Aerodynamic and Manovacuometric Vocal Measurements in Pregnant Women in the Second and Third Trimesters of Pregnancy.

OBJECTIVE: To describe and to correlate the aerodynamic and manovacuometric voice measurements of pregnant women. METHODS: The following evaluations were carried out on 49 pregnant women - between 18 and 40 years old - in the second and third trimesters of gestational age: respiratory muscle strength, maximum phonation time of /a/, habitual, minimum, and maximum sound pressure levels. The data were statistically described and tested for the hypothesis of no association between voice measurements and refence levels, and for the correlation between voice measurements. RESULTS: The maximum phon

Effect of the I/E ratio on CO2 removal during high-frequency oscillatory ventilation with volume guarantee in a neonatal animal model of RDS.

UNLABELLED: The objective of this study was to analyze the effect of I/E ratio on carbon dioxide (CO2) elimination during high-frequency oscillatory ventilation (HFOV) combined with volume guarantee (VG). Five 2-day-old piglets were studied before and after a bronchoalveolar lavage (BAL). The effect of an I/E ratio of 1:1 and 1:2 with (VG-ON) and without VG (VG-OFF) on PaCO2, as well as delta and mean airway pressures at the airway opening (∆Phf-ao, mPaw-ao) and at the tracheal level (∆Phf-t, mPaw-t) were evaluated at frequencies of 5, 8, 11, and 14 Hz. With the VG-ON, PaCO2 was significant lo

Left atrial size, chemosensitivity, and central sleep apnea in heart failure.

BACKGROUND: Central sleep apnea (CSA) is common among patients with heart failure (HF) and is promoted by elevated CO2 chemosensitivity. Left atrial size is a marker of the hemodynamic severity of HF. The aim of this study was to determine if left atrial size predicts chemosensitivity to CO2 and CSA in patients with HF. METHODS: Patients with HF with left ventricular ejection fraction ≤ 35% underwent polysomnography for detection of CSA, echocardiography, and measurement of CO2 chemosensitivity. CSA was defined as an apnea-hypopnea index (AHI) ≥ 15/h with ≥ 50% central apneic events. The rela

Eosinophilic and type 2 inflammation in bronchiectasis: from phenotype recognition toward endotype-informed therapy.

Bronchiectasis is increasingly recognized as biologically heterogeneous, yet the clinical meaning of eosinophilic and type 2 (T2)-associated signals remains uncertain. This review reassesses whether the field has progressed from recognition of an eosinophilic phenotype to a bronchiectasis-specific, therapeutically actionable T2 endotype. Blood eosinophil count (BEC), airway eosinophilia, fractional exhaled nitric oxide, total immunoglobulin E, and pathway-level signatures capture overlapping but non-equivalent features. Current evidence supports an eosinophilic phenotype more strongly than a u

Respiratory gated proton therapy for liver cancer: Setup and gating validation based on fiducial marker motion extracted from 2D kV projection images acquired during cone-beam CT.

PURPOSE: To investigate the use of CBCT projection images for improved setup and gating validation in respiratory gated proton therapy for liver cancer. METHODS: Fifteen consecutive patients receiving proton therapy with respiratory gating guided by an external gating marker block were analyzed. The amplitude gating level was set to 50% duty-cycle around the exhale phase (50th percentile motion). Implanted markers were retrospectively segmented in the 2D projection images of five free-breathing setup CBCT scans per patient and used to estimate the 3D marker trajectories during CBCT acquisitio

Computed tomography-derived pulmonary blood volume distribution may be modifiable in asthma.

BACKGROUND: A smaller relative volume of the peripheral pulmonary vasculature in asthma, termed "vascular pruning", has been linked to greater disease severity, exacerbations, and airflow obstruction. It is not known whether this is reversible. Therefore, the study objective was to determine whether "loss" of computed tomography (CT) measures of peripheral pulmonary vasculature is a modifiable feature of asthma pathophysiology by assessing its response to dupilumab. METHODS: 27 adults with moderate-to-severe asthma received dupilumab (n=19) or placebo (n=8) every 2 weeks for 16 weeks. Chest C

Pre-analytical standardization and multi-biofluid harmonization for liquid biopsy biomarkers in lung cancer: From discovery to clinical laboratory implementation.

