Cold-induced urticaria and angioedema: diagnosis and management.

The physical urticarias compromise a heterogeneous group of disorders characterized by their propensity to form urticarial lesions after exposure to certain physical stimuli, such as heat, vibration, or cold. Although generally benign, systemic hypotension and angioedematous airway compromise are known life-threatening complications. We present the case of a man who experienced benign angioedematous changes in response to cold exposure, and discuss the clinical course, diagnosis, and management of cold urticaria.

Angioedema as a complication in periodontal surgery: report of a case.

Angioedema is a diffuse swelling of the subcutaneous or submucosal tissues that occurs in both hereditary and non-hereditary forms. It can be a temporarily disfiguring condition, but not usually a serious one unless the airway is compromised. In the majority of cases, no underlying cause can be identified. In this report, a case of "idiopathic" angioedema that occurred while performing a periodontal surgical procedure is presented. This case is interesting because the patient was on long-term use of an angiotensin-converting enzyme [ACE] inhibitor for hypertension, and recent evidence has show…

Glomerulonephritis and hereditary angioedema: report of 2 cases.

Hereditary deficiency of C1 esterase inhibitor (C1 INH) responsible for hereditary angioedema (HAE) is the most common hereditary complement deficiency. HAE has occasionally been reported in association with lupus erythematosus and with glomerulonephritis (GN). We report 2 cases of GN-associated C1 INH deficiency. Renal manifestations have been discovered respectively 6 to 17 years before onset of attacks. Kidney biopsy of the 1st patient showed diffuse proliferative GN with a rare and scattered wire loop pattern whereas the 2nd patient displayed a type I membranoproliferative GN. Chronic rena…

Association of chronic urticaria and angioedema with thyroid autoimmunity.

Seventeen patients, constituting 12.1% of 140 consecutively seen cases of chronic urticaria, demonstrated thyroid autoimmunity with thyroid microsomal antibodies (TMAs) in serum titers greater than or equal to 1:1,600. Eight of these 17 patients had goiter or thyroid dysfunction. In a control group of 477 consecutively seen patients, only 27 (5.6%) had similar TMA titers. Routine and special immunologic test results in this group of 17 patients did not differ from those found in other patients with chronic urticaria and angioedema (CUA), and the only notable clinical feature was that all 17 ha…

Angioedema due to diphetarsone and a review of its toxicity.

Diphetarsone is an arsenical used in the treatment of intestinal amoebiasis and trichuriasis. It is an efficacious drug with minimal toxicity. However, in this report, a heretofore undescribed reaction, that of generalized angioedema, is presented. A review of diphetarsone toxicity is also presented.

Hereditary angioedema: the clinical syndrome and its management.

Hereditary angioedema is manifested by attacks of swelling of the extremities, face, trunk, airway, or abdominal viscera, occurring spontaneously or secondary to trauma. It is inherited as an autosomal dominant trait and is due to deficient activity of the inhibitor of the activated first component of complement. The clinical diagnosis can be confirmed by the findings of low levels of C4 or C1 esterase inhibitor activity, or both. Therapy may be divided into three phases: long-term prophylaxis of attacks, short-term prophylaxis of attacks, and treatment of acute attacks. Long-term prophylaxis …

Drug-Induced Angioedema Without Urticaria: A Case Report.

Non-steroidal anti-inflammatory drugs (NSAIDs) are widely prescribed for various conditions but are associated with numerous adverse drug reactions (ADRs). Understanding these ADRs is necessary to reduce morbidity and mortality. NSAID-induced angioedema, although rare, can be life-threatening and is often due to increased leukotriene production from COX pathway inhibition. Mast cells and basophil degranulation play vital roles in its pathogenesis. Prompt recognition and immediate cessation of the culprit drug, along with the administration of corticosteroids and antihistamines, are essential. …

Prolonged angioedema after Pfizer-BioNTech COVID-19 vaccine.

