Repair of laparoscopic injury to abdominal wall arteries complicated by cutaneous necrosis.

Operative laparoscopic techniques requiring placement of large-bore cannulas through the abdominal wall lateral to the midline result in increased numbers of injuries to abdominal wall vessels. Five cases of inferior epigastric artery hemorrhage were controlled by percutaneous transabdominal placement of polypropylene sutures, allowing the procedure to be completed with the cannula in place. In two patients the sutures were left in place for 72 to 96 hours, and cutaneous necrosis occurred requiring debridement and delayed primary closure. In the other three women, removal of the sutures less t

Laparoscopic injury of abdominal wall blood vessels: a report of three cases.

BACKGROUND: Operative laparoscopy is being used for an increasing number of applications. Many of these innovative techniques require the insertion of large trocars through the anterior abdominal wall at points lateral to the midline. Because of the rich vascular supply of the anterior abdominal wall, the incidence of abdominal wall vessel injuries appears to be increased by these techniques. CASES: Three cases are reported in which abdominal wall blood vessels were injured during operative laparoscopy. Case 1 describes laceration of the inferior epigastric artery above the pubic crest. Despi

Thoraco-abdominal birth injury--presentation, diagnosis and management in an unusual case.

As a result of a traumatic delivery a newborn male child developed thoracic and intra-abdominal problems. Haemoperitoneum and chylous ascites were diagnosed; from a scrotal enlargement secondary to bilateral patent processus vaginalis and a chylothorax became apparent during investigation of the scrotal swellings. We discuss the presentation, diagnosis and management of the case and review the literature.

Avulsion of the ampulla of Vater: an isolated injury following blunt abdominal trauma.

A case of avulsion of the ampulla of Vater from the duodenum due to blunt trauma is presented. In patients in whom leakage is confined to the extra peritoneal tissues, the condition can be insidious, making diagnosis difficult.

Blunt Abdominal Aortic Injury Managed Successfully With Endovascular Stent Graft.

Blunt abdominal aortic injury has a low incidence and accounts for only 4-6% of aortic injuries. Both prognosis and management depend on the type and the severity of injury.A 23 year-old female patient presented after a high speed motor vehicle collision, with a focal dissection and a near complete occlusion of the distal aorta leading to acute limb ischemia, an unstable L1-L2 spinal fracture and a grade 3 liver laceration. An endovascular repair with an aortobiiliac stent graft was performed with resolution of her ischemic lower extremity pain. At 18 months follow-up visit, the aortoiliac axi

Patterns and outcomes of patients with abdominal injury: a multicenter study from Iran.

BACKGROUND: Injury is one of the leading causes of death worldwide, and the abdomen is the most common area of trauma after the head and extremities. Abdominal injury is often divided into two categories: blunt and penetrating injuries. This study aims to determine the epidemiological and clinical characteristics of these two types of abdominal injuries in patients registered with the National Trauma Registry of Iran (NTRI). METHODS: This multicenter cross-sectional study was conducted with data from the NTRI from July 24, 2016, to May 21, 2023. All abdominal trauma patients defined by the In…

Laparoscopic Approach to Severe Liver Injury in a Patient With Blunt Abdominal Trauma.

With laparoscopic surgery becoming more popular in surgical practice, laparoscopic approaches in trauma patients have increased. Non-operative management is the standard treatment algorithm in patients with blunt abdominal trauma who are hemodynamically stable and have sustained a liver injury. However, laparoscopy is a safe and feasible method for exploration, irrigation, and treatment if a surgical intervention is needed in this group. In this study, we aimed to present a case of liver injury in blunt abdominal trauma and its management laparoscopically. A 22-year-old male was admitted to th

Iatrogenic ureteric injury during abdominal or pelvic surgery: a meta-analysis.

OBJECTIVE: To assess the incidence of ureteric injuries, clinical value of prophylactic ureteric stenting and impact of intra- or postoperative detection of ureteric injuries in patients treated with gynaecological or colorectal surgery. METHODS: Multiple databases were searched for articles published before September 2021 according to the Preferred Reporting Items for Systematic Review and Meta-Analyses statement. Studies were deemed eligible if they evaluated the differences in the rate of ureteric injuries between laparoscopic and open surgery, prophylactic ureteric stenting or not, and th

Ureteral Injury by a Retained Knife After Abdominal Trauma: A Case Report.

Isolated ureteral injuries are rare, occurring particularly in gunshot wounds to the abdomen. These are much rarer in the context of stab wounds. These lesions are usually silent. We report a 30-year-old man with a history of abdominal penetrating trauma with a knife, 11 years ago before the actual admission to the urology department. The patient's report describes a retained metallic foreign body in the right lumbar area. At admission, the patient presented with a four-months history of right lumbago. An abdominal computed tomography scan revealed the presence of a right para-renal small urin

Duodenal injury in blunt abdominal trauma. Case report and literature review.

There are few reported cases of small bowel injury due to blunt abdominal trauma. We describe the clinical presentation and surgical management of these lesions. This is the clinical case of a polytraumatized male with a duodenal injury IIID3 according to AAST, who underwent resection of the intestinal segment with duodeno-duodenum anastomosis with favorable results. The infrequent presentation of injuries to the small intestine due to blunt trauma may lead the clinician to overlook the need for intentional interrogation about the kinematics of the trauma, while at the same time neglecting the

Open and Endovascular Abdominal Aortic Injury Repair Outcomes in Polytrauma Patients.

INTRODUCTION: The management of patients with abdominal aortic injury (AAI) remains challenging. Open repair of AAI is still the standard of care although it is associated with high mortality. In past few years, endovascular surgery has evolved as a less invasive alternative to open surgery in emergency settings. The objective of this study was to compare outcomes after open repair versus endovascular repair of AAI in polytrauma patients. METHODS: The National Trauma Data Bank, from 2008 to 2012, was queried to identify trauma patients undergoing open and endovascular repair of AAI using Inte

Acute transfusion-related abdominal injury in trauma patients: a case report.

BACKGROUND: Secondary abdominal compartment syndrome is well known as a life-threatening complication in critically ill patients in an intensive care unit. Massive crystalloid fluid resuscitation has been identified as the most important risk factor. The time interval from hospital admittance to the development of manifest abdominal compartment syndrome is usually greater than 24 hours. In the absence of any direct abdominal trauma, we observed a rapidly evolving secondary abdominal compartment syndrome shortly after hospital admittance associated with massive transfusion of blood products and