Lung cancer remains the leading cause of cancer-related mortality worldwide, with early detection being critical for improving survival outcomes. Liquid biopsy using circulating biomarkers from blood, urine, saliva, sputum, exhaled breath condensate (EBC), and sweat has emerged as a promising non-invasive approach for lung cancer diagnosis, prognosis, and treatment monitoring. However, despite an abundance of biomarker discovery studies, clinical translation has been critically hampered by the absence of standardized pre-analytical protocols, inter-platform analytical discordance, and a lack o

Biological treatments for asthma and chronic obstructive pulmonary disease.

Asthma and chronic obstructive pulmonary disease (COPD) are highly prevalent heterogeneous airway diseases in which a subset of patients with severe disease have a disproportionate share of morbidity and mortality, largely through exacerbations. Major immunological advances have reframed both conditions around endotypes, with type 2 (inflammation driven by type 2 helper cells and group 2 innate lymphoid cells) and other inflammatory pathways defined and targeted with biological treatments. This review provides an overview of biological treatments in asthma and COPD. Biological treatments are e

Thermocycling: enhancing efficiency of anesthetic reflectors through cyclic heating and cooling-finding the optimal temperature in a bench study.

BACKGROUND: Volatile anesthetics are widely used for sedation in intensive care. Being potent greenhouse gases, their efficient utilization is imperative. We investigated whether the efficiency of anesthetic reflection devices, such as Sedaconda ACD-S (ACD), can be improved by thermocycling, i.e., cooling the reflector during expiration and warming it during inspiration, and aimed to identify the optimal temperatures required. METHODS: A test lung connected to the ACD was ventilated under body temperature pressure saturated and normocapnic conditions. Isoflurane and sevoflurane were infused a

Study protocol: a 52-week, multicentre, observational, prospective study on the use of a smart platform connected to a single-inhaler triple therapy (ICS/LABA/LAMA) to evaluate effectiveness on treatment adherence and inhaler technique in patients with poorly controlled asthma.

BACKGROUND: Good adherence to controller medication and correct inhaler technique is essential for effective asthma management. However, many patients struggle with these aspects, leading to poor symptom control and a higher risk of exacerbations. This study (SiASMARTer) aims to evaluate the impact of the SiA®SMARTer platform - a digital smart platform consisting of an app and an add-on module connected to a pressurized metered-dose inhaler delivering triple therapy (ICS/LABA/LAMA) - on improving adherence and inhaler technique in patients with poorly managed inflammation in asthma (defined as

Antenatal Maternal Smoking and Lung Function in Very Prematurely Born Children.

INTRODUCTION: Antenatal exposure to maternal smoking negatively impacts on fetal lung development resulting in infant lung function abnormalities. Lung clearance indices (LCI) were worse in term born infants exposed to antenatal maternal smoking compared to not. Our aim was to assess the effect of antenatal maternal smoking on lung function, in particular LCI, in very prematurely born children. METHODS: Lung function testing was undertaken at 11-14 years, when household members smoking and active smoking were assessed. Antenatal maternal smoking had been recorded. Lung function was assessed b

Phase 2a randomised, double-blind, placebo-controlled study to evaluate the efficacy and safety of londamocitinib (AZD4604) two times per day for 12 weeks in adult patients with moderate-to-severe asthma uncontrolled on medium-high-dose ICS-LABA.

INTRODUCTION: Asthma is a heterogeneous condition and affected individuals show variable responses to available medications. An unmet need exists for add-on therapies that target novel molecular pathways, before patients escalate to biologics. Janus kinase 1 (JAK1) is implicated in multiple inflammatory cytokine pathways critical for the pathogenesis of asthma. Londamocitinib (AZD4604) is a highly specific, inhaled, JAK1 inhibitor with high potency to block multiple JAK1-dependent signalling pathways. AJAX is a phase 2a, randomised, double-blind, partially decentralised placebo-controlled stud
