Electrical stimulation shifts healing/scarring towards regeneration in a rat limb amputation model.

Different species respond differently to severe injury, such as limb loss. In species that regenerate, limb loss is met with complete restoration of the limbs' form and function, whereas in mammals the amputated limb's stump heals and scars. In in vitro studies, electrical stimulation (EStim) has been shown to promote cell migration, and osteo- and chondrogenesis. In in vivo studies, after limb amputation, EStim causes significant new bone, cartilage and vessel growth. Here, in a rat model, the stumps of amputated rat limbs were exposed to EStim, and we measured extracellular matrix (ECM) depo

Dorsal Root Ganglion Stimulation for Complex Regional Pain Syndrome (CRPS) Recurrence after Amputation for CRPS, and Failure of Conventional Spinal Cord Stimulation.

BACKGROUND: Limb amputation is sometimes being performed in long-standing complex regional pain syndrome (CRPS), although little evidence is available guiding management decisions, including how CRPS recurrence should be managed. METHODS: This report details the management of a young soldier with CRPS recurrence 2 years after midtibial amputation for CRPS. RESULTS: Conventional spinal cord stimulation did not achieve paraesthetic coverage, or pain relief in the stump, whereas L4 dorsal root ganglion stimulation achieved both coverage and initially modest pain relief, and over time, substanti

A Moving Residual Limb: Botulinum Toxin to the Rescue.

Movement disorders post-amputation are a rare complication and can manifest as the jumping stump phenomenon, a form of peripheral myoclonus. The pathophysiology remains unknown and there is currently no standardized treatment. We describe the case of a 57-year-old male with unremitting stump myoclonus, starting one month after transtibial amputation, in his residual limb without associated phantom or neurological pain. The consequence of the myoclonus was a reduction in prosthetic wearing time. Failure to respond to oral medication led us to attempt the use of botulinum neurotoxin Type A injec

Latissimus dorsi musculocutaneous free flap transplantation to salvage below-elbow amputation in an emergency operation: a case report.

We used a free latissimus dorsi musculocutaneous flap (LD m-c flap) to cover a large skin defect at the stump of a forearm in an emergency operation. The patient we discuss is a 52-year-old man. Amputation at the distal one third of the left forearm occurred after catching his hand and wrist in a machine. The amputated left hand was severely damaged and there were wide skin defects. The function of the elbow joint was well preserved. Both the radius and ulna were cut 7 cm distal from the elbow joint. A 20 x 8 cm square of LD m-c flap was transplanted, to the stump of the forearm. The flap surv

The emerging role of microdialysis in diabetic patients undergoing amputation for limb ischemia.

Lower limb ischemia in diabetic patients is a result of macro- and microcirculation dysfunction. Diabetic patients undergoing limb amputation carry high mortality and morbidity rates, and decision making concerning the level of amputation is critical. Aim of this study is to evaluate a novel microdialysis technique to monitor tissue microcirculation preoperatively and predict the success of limb amputation in such patients. Overall, 165 patients with type 2 diabetes mellitus undergoing lower limb amputation were enrolled. A microdialysis catheter was placed preoperatively at the level of the i

Need of Revision of Lower Limb Amputations in a North Indian Tertiary Care Centre.

INRTODUCTION: Amputation of the extremity is a big challenge to mankind. Revision rate of primary amputations stands high despite of maximum care at tertiary care centres. The purpose of this study was to establish cause for the revision, identify preventable cause and to assess outcome of revision amputation surgery. MATERIALS AND METHODS: We performed a retrospective study on lower limb revision amputations in Department of Physical Medicine and Rehabilitation and Department of Orthopaedic Surgery, King George Medical University between Jan 2012 to Jan 2015. All patients of any age group an

The Boyd amputation as a treatment for osteomyelitis of the foot.

The Boyd amputation is a surgical technique used to treat osteomyelitis of the foot. This amputation is a technically more difficult procedure to perform than the Syme amputation, but it offers certain advantages. The Boyd amputation provides a more solid stump because it preserves the function of the plantar heel pad. Also, because a portion of the calcaneus is left and fused to the tibia, the weightbearing surface is more solid than in the case of a Syme amputation. The authors recommend a Boyd amputation as an alternative to a Syme or a below-the-knee amputation to treat patients with osteo

Results of prosthetic rehabilitation on managing transtibial vascular amputation with silicone liner after wound closure.

OBJECTIVE: To investigate the effect of a standardized silicone liner programme on the duration of prosthetic rehabilitation in patients who underwent transtibial amputation as a result of peripheral arterial disease. METHODS: This retrospective study enrolled patients who underwent transtibial amputation followed by one of two stump management programmes at the same rehabilitation centre over a period of 14 years. The study compared the duration of rehabilitation following a standardized silicone liner programme compared with that following a conventional soft dressing programme. RESULTS: T

Functional and esthetic reconstruction of a mutilated hand using multiple toe transfers and iliac osteocutaneous flap: a case report.

