Low-flow anaesthesia. Does it have potential pharmacoeconomic consequences?

Healthcare reform has placed increasing pressure on anaesthetists to consider the costs of current anaesthesia strategies. Although the cost of anaesthesia constitutes only a small proportion of total healthcare costs, anaesthetic drug expenditures have been a focus of cost-containment efforts. Low-flow anaesthesia is a simple method of reducing the fresh gas flow rate for anaesthetic gases during inhalational anaesthesia. A knowledge of the pharmacokinetic behaviour of inhaled anaesthetics and the use of modern equipment and monitoring technology meet the requirements for safe application of

Sevoflurane sedation: blind alley or cunning plan?

Patient-controlled sedation using inhaled anaesthetics provides several potential advantages. In a study of healthy volunteers, low-dose inhaled sevoflurane impaired volunteers' ability to complete a button-press task without significant cardiorespiratory compromise. Thus patient-controlled sedation using sevoflurane might be feasible; however, regulatory hurdles require proof of safety and efficacy through clinical trials. Although a promising concept, it must be demonstrated that patients cannot induce unconsciousness on their own despite any interactions with opioid analgesics. Significant

The effect of fentanyl on immobility after noxious stimulation in isoflurane-anaesthetized pigs: Exploring the role of the serotonergic system.

OBJECTIVE: To investigate if fentanyl induces immobility through activation of the serotonergic 5HT1A receptor, by using the 5HT1A-antagonist robalzotan. STUDY DESIGN: A prospective, blinded, randomized, two-group study. ANIMALS: A group of 12 mixed-breed pigs aged 71-79 days. METHODS: The motor response to clamping a claw was assessed in isoflurane-anaesthetized pigs at baseline, then fentanyl was infused intravenously (IV) for 40 minutes and clamping was repeated. The infusion started at 20 μg kg-1 hour-1 and was increased by 60% until fentanyl produced immobility, defined as no motor res

Decreased Thalamic Activity Is a Correlate for Disconnectedness during Anesthesia with Propofol, Dexmedetomidine and Sevoflurane But Not S-Ketamine.

Establishing the neural mechanisms responsible for the altered global states of consciousness during anesthesia and dissociating these from other drug-related effects remains a challenge in consciousness research. We investigated differences in brain activity between connectedness and disconnectedness by administering various anesthetics at concentrations designed to render 50% of the subjects unresponsive. One hundred and sixty healthy male subjects were randomized to receive either propofol (1.7 μg/ml; n = 40), dexmedetomidine (1.5 ng/ml; n = 40), sevoflurane (0.9% end-tidal; n = 40), S-keta

Special article: general anesthetic gases and the global environment.

General anesthetics are administered to approximately 50 million patients each year in the United States. Anesthetic vapors and gases are also widely used in dentists' offices, veterinary clinics, and laboratories for animal research. All the volatile anesthetics that are currently used are halogenated compounds destructive to the ozone layer. These halogenated anesthetics could have potential significant impact on global warming. The widely used anesthetic gas nitrous oxide is a known greenhouse gas as well as an important ozone-depleting gas. These anesthetic gases and vapors are primarily e

Molecular modelling of specific and non-specific anaesthetic interactions.

There has been rapid progress in molecular modelling in recent years. The convergence of improved software for molecular mechanics and dynamics, techniques for chimeric substitution and site-directed mutations, and the first x-ray structures of transmembrane ion channels have made it possible to build and test models of anaesthetic binding sites. These models have served as guides for site-directed mutagenesis and as starting points for understanding the molecular dynamics of anaesthetic-site interactions. Ligand-gated ion channels are targets for inhaled anaesthetics and alcohols in the centr

Postoperative outcomes after total sevoflurane inhalation sedation using a disposable delivery system (Sedaconda-ACD) in cardiac surgery.

INTRODUCTION: The COVID-19 pandemic prompted our team to develop new solutions for performing cardiac surgery without intravenous anesthetics due to a shortage of these drugs. We utilized an anesthetic conserving device (Sedaconda-ACD) to administer total inhaled anesthesia because specific vaporizers were unavailable for administering inhaled agents during cardiopulmonary bypass (CPB) in our center. We documented our experience and postoperative cardiovascular outcomes. The primary outcome was the peak level of troponin, with secondary outcomes encompassing other cardiovascular complications.

