On the functional anatomy of the urge-for-action.

Several common neuropsychiatric disorders (e.g., obsessive-compulsive disorder, Tourette syndrome (TS), autistic spectrum disorder) are associated with unpleasant bodily sensations that are perceived as an urge for action. Similarly, many of our everyday behaviors are also characterized by bodily sensations that we experience as urges for action. Where do these urges originate? In this paper, we consider the nature and the functional anatomy of "urges-for-action," both in the context of everyday behaviors such as yawning, swallowing, and micturition, and in relation to clinical disorders in wh…

Anatomy and physiology of the velopharyngeal mechanism.

Understanding the normal anatomy and physiology of the velopharyngeal mechanism is the first step in providing appropriate diagnosis and treatment for children born with cleft lip and palate. The velopharyngeal mechanism consists of a muscular valve that extends from the posterior surface of the hard palate (roof of mouth) to the posterior pharyngeal wall and includes the velum (soft palate), lateral pharyngeal walls (sides of the throat), and the posterior pharyngeal wall (back wall of the throat). The function of the velopharyngeal mechanism is to create a tight seal between the velum and ph…

Anatomy and physiology of the upper airway.

The nose is the major portal of air exchange between the internal and external environment. The nose participates in the vital functions of conditioning inspired air toward a temperature of 37°C and 100% relative humidity, providing local defense and filtering inhaled particulate matter and gases. It also functions in olfaction, which provides both a defense and pleasure for the individual. Understanding normal physiology provides the basis for recognizing abnormalities.

The anatomy and pathophysiology of neck pain.

Neck pain should not, and must not, be confused with cervical radicular pain. Equating the two conditions, or confusing them, results in misdiagnosis, inappropriate investigations, and inappropriate treatment that is destined to fail. So critical is the difference that pedagogically it is unwise to include the two topics in the same book, let alone the same article. However, traditions and expectations are hard to break. In deference to habit, this article addresses both entities, but does so by underplaying cervical radicular pain so as to retain the emphasis on neck pain.

Anatomy of the varus foot and ankle.

In summary, varus deformity of the foot and ankle encompasses a spectrum of deformities from mild to severe. The cause of this deformity may be bone, muscle imbalance, or a combination of both. Surgical intervention should be planned only after the patient's anatomy is understood. Uncorrected symptomatic varus deformities may have significant consequences on gait kinematics and foot biomechanics.

The clinical anatomy of the antecubital fossa.

To the anatomist, the antecubital fossa is the space through which the principal vascular and nervous trunks pass into the forearm. To the surgeon it is also the site of possible injury to the brachial artery in fractures around the elbow, with the risk of Volkmann's ischaemic contracture constantly in mind. It is here where the surgeon is tempted to use the large superficial veins for venous access, but knows only too well the dangers of inadvertent intra-arterial or intraneural injection - something that may well be encountered in a 'main-line' drug addict. It is also a convenient site for a

Structure and anatomy of the aortic root.

AIMS: Understanding the anatomy of the aortic root is particularly relevant in the current era of evolving management strategies including percutaneous and transcatheter therapeutic techniques for valve or device implantations. METHODS AND RESULTS: This review describes the aortic root as a composite structure of several elements, not only the valvar leaflets. The valvar leaflets have a unique shape with deep closure lines buttressed by the nodule of Arantius. The scalloped configuration of the hingelines of the leaflets crosses the ventriculo-arterial junction, leaving interleaflet fibrous t…

Applied anatomy of cricothyrotomy and tracheostomy.

Doctors in training are called upon to perform many practical procedures - often under emergency conditions. All of these depend on a knowledge of their anatomical basis. All too often, the complications which occur, some serious and even life-threatening, are the result of failure to appreciate these anatomical facts. Arterial injury following 'venous' cannulation, damage to the liver or stomach after insertion of a chest drain, pneumothorax consequent on the insertion of a subclavian line - the list can go on and on. In this series of articles, the anatomical basis of a number of everyday pr

[Functional anatomy of the vestibular nerve].

The vestibular system detects head movements such as angular rotation, translation, and head position relative to gravity. It acts to stabilize the eyes and posture through subcortical reflexes. Its signals are also integrated at the cortical level to participate in the elaboration of a body scheme, used for different functions such as spatial orientation and motor control. The vestibular nerve shows a resting discharge rate that is modulated up or down according to head motion or position. Central functioning depends on the detection of an asymmetry between signals coming from a pair of perip

The anatomy of cavus foot deformity.

The term "cavus foot" is used to describe a spectrum of foot shapes that have in common a high arch. The components of cavus are increased pitch and varus of the hindfoot, plantar flexion of the midfoot, and varus and adduction of the forefoot. The cavus shape is associated with changes in the mechanics of the foot. It is less flexible and balanced than a neutral foot. Although one fifth to one quarter of the population has a cavus foot, there is little anatomic description in the literature. This manuscript describes the anatomy and function of cavus foot.

Clinical anatomy of the retrocalcaneal bursa.