With reconstructive microsurgical techniques, some previously hopelessly mutilated hands have become reconstructable. The functional and esthetic results of such hands can be improved to an acceptable degree with through prereconstruction evaluation and planning. This case report demonstrates the approach to reconstruction of a mutilated hand using the following: a pedicle groin flap for soft tissue reconstruction of the first web space and the amputation stump of the thumb; a combined second and third toe transfer for reconstruction of opposable fingers at the amputated index and middle finge

[Two stage Syme amputation].

In 1843 James Syme described the amputation which bears his name. It is done in two stages and can be accomplished in the presence of wounds and infections or gangrene of the feet. The operation is of great value especially in the elderly. Patients with a Syme stump can be fitted with a strong, light, plastic prosthesis, they develop an excellent gait and the entire body weight can be borne without pain. The advantages of the Syme amputation are that the knee joint is retained and the long lever allows an almost normal gait with an energy cost only 10% above normal. The stump has a large inter

Application of Ilizarov device for transfer of deformed digits, digital stumps, and metacarpal stumps.

PURPOSE: Digital transfer on a pedicle comprising only the neurovascular bundle may lead to irreversible ischemia in cases of severe hand tissue scarring. Previously described methods of such a transfer are conditional on the donor stump's being longer than both the recipient stump and the reconstructed digit. The aim was to develop a method of transferring deformed digits and their stumps after prior distraction of both neurovascular bundles and cutaneosubcutaneous pedicles with the help of the Ilizarov device. METHODS: We performed 2-staged transfer of mutilated digits, digital stumps, or m

Reconstruction of Soft Tissues of the Postamputation Lower Leg Stump with a Free Anterolateral Thigh Flap for Optimal Prosthesis.

Patients without proper covering of the bone stump with soft tissues after below-knee amputation have limited opportunities for prosthesis. The resulting high degree of disability severely restricts their proper functioning in social and professional life. The commonly used significant reduction of the bone length for local coverage limits rehabilitative options to the less comprehensive prosthesis. We aimed to describe a delayed reconstruction with soft tissues of the lower leg stump using free anterolateral thigh flap as an alternative surgical method allowing for optimal prosthesis. A 20-ye

Genetics of acheiropodia ("the handless and footless families of Brazil"): XI. Pathologic aspects.

We report another case of acheiropodia, a rare autosomal recessive trait of distal limb deficiency, to date seen only in Brazil in persons of Portuguese ancestry. The patient's parent were first cousins; she had two younger affected sisters. She presented with signs of vascular insufficiency of the lower stumps, which after amputation were found to lack large vessels below the knee.

Dermatosis contributing factors in bilateral lower limb war-amputees.

This study conducted to assess the factors related to skin lesions in bilateral lower limb war-amputees. This is a clinical based, cross-sectional study of 335 bilateral amputees who injured during Iran-Iraq War. Their demographics and injuries detail, amputation, prosthesis and gait-aid characteristics, stump hygiene and skin problems, have been investigated. Through logistic regression analysis, the three factors of the shape of the stumps, the length of the stumps and excessive perspiration were recognized to have an independent association in skin lesions development, among the population

Congenital amputation involving the hands and feet: a case report.

BACKGROUND: Congenital amputation of the limbs is not uncommon. However, it is very rare when this involves both the upper and lower limbs. … eft little finger. There were forefoot amputations on both lower limbs. Scars were noticed over the amputation stumps with no associated congenital anomaly. CONCLUSION: Congenital amputation involving all limbs as an isolated entity is a rare condition; the cause of which is probably as a result of congenital amniotic bands.

Fourteen-year outcome of unilateral leg replantation after bilateral lower leg and unilateral upper extremity amputation following traumatic injury.

In cases of traumatic major extremity amputations, particularly in the lower extremity, every stage-ranging from decision-making to implementation and outcomes-remains a topic of debate. Managing major amputation cases with high mortality rates is extremely challenging both during the injury and treatment processes. We present a rare and severe case of a 30-year-old patient who sustained injuries from a concrete machine, resulting in bilateral lower extremity amputations and a unilateral proximal arm amputation. We successfully performed a replantation at the proximal ankle level on one side.

Free fillet flap of the hand for elbow preservation in nonreplantable forearm amputation.

The use of non-replantable amputated parts for reconstruction of the stump is a well-established technique. The use of a free fillet flap of the hand and forearm for elbow preservation in massive trauma of the upper extremity is reported in two cases. These free flaps allowed for covering and preserving a functional elbow and a more useful stump.

Quantitative analysis of interface pressures in transfemoral prosthetic sockets.

BACKGROUND: Among the different factors affecting socket comfort, the pressure applied on residual limb tissues is a crucial parameter for the success or failure of any prosthetic device. However, only a few incomplete data are available on people with transfemoral amputation, in this regard. This work aims at filling this gap in the literature. METHODS: Ten people with transfemoral amputation wearing 3 different socket designs were recruited in this study: 2 ischial containment sockets featured by proximal trim lines that contain the ischial tuberosity and ramus and greater trochanter, 2 sub