Successful management of hypothermic cardiopulmonary bypass in a malignant hyperthermia susceptible patient.

Malignant hyperthermia (MH) is a potentially lethal reaction in those that are genetically predisposed, frequently triggered by inhaled anesthetics. MH is often difficult to diagnose because it is accompanied by signs and symptoms that are shared with other disorders. The diagnosis is further obscured in cardiac surgical patients, as the signs of MH can be masked by the cardiopulmonary bypass circuit (CPB) and the use of induced hypothermia. In this case-report, we describe the successful anesthetic management of a 65-year-old MH-susceptible female, confirmed via caffeine halothane contracture

Drug-induced perioperative malignant hyperthermia: a real-world study based on the Food and Drug Administration Adverse Event Reporting System (FAERS) database.

BACKGROUND: Perioperative malignant hyperthermia (MH) remains an established adverse drug reaction (ADR) for certain drug classes, but the comprehensive profile of current potential MH-triggering drugs is still lacking. METHODS: The adverse event (AE) reports related to perioperative MH in the Food and Drug Administration Adverse Event Reporting System (FAERS) from the first quarter of 2005 to the fourth quarter of 2024 were collected to count and trend the drugs associated with perioperative MH or perioperative MH with fatal outcomes. The disproportionality analysis was applied to identify A

A Comparison of Three Anesthetic Drug Combinations for Use in Inducing Surgical Anesthesia in Female Guinea Pigs (Cavia porcellus).

Guinea pigs are often used in translational research, but providing them with safe and effective anesthesia is a challenge. Common methods like inhalant anesthesia and injectable ketamine/xylazine induce surgical anesthesia but can negatively affect cardiovascular, respiratory, and thermoregulatory systems and complicate the interpretation of research outcomes. Several alternative anesthetic regimens have been investigated, but none have consistently achieved a surgical plane of anesthesia. Therefore, identifying an anesthetic regimen that achieves a stable state of the surgical plane of anest

Laparoscopic gynecological surgery in an adult woman with Becker muscular dystrophy performed with sevoflurane with cisatracurium anesthesia: A case report.

RATIONALE: Becker muscular dystrophy (BMD) and Duchenne muscular dystrophy (DMD) are progressive neuromuscular disorders caused by mutations in the dystrophin gene. The management of anesthesia in patients with BMD is complicated because they are highly sensitive to the conventional anesthetics such as volatile anesthetics and muscle relaxants. It is reported that anesthesia in patients with DMD is associated with several complications. However, a few case reports have been published on adult patients with BMD undergoing surgery with general anesthesia. Reports indicate that children with BMD

Observations on the effects of inhaled isoflurane in long-term sedation of critically Ill children using a modified AnaConDa©-system.

Long-term intravenous sedation may present problems due to dependence and side effects. Medical records of children who were administered isoflurane were reviewed. 15 patients (9 boys, 6 girls) with a mean age of 11.8 month (+2.4) were analysed.Analgesia and sedation was given in mean 9.7+1.1 days before commencing inhalation using a modified application device (AnaConDa©). Administration was given over a period of 7.2+1.4 days. Depth of sedation was monitored by using Comfort- and Hartwig-scores. Observations included continuous monitoring of heart-rate, pulse oxymetry, blood pressure and cer

Does the Diagnosis of Intraoperative Malignant Hyperthermia Require Case Termination? A Case Report.

Malignant hyperthermia (MH) is a rare genetic disorder triggered by inhalational anesthetics or depolarizing neuromuscular blocking agents that carries significant mortality if not promptly treated. The following case presents a healthy 39-year-old man who developed MH several hours into an anesthetic exposure. Rapid intraoperative stabilization tactics that paralleled intensive care unit (ICU) level care allowed for continuation of operative management as opposed to case termination given the patient was at high risk for permanent nerve palsy if the case were to be aborted during dissection.

Error Traps in Infant Spinal Anesthesia.

Infant spinal anesthesia is a viable alternative to general anesthesia for short procedures below the upper abdomen. It provides a hemodynamically stable anesthetic technique that avoids airway manipulation and associated respiratory complications. Spinal anesthesia allows surgery to be performed without inhaled anesthetic agents or, in certain cases, opioids. This manuscript highlights five preventable perioperative error traps that may occur while performing infant spinal anesthesia with the goal of providing expert clinical guidance for the provision of safe, effective, and efficient spinal
