[Tuberculosis prevention].

UNLABELLED: Tuberculosis prevention is classically based on early detection of potential contagious cases and their effective treatment. But due to new parameters such as migration flows around the world, the emergence of Mycobacterium tuberculosis resistant strains and the increase of the population at risk, screening should be more active and target those who are more vulnerable to developing the disease. Traditional screening methods such as chest X-ray and tuberculin skin test, due to their high sensitivity and low cost, remain valid especially in populations with a high prevalence of the

Evaluating the concordance, prevalence, and risk factors of latent tuberculosis infection among healthcare workers in Makkah: A comparative analysis of TST and IGRA.

BACKGROUND: Latent tuberculosis infection (LTBI) poses a significant occupational health risk for healthcare workers (HCWs), particularly in regions with moderate TB burden. The Tuberculin Skin Test (TST) and Interferon-Gamma Release Assay (IGRA) are widely used for LTBI screening, but their concordance and diagnostic performance remain controversial. This study aims to assess the prevalence of LTBI among HCWs in the Makkah region, evaluate risk factors, and compare the diagnostic accuracy and concordance of TST and IGRA METHODS: A cross-sectional study was conducted among 400 HCWs in Makkah.

Interferon γ release assay for the diagnosis of uveitis associated with tuberculosis: a Bayesian evaluation in the absence of a gold standard.

AIM: To evaluate an interferon γ release assay in the diagnosis of uveitis associated with tuberculosis (TB), in the absence of a gold standard. METHODS: We conducted a prospective 1-year study on consecutive patients with a new diagnosis of uveitis presenting with ocular signs suggestive of TB-associated uveitis (TAU). All subjects underwent ocular and systemic review with investigations, including T-SPOT.TB assay and tuberculin skin test (TST). The main outcome measures were sensitivity and specificity; positive and negative predictive values (PPV, NPV) of T-SPOT.TB were estimated using Bay

Bayesian Modeling of Bovine Tuberculosis Prevalence for Enhanced Estimation and Control in Intensive Dairy Farms: A Longitudinal Decision-Support Tools for Veterinarians and Policymakers.

Accurate prevalence estimation is crucial for effective bovine tuberculosis (bTB) control, as unreliable data often gives rise to strategic failures, misallocation of resources, and loss of stakeholder trust. In an epidemiological study, obtaining true prevalence (Tp) requires accounting for misclassification and measurement errors inherent in diagnostic testing. Given the imperfect specificity and sensitivity of single intradermal comparative tuberculin tests (SICCTT), Bayesian methods present a natural way to propagate these uncertainties. This study uses a Bayesian approach to improve bTB p…

Performance of QuantiFERON-TB Gold In-Tube (QFTGIT) for the diagnosis of Mycobacterium tuberculosis (Mtb) infection in Afar Pastoralists, Ethiopia.

BACKGROUND: Currently, T-cell based gamma interferon (IFNγ) release assays (IGRAs) are acknowledged as the best methods available for the screening of latent tuberculosis infection (LTBI) and also as aid for the diagnosis of active tuberculosis (TB). To our information, the performance of these diagnostic tests has not been evaluated in Ethiopia. Therefore, the intent of this study was to evaluate the performance of QuantiFERON-TB Gold In-Tube (QFTGIT) in patients clinically suspected of active pulmonary TB (PTB) as well as in healthy subjects prior to its utilization for the epidemiological s

Refining cytokine signatures: flow cytometry-based differentiation between TB infection and disease.

BACKGROUND: Distinguishing TB infection (TBI) from TB disease remains difficult because routine tests, including tuberculin skin test and interferon-gamma release assay (IGRA), cannot separate these states. We assessed a flow cytometry-based assay that measures T-cell cytokine responses after stimulation with Mycobacterium tuberculosis. METHODS: We enrolled 118 IGRA-positive participants, including individuals with TB/TBI. Individuals were grouped by age and disease category into paediatric/adult TB, TBI, and immunocompromised TBI. Peripheral blood lymphocytes were stimulated with ESAT-6/CFP-

Mycobacterium tuberculosis-dependent monocyte expression quantitative trait loci, cytokine production, and TB pathogenesis.

