Symptomatic patients with penetrating atherosclerotic ulcers of the descending thoracic aorta have traditionally been treated by using an open surgical repair. The emergence of thoracic aortic stent-graft technologies has introduced a less invasive treatment option, which often involves covering excess portions of a normal aorta with stent-graft material. We describe the mid-term follow-up of a patient with a symptomatic penetrating atherosclerotic ulcer of the descending thoracic aorta treated with an Amplatzer septal occluder device (AGA Medical Corp, Plymouth, MN), which is typically used f

BACKGROUND: A rapidly increasing number of thoracic aortic lesions are now treated by endoluminal exclusion by using stent grafts. Many of these lesions abut the great vessels and limit the length of the proximal landing zone. Various methods have been used to address this issue. We report our experience with subclavian artery revascularization in association with endoluminal repair of acute and chronic thoracic aortic pathology.
METHODS: Thirty (43%) of 70 patients undergoing thoracic endovascular stent-graft placement from January 2001 to August 2005 had lesions adjacent to or involving the

BACKGROUND: Aortic aneurysm and dissection (AAD) is a life-threatening vascular disease that currently lacks effective pharmacological therapies. Clinical evidences suggest that aspirin may exert a protective effect on AAD, but the underlying molecular mechanism is still unclear at present.
METHODS: Three mouse animal models of AAD and H2O2-induced ferroptosis model in human aortic vascular smooth muscle cells (HAVSMCs) were established. On this basis, RNA sequencing was performed to investigate potential molecular mechanisms, and further experiments were conducted using STING (stimulator of

OBJECTIVE: Proximal ExTension to Induce COmplete ATtachment (PETTICOAT), which uses downstream bare metal stents for structural support, demonstrates potential, yet its adoption is limited by variable outcomes. This study elucidates the potential of PETTICOAT in aortic dissection, emphasizing the determinants that guide patient selection.
METHODS: A retrospective analysis of 60 patients who underwent full PETTICOAT for aortic dissections was conducted. A multivariate logistic regression model identified predictors of favorable aortic remodeling. Patients underwent standardized follow-up with

BACKGROUND AND OBJECTIVES: In personalised external aortic root support (PEARS), a custom-made, macroporous mesh is used to stabilise a dilated aortic root and prevent dissection, primarily in patients with genetically driven aortopathies. Data are needed on the safety and postoperative incidence of aortic events.
METHODS: We present a multicentre cohort study evaluating the first 200 consecutive patients (median age 33 years) undergoing surgery with an intention to perform PEARS for aortic root dilatation in 23 centres between 2004 and 2019. Perioperative outcomes were collected prospectivel

OBJECTIVE: The outcomes of subclavian artery revascularization (SAR) have been examined extensively in the setting of atherosclerotic occlusive disease but have been poorly characterized in the setting of thoracic endovascular aortic repair (TEVAR). As trials for branched thoracic endovascular stent grafts materialize, the outcomes of the subclavian artery branched prosthesis will need to be compared with TEVAR with SAR by carotid-subclavian bypass or subclavian transposition.
METHODS: A database of 1516 patients undergoing TEVAR from 2000 to 2015 was queried. Of those undergoing TEVAR, 19% (

OBJECTIVES: This study sought to evaluate variability in aortic measurements with multiple imaging modalities in clinical centers by comparing with a standardized measuring protocol implemented in a core laboratory.
BACKGROUND: In patients with aortic disease, imaging of thoracic aorta plays a major role in risk stratifying individuals for life-threatening complications and in determining timing of surgical intervention. However, standardization of the procedures for performance of aortic measurements is lacking.
METHODS: To characterize the diversity of methods used in clinical practice, we

OBJECTIVE: Endovascular repair of the aortic arch represents a formidable challenge because of aortic diameter, angulation, elasticity, and greater distance to the femoral access vessels. Whereas both fenestrated and branched endografts have been customized to accommodate complex pathologic processes of the arch, no data comparing the techniques are available. The aim of this study was to compare the outcomes of custom-made fenestrated vs branched thoracic endovascular aortic repair (fTEVAR vs bTEVAR).

OBJECTIVE: Structural changes and incomplete endograft apposition to the aortic arch (bird-beak configuration) after thoracic endovascular aortic repair are poorly understood. The aim of this study was to analyze the morphologic changes, conformability, and angulation factors in patients who underwent stainless steel-based stent graft repair of thoracic aortic pathology.
METHODS: From March 2011 to March 2012, the study enrolled 19 patients with aortic pathology requiring proximal fixation in zones 2 and 3 who underwent stent graft repair using Zenith Pro-Form TX2 stent grafts (Cook Medical,

In this study we evaluated the relationship between the site of the proximal landing zone during endovascular repair of thoracic aortic pathology and treatment outcomes. We reviewed all cases of endovascular repair of thoracic aortic lesions at our institution in the past 42 months. Thirty-seven Talent thoracic endografts were used to treat 20 thoracic aneurysms, 8 intramural hematomas, 6 aortic dissections, and 3 post-traumatic aneurysms. The proximal edge of the covered endograft was situated proximal to the left common carotid artery (zone 1) in 3 patients, between the left common carotid a

BACKGROUND: Antegrade cerebral perfusion (ACP), including unilateral and bilateral, is most commonly used for cerebral protection in aortic surgery. There is still no consensus on the superiority of the two methods. Our research aimed to investigate the clinical effects of u-ACP and b-ACP.
METHODS: 321 of 356 patients with type A aortic dissection were studied retrospectively. 124 patients (38.6%) received u-ACP, and 197 patients (61.4%) received b-ACP. We compared the incidence of postoperative neurological complications and other collected data between two groups. Besides, we also analyzed

Surgery for acute aortic dissection is challenging, especially in cases of cerebral malperfusion. Should we perform only the aortic repair, or should we also reconstruct the arch vessels when they are severely affected by the disease process? Here we present a case of acute aortic dissection with multiple tears that involved the brachiocephalic artery and caused cerebral and right upper-extremity malperfusion. The patient successfully underwent complete replacement of the brachiocephalic artery and the aortic arch during deep hypothermic circulatory arrest, with antegrade cerebral protection.

A 47-year-old male patient presented with aortic dissection originating from an aberrant right subclavian artery. Intraoperative arteriography showed an anomaly of the aortic arch including a common carotid trunk and an aberrant right subclavian artery. An intimal tear was located in the aberrant right subclavian artery. Dissection retrogradely involved the aortic arch and then anterogradely involved the distal aorta. We treated the patient endovascularly with a Wallgraft endoprosthesis deployed in the aberrant right subclavian artery to seal the entry site. Follow-up computed tomography showe
