Evolving surgical prosthetic rehabilitation protocol for success of dental implant placed in distracted alveolar ridge.

BACKGROUND: Remodelling of bone in the form of resorption generally follows the extraction of a tooth. During all stages of atrophy of the alveolar ridge, characteristic shapes result from the resorptive process, as influenced by anatomic alterations in the alveolar bone. Various ridge augmentation procedures have been documented as predictable means of establishing new vital bone for implant placement, out of which distraction osteogenesis is one such modality. Hence the following study has been conducted to evolve a surgico- prosthetic rehabilitation protocol in grossly atrophic alveolar rid

Correction of postsurgical alveolar ridge defect with vertical alveolar distraction of the onlay block graft.

Alveolar bone augmentation for dental implant rehabilitation is one of the greatest challenges for oral and maxillofacial surgeons. Bringing out an inadequate quantity of vertical bone during augmentation compromises correct implant positioning and the resulting prosthetic restoration. Alveolar distraction osteogenesis is now generally used in correcting alveolar ridge atrophy due to trauma, congenital defects, or periodontal defects. Onlay block grafting is a suitable method for restoring the alveolar bony defects. However, it sometimes can become a complicated procedure to repair the horizon

Survival of immediately loaded dental implants in deficient alveolar bone sites augmented with beta-tricalcium phosphate.

PURPOSE: Dental implant placement in atrophic alveolar ridges often necessitates grafting procedures, followed by immediate or delayed implant placement. This study assessed the survival of immediately loaded dental implants placed in deficient alveolar bone sites at bone grafting. MATERIALS: From 1999 to May 2002, 1 operator (A.P.) inserted 1065 implants (607 in mandibles, 458 in maxillae) into 338 partially edentulous patients. Most implants were placed into compromised residual ridges or prepared tooth extraction sockets. Implants placed in augmented areas were splinted to implants in nona

Are Bone Expansion Techniques Effective for Implant Placement in Narrow Alveolar Ridges?

Because of the fact that sufficient alveolar ridge bone is essential for the success of implant placement, the authors often face a challenge with augmenting residual maxillary alveolar ridges. The aim of the study was to compare the amount of ridge gain by the Osseodensification method with the manual bone expanders method for healed edentulous sites in the maxilla at different time intervals (immdiate postoperative and 6 mo postoperative). Two groups were equally and randomly divided with a total of 14 implants (Biohorizons) placed in the maxillary arch for 8 patients, osseodensification was

Histological and histomorphometric evaluation of the synthetic biomaterial Natix® in horizontal reconstruction of alveolar ridge.

BACKGROUND: Following loss of teeth, atrophy of alveolar ridge of the jaws is a substantial problem and unintended outcome that compels clinicians to perform bone reconstruction ahead of implant placement. Although autogenous bone is recommended as the gold standard in bone reconstruction, aninvasive second surgery harvestinga limited volume of bone (from intraoral source) has led a significant approachingthe use of synthetic bone substitute materials. The aim of this study was to evaluate the histologic and histomorphometric properties of porous titanium granules (Natix®) used in horizontal r

Improved guided bone regeneration by combined application of unmodified, fresh autologous adipose derived regenerative cells and plasma rich in growth factors: A first-in-human case report and literature review.

BACKGROUND: Novel strategies are needed for improving guided bone regeneration (GBR) in oral surgery prior to implant placement, particularly in maxillary sinus augmentation (GBR-MSA) and in lateral alveolar ridge augmentation (LRA). This study tested the hypothesis that the combination of freshly isolated, unmodified autologous adipose-derived regenerative cells (UA-ADRCs), fraction 2 of plasma rich in growth factors (PRGF-2) and an osteoinductive scaffold (OIS) (UA-ADRC/PRGF-2/OIS) is superior to the combination of PRGF-2 and the same OIS alone (PRGF-2/OIS) in GBR-MSA/LRA. CASE SUMMARY: A 7…

Clinical Outcomes of Titanium Mesh for Alveolar Bone Augmentation: An Umbrella Review.

OBJECTIVES: To critically evaluate the evidence derived from systematic reviews for titanium mesh as a bone augmentation method for alveolar ridge deficiencies. MATERIALS AND METHODS: Five databases were searched for systematic reviews describing any interventions to increase the dimensions of the alveolar ridge using titanium mesh. Included reviews had to report on the primary outcome of vertical and/or horizontal bone gain after augmentation with titanium mesh. The methodological quality of the systematic reviews was assessed with the AMSTAR 2 tool, and the certainty of evidence was evaluat

A retrospective analysis of the resorption rate of deproteinized bovine bone as maxillary sinus graft material on cone beam computed tomography.

