BACKGROUND: Depressive mixed state (DMX), defined by hypomanic features during a major depressive episode (MDE) is under-researched. Accordingly, study aims were to find DMX prevalence in unipolar major depressive disorder (MDD) and bipolar II depressive phase, to delineate the most common hypomanic signs and symptoms during DMX, and to assess their sensitivity and specificity for the diagnosis of DMX and bipolar II.
METHODS: 161 unipolar and bipolar II MDE psychotropic drug- and substance-free consecutive outpatients were interviewed during an MDE with the Structured Clinical Interview for D

Very low birth weight (VLBW) premature infants experience numerous, often self-limited non-bradycardic episodes of intermittent hypoxemia (IH). We hypothesized that these episodes of IH affect postnatal white matter (WM) development causing hypomyelination and neurological handicap in the absence of cellular degeneration. Based on clinical data from ten VLBW neonates; a severity, daily duration and frequency of non-bradycardic IH episodes were reproduced in neonatal mice. Changes in heart rate and cerebral blood flow during IH were recorded. A short-term and long-term neurofunctional performan

BACKGROUND AND OBJECTIVES: Cerebral small vessel disease (SVD) is the most common vascular contributor to dementia. SVD markers often coexist, contributing to difficulty assessing their independent contributions to cognitive domains. MRI-visible perivascular spaces (PVSs) are an emerging SVD marker visualized by MRI. We previously showed that basal ganglia PVSs cross-sectionally contribute to worse cognition, independent of other SVD markers. To further characterize the clinical relevance of PVS, we studied its role as a unique SVD marker of longitudinal cognitive decline.

Objective: Accumulation of cerebral amyloid-β (Aβ) is a risk factor for cognitive decline and defining feature of Alzheimer's disease (AD). Aβ is implicated in brain network disruption, but the extent to which these changes correspond with observable cognitive deficits in pre-clinical AD has not been tested. This study utilized individual-specific functional parcellations to sensitively evaluate the relationship between network connectivity and cognition in adults with and without Aβ deposition. Participants and Methods: Cognitively unimpaired adults ages 45-85 completed amyloid positron emiss

BACKGROUND AND OBJECTIVES: To assess the clinical practice applicability of autoimmune encephalitis (AE) criteria (2016).
METHODS: Medical records of 538 adults diagnosed with AE or related autoimmune encephalopathy at Mayo Clinic (not including pure movement disorders) were reviewed and AE guideline criteria applied.
RESULTS: Of 538 patients, 288 were male (52%). The median symptom onset age was 55 years (range, 11-97 years; 16 had onset as children). All had other non-AE diagnoses reasonably excluded. Of 538 patients, 361 (67%) met at least possible criteria, having all 3 of subacute onset

The ability to mentally travel forward through time allows humans to envisage a diverse array of possible events taking place in the future, helping us to choose which pathway to take in life. In epilepsy, we assume that patients use this cognitive ability when deciding between various treatment options, but this assumption has not been robustly tested. The temporal lobes are key contributors to this 'future thinking' and its building blocks include cognitive functions commonly impaired in temporal lobe epilepsy such as memory and language, giving rise to a hypothesis that 'future thinking' is

BACKGROUND AND OBJECTIVE: Patients with dementia with Lewy bodies perform worse than those with Alzheimer disease (AD) on tests of visual perception, but the clinical utility of these tests remains unknown because studies often had clinically diagnosed groups that may inadvertently cross-contaminate Lewy body disease (LBD) with pure AD pathology, used experimental tests not easily adaptable for clinical use, and had no way to examine relationships between the severity of LBD pathology and degree of cognitive impairment. Therefore, we sought to determine whether performance on a widely used cli

IMPORTANCE: Worldwide, 47 million people live with dementia and, by 2050, the number is expected to increase to 131 million.
OBSERVATIONS: Dementia is an acquired loss of cognition in multiple cognitive domains sufficiently severe to affect social or occupational function. In the United States, Alzheimer disease, one cause of dementia, affects 5.8 million people. Dementia is commonly associated with more than 1 neuropathology, usually Alzheimer disease with cerebrovascular pathology. Diagnosing dementia requires a history evaluating for cognitive decline and impairment in daily activities, wi

OBJECTIVES: There is much evidence that shows that a substantial number of individuals with DSM-IV-defined unipolar depression (UP) manifest hypomanic sub-syndrome and bipolar diathesis. Other definitions have conceptualized the term soft bipolar spectrum (SBP) for these individuals. Little is known about the cognitive profiles of individuals with SBP. We hypothesized that they are representative of individuals with bipolar II disorder and are different from that of 'strict' UP.
METHODS: Consecutive referrals suffering major depressive episodes were categorically assigned to groups of either

