Cervical pessary placement for prevention of preterm birth in unselected twin pregnancies: a randomized controlled trial.

BACKGROUND: Preterm birth is the leading cause of neonatal death and handicap in survivors. Although twins are found in 1.5% of pregnancies they account for about 25% of preterm births. Randomized controlled trials in singleton pregnancies reported that the prophylactic use of progestogens, cervical cerclage and cervical pessary reduce significantly the rate of early preterm birth. In twin pregnancies, progestogens and cervical cerclage have been shown to be ineffective in reducing preterm birth. OBJECTIVE: The objective of this study was to test the hypothesis that the insertion of a cervica

Necrotising enterocolitis in preterm infants: epidemiology and antibiotic consumption in the Polish neonatology network neonatal intensive care units in 2009.

UNLABELLED: The aim of this study was to describe the epidemiology of necrotising enterocolitis (NEC), antibiotic consumption and the usefulness of microbiological tests in very low birth weight (VLBW) Polish newborns. METHODS: Prospective surveillance was performed in the year 2009 by local infection control teams. The study covered 910 infants hospitalized in six Polish neonatal intensive care units. Two kinds of indicators were used for the description of antibiotic usage: the duration of treatment (days of treatment, DOTs) and the defined daily dose (DDD). RESULTS: NEC incidence was 8.7%

Parenteral-nutrition-associated liver disease after intestinal perforation in extremely low-birthweight infants: consequent lethal portal hypertension.

BACKGROUND: Parenteral nutrition (PN)-associated liver dysfunction (PNALD) in term infants usually manifests as intrahepatic cholestasis, which recovers with enteral nutrition (EN) in most cases; however, as the number of extremely low-birthweight infants (ELBWI) has been increasing, and consequently intestinal diseases associated with ELBWI have been increasing, more intractable PNALD has been encountered after surgical treatment in ELBWI, which does not resolve or rather worsens with EN. METHODS: Three cases of ELBWI with intestinal perforation, which developed PNALD and eventually died of

Risk factors associated with necrotising enterocolitis in very low birth weight infants in Malaysian neonatal intensive care units.

INTRODUCTION: This study aimed to identify the risk factors associated with necrotising enterocolitis (NEC) in very low birth weight (VLBW; weight < 1,501 g) infants in Malaysian neonatal intensive care units (NICUs). METHODS: This was a retrospective study based on data collected in a standardised format for all VLBW infants born in 2007 (n = 3,601) and admitted to 31 NICUs in Malaysian public hospitals. A diagnosis of NEC was made based on clinical, radiological and/or histopathological evidence of stage II or III, according to Bell's criteria. Logistic regression analysis was performed to

Z-shaped anastomosis for the treatment of a benign rectosigmoid stricture.

A girl with malrotation underwent a Ladd's operation at 35 days of age and later had an adhesiotomy at 115 days of age. After the adhesiotomy, she suffered from sepsis and subsequent disseminated intravascular coagulation (DIC). At 178 days of age, she developed an intestinal obstruction because of a rectosigmoid stricture probably caused by DIC-associated ischemia. As a result, an ileostomy was performed. At one year, 4 months of age, she underwent a resection of the proximal part of the rectosigmoid stricture and a reconstruction by means of a Z-shaped anastomosis. Based on our experience, Z

Delay of catch-up growth in very low birthweight infants.

AIM: To determine if appropriate for gestational age, very low birth weight infants demonstrate catch-up growth. METHODS: The notes of 74 appropriate for gestational age, very low birth weight infants were reviewed and anthropometric measures converted into standard deviation scores (Z-scores). The growth of "well" infants was compared with that of "unwell" infants (those who developed necrotising enterocolitis, bronchopulmonary dysplasia or were not feeding at one week of age). RESULTS: All infants showed reduced weight (p<0.001) and head circumference (p<0.01) Z-scores at one week of age.

The bacterial communities of the small intestine and stool in children with short bowel syndrome.

Short bowel syndrome (SBS) presents an increasing problem in pediatrics. SBS often results from surgical resection of necrotic bowel following necrotizing enterocolitis or treatment of anatomic gastrointestinal defects. SBS is associated with significant morbidity and mortality, and creates substantial burdens for patients, families, and the health system. Recent reports have demonstrated that the fecal microbiome of children with SBS is significantly different from healthy control and severe intestinal microbial imbalances is associated with poor growth. We hypothesized that children with SBS

Volvulus of the entire small bowel with normal bowel fixation simulating malrotation and midgut volvulus.

BACKGROUND: Midgut volvulus is a complication of malrotation of bowel and mesenteric malfixation. In contrast, primary volvulus of the small bowel is a distinctly different and rare entity characterized by torsion of the entire small bowel with normal mesenteric fixation. OBJECTIVE: To present the clinical and imaging findings in four infants with primary small bowel volvulus and normal bowel fixation in order to improve awareness of this entity among clinicians and radiologists and to discuss the potential etiologies of this entity to distinguish it from other causes of small bowel volvulus.

Comparison of Feeding, Nutritional, and Procedural Outcomes in Infants with Ductal-Dependent Pulmonary Blood Flow Undergoing Surgical Systemic-to-Pulmonary Artery Shunt Versus Ductal Stenting.

Ductal stenting and surgical systemic-to-pulmonary artery shunting (surgical shunt) are alternative strategies to establish pulmonary blood flow in infants with ductal-dependent circulation. The impact of palliation strategy on feeding, nutrition, and neurodevelopment remains unclear. We conducted a single-center retrospective study of 100 infants with ductal-dependent pulmonary blood flow (2017-2024) undergoing surgical shunt (n = 57) or ductal stenting (n = 43). Outcomes included feeding, nutrition, hospital course, and neurodevelopment. Multivariable logistic regression evaluated factors as

Food and Drug Administration Expert Panel on Infant Formula "Operation Stork Speed" June 2025: Part 3, Marketing of Infant Formulas, Breastfeeding Support, Hypoallergenic Formulas, and Nutrition for Preterm Infants.

The United States formula industry uses aggressive marketing strategies that exploit parental anxieties, undermine breastfeeding, and violate ethical international standards, all of which contribute to early formula introduction and disparities in breastfeeding. In addition, misleading or complex labeling contributes to caregiver and provider confusion regarding the content of formulas. A centralized, Food and Drug Administration-maintained database of infant formulas could support caregivers and healthcare professionals in countering misinformation and making evidence-based decisions. From Powder to Pathogen: A Systematic Review of Detection, Pathogenicity, and Mitigation of Cronobacter sakazakii in Infant Formula.

Cronobacter sakazakii is an emerging foodborne pathogen that has gained increasing global attention due to its association with severe infections in neonates, particularly meningitis, sepsis, and necrotizing enterocolitis. These infections are often associated with contaminated powdered infant formula (PIF), a nonsterile but commonly used alternative to breast milk. We conducted a systematic review using the PRISMA approach and registered it in PROSPERO to synthesize current scientific knowledge on the detection, pathogenic mechanisms, antimicrobial resistance, and mitigation strategies of C.