The goal of the study was to perform a detailed anatomical description of the retrocalcaneal bursa (RB). Its morphological arrangement was studied on 10 fresh and 30 embalmed lower extremities by microdissection and light microscopy. The RB was present constantly and in all the cases contained 1-2 cm long synovial fold, beginning on the upper wall of RB and distally interposed between the anterior surface of the Achilles tendon and the posterior surface of the calcaneal tuberosity. The volume of RB was 1-1.5 ml. The histological analysis confirmed that the inner surface of the superior and pos

Anatomy of the middle-turn cochleostomy.

OBJECTIVE: Middle-turn cochleostomies are occasionally used for cochlear implant electrode placement in patients with labyrinthitis ossificans. This study evaluates the anatomic characteristics of the middle-turn cochleostomy and its suitability for placement of implant electrodes. METHODS: Ten cadaveric human temporal bones were dissected using a facial recess approach. A middle-turn cochleostomy was drilled 2 mm anterior to the oval window and just inferior to the cochleariform process. The preparations were then stained with osmium tetroxide and microdissections were performed. The locatio

Endoscopic anatomy of the posterior tympanum.

OBJECTIVES: This study aims to describe anatomical variations in the posterior tympanum. Different configurations which influence surgical decisions in middle ear surgery are described. Surgical access to the sinus tympani remains a challenge for otologic surgeons. Usually, the retrotympanum is approached through the middle ear in the anterior-posterior direction during chronic ear surgery. An endoscopic approach is necessary to detail all reliefs of the middle ear and control the total removal in a cholesteatoma surgery. MATERIALS AND METHODS: Our study included 120 ears with otosclerosis th

Endoscopic bronchial anatomy in the cat.

The aim of this paper is to define the endoscopic bronchial anatomy of the cat and to adapt these findings to the bronchoscopic nomenclature previously described for the dog by Amis and McKiernan (Amis TC, McKiernan BC (1986) Systematic identification of endobronchial anatomy during bronchoscopy in the dog. American Journal of Veterinary Research 47(12), 2649-2657). In this study, the normal endoscopic bronchial anatomy was identified based on analysis of 10 air-dried feline lungs, two euthanased and eight healthy anesthetized cats using either a rigid or a flexible endoscope. Endoscopic photo…

Microsurgical anatomy of the mesencephalic veins.

OBJECT: The mesencephalic veins drain crucial brainstem areas. Due to the narrowness of the tentorial notch, these veins can become obstructed as a result of herniation or surgery, leading to hemorrhage and severe consequences for the patient. There is little in the literature about the mesencephalic veins. The aim of this study was to perform an exact analysis of their microanatomy. METHODS: Fifty-two cadaveric hemispheres were examined under an operating microscope, and measurements were made with a digital caliper. The authors focused on the basal vein (BV), pontomesencephalic vein (PMV), …

Endoscopic anatomy of the pterygopalatine fossa.

BACKGROUND: The pterygopalatine fossa can be involved with a variety of infectious and neoplastic processes. This region can be entered endoscopically, but endoscopic landmarks to localize the neurovascular structures in the pterygopalatine fossa have not yet been reported. OBJECTIVES: The purpose of this study is to describe the location of the neurovascular structures in the pterygopalatine fossa in relation to consistent intranasal landmarks. METHODS: Endoscopic dissections of cadaveric heads were performed. The locations of neurovascular structures in the region were defined. RESULTS: T

Anatomy and quantitation of the subscapular nerves.

Information regarding branches of the brachial plexus can be of utility to the surgeon for neurotization procedures following injury. Sixty-two adult cadaveric upper extremities were dissected and the subscapular nerves identified and measured. The upper subscapular nerve originated from the posterior cord in 97% of the cases and in 3% of the cases directly from the axillary nerve. The upper subscapular nerve originated as a single nerve in 90.3% of the cases, as two independent nerve trunks in 8% of the cases and as three independent nerve trunks in 1.6% of the cases. The thoracodorsal nerve

Basic anatomy and physiology of sleep.

This paper seeks to give a basic look at the structure and physiology of those aspects of the nervous and respiratory systems most involved with the cycles of sleep. The brain is examined from the hindbrain (the medulla and the pons) to the midbrain up through the forebrain (the diencephalon and the telencephalon). Also noted are structures that may have a role in sleep and wake cycles, such as the reticular activating system, the red nucleus, basal ganglia, pineal gland, and the like. The respiratory system's structure and physiology is discussed in broad terms. Some of the other factors invo

The surgical anatomy of the ansa pectoralis.

Detailed anatomical knowledge of the pectoral nerves is of clinical importance in surgeries as diverse as limb neurotization, mastectomy, orthopedic procedures and operations related to trauma. The brachial plexus of 200 cadavers were examined in an attempt to clarify the normal origins, courses and variations of the nerves with special emphasis on the ansa pectoralis (AP). In 75% the MPN arose from the anterior division of the inferior trunk of the brachial plexus and in 25% it arose from the medial cord. In 40% of specimens, the LPN arose from a single contributing nerve (anterior division o