BackgroundCognitive impairment (including Alzheimer's disease), frailty, and hypertension are interlinked challenges in older adults. Among non-frail adults, prefrail older adults represent a crucial window for prevention. However, whether systolic blood pressure (SBP) affects cognitive decline differently across age-frailty strata, or what constitutes an optimal age-frailty-stratified target, remains unclear.ObjectiveTo investigate the association of SBP with annual cognitive aging rate in non-frail adults (robust/prefrail, ≥45years), focusing on nonlinearity and modification by age-frailty s

OBJECTIVES: Postoperative delirium (POD) is a common complication after cardiac surgery in older patients, associated with higher mortality and prolonged stays. Continuous theta-burst stimulation (cTBS), a noninvasive neuromodulation technique, has been shown to modulate cortical excitability, offering potential as a treatment for cognitive dysfunction after surgery. The posterior cingulate cortex (PCC) is strongly involved in resource allocation, episodic memory, and executive function. This study aims to investigate the efficacy of cTBS on PCC in reducing POD in older patients having cardiac

Dietary (poly)phenols and methylxanthines may help lower the risk of developing neurodegenerative diseases. However, the relationship between (poly)phenols and methylxanthines and age-related cognitive impairment has not been fully explored, in particular (poly)phenol-rich diets using biomarkers. A cross-sectional analysis was conducted on 202 participants aged 60 - 80 diagnosed with mild cognitive impairment (MCI) or two or more cardiometabolic disorders (CMD). The associations between metabolic signature/metabolites, cognition, and cardiometabolic risk were assessed using linear models, adju

The formation of associations, which involves binding disparate pieces of information, is fundamental to constructing episodic memory. This process primarily relies on the neural circuitry within the medial temporal lobe, specifically the hippocampal-parahippocampal network. Within this network, the perirhinal cortex (PER) and the hippocampus (HPC) are recognized as essential components for associative processing. While the traditional dual-pathway model depicts a hierarchically organized, sequential transmission of information along the medial temporal lobe, recent anatomical and functional s

OBJECTIVE: Postoperative delirium (POD) is common after cardiac surgery, linked to prolonged hospitalization and adverse outcomes. Despite its clinical impact, objective preoperative neuroimaging markers with robust POD predictive value remain elusive. This study explored whether preoperative regional homogeneity (ReHo) alterations correlate with POD and validated their predictive utility, with a focus on heart-brain interaction implications.
METHODS: In this prospective observational study, resting-state functional MRI data were collected from patients undergoing valve replacement surgery wi

INTRODUCTION: Refractory status epilepticus (RSE) is a serious neurological emergency in children associated with significant mortality and morbidity. General anesthetics (GAs) are routinely used to treat RSE. However, the optimal selection of GAs for pediatric RSE remains unknown.
OBJECTIVE: We aimed to identify the optimal agent by comparing the efficacy and safety of different GAs in a rat model of RSE.
METHODS: Sprague-Dawley rats at postnatal day 20 were subjected to lithium-pilocarpine-induced SE. Animals that developed RSE (pharmacoresistant to the first- and second-line therapies) we

BACKGROUND: Prenatal alcohol exposure (PAE) is the major cause of developmental disability, ultimately resulting in a wide range of long-term health consequences across the lifespan. PAE adults are at increased risk for metabolic, immune, and cardiovascular disease, and neurocognitive disabilities. However, little is known about the contribution of PAE to adult-onset cerebrovascular disease, specifically ischemic stroke during middle age, a time when stroke incidence and severity typically increase. We hypothesized that PAE would exacerbate both acute consequences and long-term cognitive impai

OBJECTIVE: Apathy and depression are both common after moderate to severe traumatic brain injury (TBI) and may be especially important to distinguish in older adults with TBI. The authors examined apathy and depression in relation to cognitive performance domains and their potentially unique contribution to psychosocial functioning in this patient population.
METHODS: A total of 106 participants (mean±SD age=64±8 years) with chronic moderate to severe TBI (≥1 year) completed questionnaires assessing severity of apathy (Frontal Systems Behavior Scale-apathy subscale) and depression (Geriatric

BACKGROUND: Diffuse axonal injury (DAI) is a frequent mechanism of traumatic brain injury (TBI) that triggers a sequence of parenchymal changes that progresses from focal axonal shear injuries up to inflammatory response and delayed axonal disconnection.
OBJECTIVE: The main purpose of this study is to evaluate changes in the axonal/myelinic content and the brain volume up to 12 months after TBI and to correlate these changes with neuropsychological results.
METHODS: Patients with DAI (n = 25) were scanned at three time points after trauma (2, 6, and 12 months), and the total brain volume (TB