This case reports a woman in her 40s with a history of allergic reaction to shellfish and iodine who presented with tongue angioedema, difficulty breathing and chest tightness after receiving the first dose of the Pfizer-BioNTech (BNT162b2) COVID-19 vaccine. Her angioedema remained for 10 days post-exposure to the vaccine, requiring 3 days of epinephrine infusion. She was discharged with advice to avoid further mRNA vaccines. This case highlights the increasing awareness needed of polyethylene glycol (PEG) allergy and the protracted nature of her reaction. A firm conclusion cannot be reached b…

Driving towards Precision Medicine for angioedema without wheals.

Evidence accumulated over the last two decades indicates that recurrent angioedema without wheals constitutes a diverse family of disorders with a much higher complexity than was previously regarded. Indicatively, during the last two years, novel variants of three genes other than SERPING1 and F12 have been identified in association with hereditary angioedema. Most interestingly, functional studies of at least one of these variants (the variant c.807G > T of ANGPT1 gene) imply the existence of a new disease endotype in which the altered bradykinin metabolism and function does not play a centra…

Pharmacogenetics of angiotensin-converting enzyme inhibitor-induced angioedema.

Angioedema is a rare adverse effect of the commonly used angiotensin-converting enzyme inhibitors (ACEi) and is reported to occur with a prevalence of 0.1%-0.7%. Although most ACEi-induced angioedema (ACEi-A) cases are mild, severe cases requiring intensive care and even resulting in death have been reported in the literature. The mechanisms underlying ACEi-A are not yet fully understood, but bradykinin and/or substance P accumulation resulting from inhibition of ACE is believed to play a crucial role. ACEi-A occurs at variable frequencies across different racial groups, suggesting a genetic a…

Angiotensin-converting enzyme inhibitor-induced unilateral tongue angioedema.

Angioedema is a frequently reported side effect of angiotensin-converting enzyme inhibitors. The literature suggests that immunosuppressed transplant patients are at an increased risk for this adverse condition. A 62-year-old African American man presented with acute unilateral angioedema attributed to angiotensin-converting enzyme inhibitor initiation. A PubMed literature search regarding unilateral angioedema produced only 3 reported cases confirming an uncommon presentation of an otherwise common adverse drug event. The cases raised questions regarding evidence-based management of drug-indu…

[Increasing incidence of angioedema without urticaria--clinical features].

The causes of angioedema (AE), a self-limited, localized swelling of subcutaneous tissue or mucosa unaccompanied by urticaria, are diverse. The commonly applied label of "allergic" is frequently wrong and standard anti-allergic therapy can be ineffective. Types of AE could be categorized according to mediators which mediate vascular leakage: bradykinin AE (hereditary, acquired, angiotensin-converting enzyme inhibitor (ACEi)-related), histamine AE (allergic etiology), and various mediators mediated AE (pseudoallergic reaction to non-steroidal anti-inflammatory drugs). Idiopathic AE is a poorly …

Perioral angioedema associated with angiotensin-converting enzyme inhibitor.

BACKGROUND: Angioedema is a non-pruritic swelling usually limited to the skin and mucous membranes of the face and perioral soft tissues. It can be life threatening but usually is not, and can be managed with conservative medical treatment unless the airway is endangered. Recent reports suggest that angiotensin-converting enzyme (ACE) inhibitors can predispose and/or precipitate angioedema, with a predilection toward patients of African American ancestry. METHODS: This case report involved a 65-year-old African American female who was being treated surgically for localized chronic periodontit…

[Life-threatening angioedema associated with ACE inhibitor treatment].

A case of severe late-onset angioedema with life-threatening upper airway obstruction, related to ACE inhibitor treatment is presented. The patient was a middle-aged man treated for hypertension, presenting with stridor requiring intubation, which was difficult. The patient developed cardiac arrest but was successfully resuscitated, after intubation. Etiology, risk factors, and treatment, with special attention to air way management, are discussed.
