Quality of perinatal depression care in primary care setting in Nigeria.

BACKGROUND: Even though integrating mental health into maternal and child health (MCH) is widely accepted as a means of closing the treatment gap for maternal mental health conditions in low- and middle-income countries (LMIC), there are not many studies on the quality of the currently available mental health care for mothers in these countries. This study assessed the existing organization of service for maternal mental health, the actual care delivered for perinatal depression, as well as the quality of the care received by affected women presenting to primary care clinics in Ibadan, Nigeria

The Long-Term Effects of Maternal Postnatal Depression on a Child's Intelligence Quotient: A Meta-Analysis of Prospective Cohort Studies Based on 974 Cases.

OBJECTIVE: Epidemiologists have explored the relationship between maternal postnatal depression (PND) and the intelligence quotient (IQ) of the resulting offspring, but the results remain inconclusive. This study aims to analyze the literature regarding the association between maternal PND and a child's IQ. DATA SOURCES: A search of articles in PubMed, Web of Science, and MEDLINE databases from inception to September 2015 was conducted and supplemented by a manual search of relevant reference lists. The following search terms were used: (postpartum OR postnatal OR puerperal) AND (depression O

Antidepressant treatment for postnatal depression.

BACKGROUND: Postnatal depression is a common disorder that can have adverse short- and long-term effects on maternal morbidity, the new infant and the family as a whole. Treatment is often largely by social support and psychological interventions. It is not known whether antidepressants are an effective and safe choice for treatment of this disorder. This review was undertaken to evaluate the effectiveness of different antidepressants and to compare their effectiveness with other forms of treatment, placebo or treatment as usual. It is an update of a review first published in 2001. OBJECTIVES

Becoming a Mother During COVID-19 Pandemic: How to Protect Maternal Mental Health Against Stress Factors.

During the COVID-19 pandemic, there were an increasing prevalence of perinatal psychiatric symptoms, such as perinatal anxiety, depression, and post-traumatic stress disorders. This growth could be caused by a range of direct and indirect stress factors related to the virus and changes in health, social and economic organization. In this review, we explore the impact of COVID-19 pandemic on perinatal mental health, and propose a range of hypothesis about their etiological mechanisms. We suggest first that the fear of being infected or infected others (intrauterine transmission, passage of the

Peripartum cardiomyopathy: a short review.

Peripartum cardiomyopathy is an unusual form of dilated cardiomyopathy, which manifests as acute heart failure in the last trimester of pregnancy or early postpartum period. Its aetiology is currently unknown. The presenting signs and symptoms are those of congestive heart failure and more specifically those of left ventricular failure. Its importance lies in the fact that it has a high mortality rate and strikes the patient in the prime of life. Peripartum cardiomyopathy has far reaching implications for the anaesthesiologist. The reason for this is that many of the signs and symptoms of norm

The reward system and maternal behavior in an animal model of depression: a microdialysis study.

RATIONALE AND OBJECTIVES: Flinders sensitive line (FSL) rats, an animal model of depression, display a different pattern of maternal behavior compared to Sprague-Dawley (SD) controls. In this study, we examined the rewarding value of mother-infant interaction for FSL dams. MATERIALS AND METHODS: In the main study, we measured monoamine levels in the nucleus accumbens (NAc) of early postpartum FSL and SD dams during an interaction with pups, using the microdialysis technique. In addition, we compared the preference patterns of FSL and SD rats using the conditioned place preference paradigm, wi

Infertility patients and their partners: differences in the desire for twin gestations.

OBJECTIVE: To explore how the individuals within an infertile couple differ from one another in their attitudes toward the conception of twin gestations. METHODS: From November 1999 through February 2000, consecutive couples undergoing treatment at a university-based infertility clinic were invited to participate in a face-to-face interview that ascertained their individual desires for singleton and twin gestations and their perception of the risks associated with these pregnancies. RESULTS: Of the 94 couples approached, 90 (96%) agreed to participate in the study. Patients estimated the inc

[Clinical evaluation of diagnosis and treatment in three cases of primary pulmonary cryptococcosis].

Pulmonary cryptococcosis has been considered as a disease associated with depressed immune function of the host. However, it may develop in healthy individuals without any underlying disease. Recently, we experienced three cases of primary pulmonary cryptococcosis. Case 1. A 30-year-old woman in the sixth week postpartum developed cough and low-grade fever. Chest X-ray showed infiltrative shadows in right S2 and left S6. A diagnosis of pulmonary cryptococcosis was made from transbronchial brushings and histological examination. Case 2. A 51-year-old man was noted to have abnormal shadows on ch

Fatal enteroviral infection in a neonate.

