Impact of housing conditions on metabolic outcomes in high-fat diet-fed Mice.

Spontaneous physical activity (SPA) represents a behavioral component contributing to energy dissipation and metabolic balance. However, it remains untested whether a cage-dimension-induced increase in SPA can mitigate the deleterious effects of high-fat diet-induced obesity in mouse models. This study aimed to evaluate metabolic outcomes in C57BL/6 mice housed in small (SC) or large cages (LC) and fed either AIN-93 M (control diet; CD) or a lard-enriched high-fat diet (HFD). Differential housing and diets began at 40 days of age (adolescent phase) and were maintained for 8 weeks. While animal

Aggressive Resection of Malignant Paraaortic and Pelvic Tumors Accompanied by Arterial Reconstruction with Synthetic Arterial Graft.

BACKGROUND Advanced malignancies in the lower abdomen easily invade the retroperitoneal and pelvic space and often metastasize to the paraaortic and pelvic lymph nodes (LNs), resulting in paraaortic and/or pelvic tumor (PPT). CASE REPORT A total of 7 cases of aggressive malignant PPT resection and orthotopic replacement of the abdominal aorta and/or iliac arteries with synthetic arterial graft (SAG) were experienced during 16 years. We present our experience with aggressive resection of malignant PPTs accompanied by arterial reconstruction with SAG in detail. The primary diseases included 2 ca

Single-stage total aortic replacement in patients with mega-aortic syndrome.

BACKGROUND: Staged total aortic replacement (TAR) is standard for patients with mega-aortic syndrome (MAS) and severe comorbidities, but a single-stage approach may be better for younger and fit patients. This report described the mid-term results of this approach. METHODS: We conducted a retrospective medical chart review of all MAS patients in our center between May 2016 and December 2020 to analyze outcomes of single-stage TAR. Primary endpoints were mortality and major adverse postoperative events; secondary endpoints included aortic re-intervention, all complications, and survival. RESU

Superior mesenteric artery branch pseudoaneurysm rupture mimicking acute pancreatitis in a patient with acute type B aortic dissection: A case report.

RATIONALE: Visceral arterial pseudoaneurysms are rare but important vascular entities because of their inclination to cause life-threatening hemorrhage. They were commonly reported to be associated with trauma, infection, inflammatory disease, or occurred as postoperative complication. To date, there has been no published report of a superior mesenteric artery (SMA) branch pseudoaneurysm rupture mimicking acute pancreatitis in a patient with acute type B aortic dissection. PATIENT CONCERNS: The patient's medical history, clinical information, imaging findings including follow-up computed tomo

Transperitoneal laparoscopic repair of retrocaval ureter: Our experience and review of literature.

CONTEXT AND AIM: Retrocaval ureter (RCU), also known as circumcaval ureter, occurs due to anomalous development of inferior vena cava (IVC) and not ureter. The surgical approach for this entity has shifted from open to laparoscopic and robotic surgery. This is a relatively new line of management with very few case reports. Herein, we describe the etiopathology, our experience with six cases of transperitoneal laparoscopic repair of RCU operated at tertiary care center in India and have reviewed different management options. METHODS: From 2013 to 2016, we operated total six cases of transperit

Distribution and characteristics of hematogenous disseminated tuberculosis within the abdomen on contrast-enhanced CT.

BACKGROUND: The incidence of abdominal tuberculous lymphadenopathy never appears low. In addition to infecting the abdominal lymph nodes through the digestive tract, the tuberculosis bacteria can also infect the abdominal lymph nodes and extra-nodal organs through hematogenous dissemination. This study investigated contrast-enhanced CT features and anatomic locations of hematogenous disseminated tuberculosis involving abdominal organs. METHODS: Thirteen patients with documented hematogenous disseminated tuberculosis involving abdominal organs were recruited. Contrast-enhanced CT scanning was

Back-Table Reduction of a Deceased Adult Donor Kidney for Transplantation Into a Small Child: A Six-Year Follow Up.

BACKGROUND: Transplantation of adult-sized kidneys into very small pediatric recipients can be limited by pronounced donor-recipient size mi … bilateral nephrectomy. The 14-cm graft had three arteries of comparable caliber. Because the right retroperitoneal space was insufficient to accommodate the graft, the upper pole, supplied by an independent polar artery, was resected ex vivo on the back table, reducing the graft volume by approximately one-third. The remnant kidney was implanted extraperitoneally with …

Posterior Intradiscal Osteotomy With Posterior Approach Anterior Column Release at L5-S1 for Rigid Flat Back Deformity: A Single-Approach Alternative to ALIF or PSO for Sagittal Balance Restoration.

BACKGROUND AND IMPORTANCE: Restoring distal lumbar lordosis at a fused L5-S1 junction is challenging in adult spinal deformity surgery, particularly in revision cases with limited anterior access. Posterior intradiscal osteotomy (IDO) with posterior approach anterior column release (P-ACR) provides a single-posterior approach for mobilizing the anterior column and restoring distal lumbar lordosis without the risks of retroperitoneal exposure. This report details the operative nuances and technical sequence for L5-S1 IDO with P-ACR and summarizes early outcomes from complex revision cases. CLI

[Laparoscopic ileal ureteral replacement for ureteral avulsion].

INTRODUCTION: Ureteral avulsion is a serious complication of ureteroscopy and may require reconstructive procedures on the upper urinary tra … atients. In 4 patients, ureteral avulsion occurred in other clinics: three underwent exploration of retroperitoneal space with insertion of nephrostomy tube, and in one case the avulsed ureter was repositioned and anastomosed to its proximal end. In our institution, ureteral avulsion occurred in two patients; both underwent emergency ileal ureteroplasty, whereas the re …

Involvement of transverse mesocolon is associated with development of colonic fistula in patients with acute necrotizing pancreatitis.

BACKGROUND: Involvement of transverse mesocolon (TM) during acute necrotizing pancreatitis(ANP) indicates that inflammation has spread from retroperitoneal space to peritoneum. Nevertheless, the impact of TM involvement, as confirmed by contrast-enhanced computed tomography (CECT), on local complications and clinical outcomes was poorly investigated. PURPOSE: This study aimed to explore the association between CECT-diagnosed TM involvement and the development of colonic fistula in a cohort of ANP patients. METHODS: This is a single-center, retrospective cohort study involving ANP patients ad…

Successful robot-assisted laparoscopic resection of pheochromocytoma in a patient with dilated cardiomyopathy: A case report on extremely high-risk anesthesia management.

RATIONALE: Anesthetic management during resection of pheochromocytoma is a huge challenge, especially when accompanied by dilated cardiomyopathy (DCM). However, there is a lack of research evidence in this area. PATIENT CONCERNS: A 36-year-old man was admitted with a left retroperitoneal space-occupying lesion, present for 2 years. The patient also had DCM for 2 years. Blood analysis on admission showed elevated levels of norepinephrine and the N-terminus of the brain natriuretic peptide precursor. Abdominal computed tomography revealed a circular shadow in the left adrenal area. Echocardiogr…

Techniques to perform robotic left adrenalectomy in the obese patient.

INTRODUCTION: Minimally invasive adrenalectomy may be associated with reduction in postoperative pain, morbidity, and length of stay and, as a result, has become a preferred approach for many adrenal tumors. Left-sided adrenal tumors, however, are particularly challenging to address in the morbidly obese patient due to difficulties in maintaining exposure and dissection. The robotic platform offers instruments with greater degrees of freedom that aid in retraction and dissection, especially of the adrenal vein, but fixed patient positioning and the large distance needed between patient ports t