OBJECTIVES: Graft resorption remains a major disadvantage of the various employed materials in maxillary sinus augmentation. Our aim was to assess the percentage of graft volume reduction in deproteinized bovine bone (DBB) using cone beam computed tomography (CBCT). MATERIALS AND METHODS: A retrospective analysis of 29 augmented sinuses in 19 patients was conducted. The graft material was either DBB alone or DBB mixed with autologous bone chips (80% DBB/20% bone chips). CBCT scans of the augmented sinuses were obtained pre- and post-operatively up to 2-year follow-up. RESULTS: In total, 55 d

Full block or split block?-Comparison of two different autogenous block grafting techniques for alveolar ridge reconstruction.

OBJECTIVE: To evaluate radiographic bone gain after alveolar ridge augmentation with two different designs of autogenous block graft harvested from the mandible. MATERIALS AND METHODS: Alveolar ridge defects were evaluated by preoperative cone beam computed tomography (CBCT) and grafted in a staged approach using intraoral block grafts. The ridge augmentation was either performed using the full-block technique (group 1) or the split-block technique (cortical plate with autogenous bone chips) (group 2). After 4 months of bone healing, a further CBCT scan was performed before implant placement.…

Objective computerised assessment of residual ridge resorption in the human maxilla and maxillary sinus pneumatisation.

OBJECTIVES: Atrophic resorption of the maxillary alveolar ridge is a complication that makes implantological rehabilitation critical. Our aim was to develop a novel computer aided procedure for the accurate quantitative assessment of maxillary residual ridge resorption including pneumatisation of the maxillary sinus that goes beyond previously described approaches and to apply it to a large dataset. MATERIALS AND METHODS: To develop and refine the method, we performed a retrospective analysis using computed tomography data from 405 patients to generate segmented, three-dimensional models of z

A randomized controlled trial on the impact of healing time on wound healing following ridge preservation using a 70%/30% combination of mineralized and demineralized freeze-dried bone allograft.

BACKGROUND: To compare the histologic difference in healing between ridge preservation sites treated with a combination allograft of 70% mineralized and 30% demineralized freeze-dried bone allograft (FDBA) evaluated at 8 to 10 weeks versus 18 to 20 weeks post-extraction. Changes in morphological ridge dimensions were also evaluated. METHODS: Forty-four patients with a single-rooted tooth to be extracted and replaced by a dental implant were recruited for this study. At time of extraction, measurements were taken with a custom acrylic stent, and the extraction socket was grafted with the combi

The Alveolar Ridge Splitting Technique on Maxillae: A Biomechanical Human Cadaveric Investigation.

The alveolar ridge splitting technique (ARST) offers an alternative to classic ridge augmentation techniques for successful insertion of dental implants. However, the buccal lamella is at risk of fracturing during ARST distraction. To better understand the fracture mechanisms and displacement limits of the split lamella, this study conducted biomechanical tests on human cadaveric maxilla specimens having extremely atrophied alveolar ridges treated with ARST. A total of 12 standardized alveolar splits were prepared on the maxillae of 3 elderly female donors using an oscillating piezoelectric sa

[Implant-prosthetic rehabilitation of patients with absence of maxillary lateral incisors].

INTRODUCTION: The absence of maxillary lateral incisors caused by traumatic or genetic reasons or as a result of endodontic failure is a difficult problem for an implantologist. The difficulties are the result of bone deficiency in all three dimensions, irregularity of interdental papillae locations and often convergence of roots of the neighbouring teeth. The aim of this study was to present the complexity of orthodontic and implant-prosthetic treatment of the absence of maxillary lateral incisors. MATERIAL AND METHODS: Between 2003 and 2007, 11 patients with absence of maxillary lateral inc

Alveolar bone grafts: the surgical/orthodontic management of the cleft maxilla.

The purpose of this paper is to discuss the management of the cleft maxilla with emphasis on the controversies concerning the decisions which a cleft palate team makes with limited evidence but strong beliefs in the anticipated outcomes. The orthodontist and the surgeon need to collaborate in determining the timing and sequencing of alveolar bone grafting. Currently the emphasis on secondary bone grafting has superseded primary bone grafting in its effectiveness and efficiency. Contemporary management of infants born with clefts of the lip and palate is to delay bone grafting until the early m