INTRODUCTION: The heterogeneity of outcomes after Mycobacterium tuberculosis (Mtb) exposure is a conundrum associated with millennia of host-pathogen co-evolution. We hypothesized that human myeloid cells contain genetically encoded, Mtb-specific responses that regulate critical steps in tuberculosis (TB) pathogenesis. METHODS: We mapped genome-wide expression quantitative trait loci (eQTLs) in Mtb-infected monocytes with RNAseq from 80 Ugandan household contacts of pulmonary TB cases to identify monocyte-specific, Mtb-dependent eQTLs and their association with cytokine expression and clinica

[Latent tuberculosis infection in children in Bangui: about 524 cases exposed at home to index cases of positive microscopy pulmonary tuberculosis].

INTRODUCTION: in endemic areas, despite BCG vaccination, the risk of developing tuberculosis (TB) in young children is high after exposure to adults with tuberculosis. The purpose of this study is to reduce the risk of active tuberculosis in children experiencing household exposure to adult index cases. METHODS: we conducted a cross-sectional multi-site study (April 2016- January 2019) of children aged 0 to 59 months experiencing household exposure to index cases. They were screened and followed up at the pediatric center in Bangui. RESULTS: five hundred twenty four children were included in

Outcomes and complications of hospitalised patients with HIV-TB co-infection.

BACKGROUND: Tuberculosis is one of the most common causes of hospitalisation in patients with HIV. Despite this, hospital outcomes of patients with this co-infection have rarely been described since antiretroviral therapy became widely available. METHODS: Prospective cohort study of HIV-infected adult patients hospitalised with TB in six referral hospitals in Medellin, Colombia, from August 2014 to July 2015. RESULTS: Among 128 HIV-infected patients hospitalised with tuberculosis, the mean age was 38.4 years; 79.7% were men. HIV was diagnosed on admission in 28.9% of patients. The median CD4

Low body mass index and latent tuberculous infection: a systematic review and meta-analysis.

BACKGROUND: The well-documented association between underweight and increased incidence of active tuberculosis (TB) has not been extended to incidence or prevalence of latent tuberculous infection (LTBI). DESIGN: After identifying studies that reported a categorical measure of body mass index (BMI) and used the tuberculin skin test (TST) or QuantiFERON®-TB Gold In-Tube (QFT) to measure LTBI, a maximum likelihood random-effects model was used to examine the pooled association between LTBI and low BMI (<18.5 kg/m2), compared with 1) normal BMI (18.5-25 kg/m2) and 2) a complementary group of all

Twelve-dose weekly rifapentine plus isoniazid for latent tuberculosis infection: A multicentre randomised controlled trial in Taiwan.

UNLABELLED: Treatment of latent tuberculosis (TB) infection (LTBI) effectively prevents its progression to active TB. However, long treatment duration and drug-related hepatotoxicity limit the effectiveness of the 9-month daily isoniazid (9H). Data on the 3-month weekly rifapentine plus isoniazid (3 HP) in Asian populations are currently unavailable. We prospectively randomised the LTBI contacts aged ≥12 years with positive tuberculin skin test into 9H and 3 HP groups in four hospitals between January 2014 and May 2016 in Taiwan. The primary and secondary outcomes were treatment completion rat

Serial testing of healthcare workers for latent tuberculosis infection and long-term follow up for development of active tuberculosis.

BACKGROUND: Healthcare workers (HCWs) are at high risk of tuberculosis (TB) infection due to occupational exposure. It is important to diagnose TB infections in HCWs to prevent nosocomial transmission, particularly among immunocompromised patients. OBJECTIVE: The aim of this study was to analyze the rate of tuberculin skin test (TST) conversion and to assess the incidence of active TB after the latent TB infection screenings in high-risk HCWs. METHODS: This retrospective cohort study involved 458 HCWs in TB-related departments between 2009 and 2013. All HCWs underwent a TST and a chest radio

Latent tuberculosis infection and tuberculosis in patients with rheumatic diseases treated with anti-tumor necrosis factor agents.

The introduction of biological agents, especially the tumor necrosis factor inhibitors (anti-TNF), for the treatment of rheumatic diseases increased the risk of developing tuberculosis (TB). Screening for latent TB infection (LTBI) is strongly recommended before starting therapy with anti-TNF agents. The objective of this study was to identify the prevalence of LTBI and TB among patients with rheumatic diseases on anti-TNF agents. This is a cross-sectional study. The electronic medical records of all adult patients (≥18 years old) undergoing anti-TNF treatment were reviewed. Every patient unde
