MIBG cardiac scintigraphy for LBDs. This study sought to evaluate the

BACKGROUND: α-Synuclein aggregation is a pathological hallmark of synucleinopathies, including Parkinson's disease (PD) and multiple system atrophy (MSA). Although pathogenic α-synuclein seeds can be detected by real-time quaking-induced conversion (RT-QuIC), current assays primarily rely on cerebrospinal fluid, whose invasiveness limits repeated and large-scale applications. We therefore evaluated the feasibility of detecting pathogenic α-synuclein using urine-derived neuronal extracellular vesicles (NEVs).
METHODS: Urine-derived NEVs were enriched by direct immunoprecipitation using L1CAM-c

BACKGROUND: Phase-contrast MRI of CSF and spinal cord dynamics has evolved among diseases caused by altered CSF volume (spontaneous intracranial hypotension, normal pressure hydrocephalus) and by altered CSF space (degenerative cervical myelopathy (DCM), Chiari malformation). While CSF seems to be an obvious target for possible diagnostic use, craniocaudal spinal cord motion analysis offers the benefit of fast and reliable assessments. It is driven by volume shifts between the intracranial and the intraspinal compartments (Monro-Kellie hypothesis). Despite promising initial reports, comparison

Na+,K(+)-ATPase density in human cerebral cortex was for the first time studied by vanadate facilitated [3H]ouabain binding to intact samples. Fresh human cerebral cortical biopsies were obtained as a result of diagnostic frontal lobe biopsy from patients with normal pressure hydrocephalus (NPH) syndrome and associated dementia. For control measurements post-mortem samples were obtained from patients without clinically observed dementia. [3H]ouabain binding kinetics were evaluated: when incubating samples in 1 microM [3H]ouabain binding equilibrium was obtained after 6 h of incubation, non-spe