Enterovirus infection has been recognized as one of the most common viral infections in the perinatal and neonatal periods. It frequently leads to significant mortality. One fatal case of neonatal enteroviral infection was experienced in last year. The patient was a one-day-old male, presenting with neonatal sepsis. He has a biphasic illness, first with a mild febrile prodrome then followed by severe systemic involvement, with meningitis, myocarditis, hepatosplenomegaly and disseminated intravascular coagulation. All bacterial cultures were negative, but the rectal swab isolated enterovirus. T

The effect of breast massage combined with co-parenting interventions on breastfeeding in mother-infant separated mothers: a quasi-experimental study.

BACKGROUND: Exclusive breastfeeding is the optimal source of infant nutrition. Mother-infant separation poses a critical barrier to establishing and sustaining lactation, increasing the risk of early breastfeeding cessation. While breast massage supports milk production, the role of partner involvement in this context remains underexplored as a strategy to improve both nutritional and psychosocial outcomes. OBJECTIVE: This study aimed to evaluate the effect of a co-parenting intervention on exclusive breastfeeding rates and related maternal outcomes among postpartum mothers separated from new

Persistent Pain After Cesarean Delivery and Vaginal Delivery: A Prospective Cohort Study.

BACKGROUND: Persistent pain after cesarean delivery and vaginal delivery has been the subject of only a few research articles. The primary outcome of our prospective study was the incidence of persistent pain and its association to mode of delivery. We also studied the nature and intensity of pain after delivery. METHODS: A questionnaire was distributed on postpartum day 2 to 1052 women who had given birth vaginally and to 502 who had undergone cesarean delivery in a tertiary maternity hospital in Helsinki, Finland, in 2010. A second questionnaire was mailed to the women 1 year later. We reco

Interventions for preventing or reducing domestic violence against pregnant women.

BACKGROUND: Domestic violence during pregnancy is a major public health concern. This preventable risk factor threatens both the mother and baby. Routine perinatal care visits offer opportunities for healthcare professionals to screen and refer abused women for effective interventions. It is, however, not clear which interventions best serve mothers during pregnancy and postpartum to ensure their safety. OBJECTIVES: To examine the effectiveness and safety of interventions in preventing or reducing domestic violence against pregnant women. SEARCH METHODS: We searched the Cochrane Pregnancy an

The design, implementation and acceptability of an integrated intervention to address multiple behavioral and psychosocial risk factors among pregnant African American women.

BACKGROUND: African American women are at increased risk for poor pregnancy outcomes compared to other racial-ethnic groups. Single or multiple psychosocial and behavioral factors may contribute to this risk. Most interventions focus on singular risks. This paper describes the design, implementation, challenges faced, and acceptability of a behavioral counseling intervention for low income, pregnant African American women which integrated multiple targeted risks into a multi-component format. METHODS: Six academic institutions in Washington, DC collaborated in the development of a community-w

Impact of a manualized multifocal perinatal home-visiting program using psychologists on postnatal depression: the CAPEDP randomized controlled trial.

CONTEXT: Postnatal maternal depression (PND) is a significant risk factor for infant mental health. Although often targeted alongside other factors in perinatal home-visiting programs with vulnerable families, little impact on PND has been observed. OBJECTIVE: This study evaluates the impact on PND symptomatology of a multifocal perinatal home-visiting intervention using psychologists in a sample of women presenting risk factors associated with infant mental health difficulties. METHODS: 440 primiparous women were recruited at their seventh month of pregnancy. All were future first-time moth

Exploring perinatal depression and its influencing factors: a cross-sectional study among pregnant women and mothers of children under two years in Burera district, Rwanda.

BACKGROUND: Perinatal depression is a common mental health problem, affecting about 10-15% of women during pregnancy and after childbirth. The condition leads to detrimental health impacts on mothers and their children. Despite high prevalence and impact, diagnosis of depression among pregnant women and women with young children is not done routinely in most resource-limited settings. Thus, the scope of perinatal depression remains underestimated. This study assessed perinatal depression and associated factors in pregnant women and mothers of children under 2 years old in rural Rwanda. METHOD
