Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association2005eng
A 21-year-old West Point cadet presented to our institution with a history of chronic left lateral ankle instability. The initial injury occurred 5 years earlier. Physical examination results and stress radiographs were consistent with lateral instability. The patient underwent an ankle arthroscopy and lateral ankle ligament reconstruction. Arthroscopic findings included moderate synovitis, grade II anterolateral chondrosis, and an anterior talar osteophyte. The patient had an uneventful postoperative course and returned to activity. Eleven months after surgery he presented with increased left…

MR imaging can be extremely helpful in evaluating congenital and acquired deformities of the foot and ankle. Knowledge of the normal developmental anatomy of the foot and ankle is important to accurately identify abnormalities versus normal variants. The foot and ankle may be abnormal because of trauma, infection, tumor, or systemic disease. MR imaging can help in the differential diagnosis of abnormalities as well as in treatment planning and follow-up.

We undertook a prospective study to determine the type and distribution of foot and ankle snowboarding injuries. Reports of 3213 snowboarding injuries were collected from 12 Colorado ski resorts between 1988 and 1995. Of these, 491 (15.3%) were ankle injuries and 58 (1.8%) were foot injuries. Ankle injuries included 216 (44%) fractures and 255 (52%) sprains. Thirty-three (57%) of the foot injuries were fractures and 16 (28%) were sprains. The remaining injuries were soft tissue injuries, contusions, or abrasions. There was no significant correlation between boot type (soft, hybrid, or hard) an…

Injuries to the lateral ankle ligaments are extremely common. Many patients with acute injuries to the lateral ligament complex develop chronic ankle instability that requires surgical intervention. MR arthrography of the ankle plays a valuable role in identifying those patients who are most at risk for the development of chronic instability. MR arthrography of the ankle also aids in preoperative planning and in differentiating among other entities that can present as ankle instability.

For specific indications, ultrasound is an inexpensive and efficient alternative to magnetic resonance (MR) imaging of the ankle and foot. In addition to the tendons, the anterior joint, retrocalcaneal bursa, ligaments, plantar fascia, and soft tissues can be imaged with ultrasound. Tenosynovitis, tendon tears and tendinosis, joint effusions, intra-articular loose bodies, ganglion cysts, plantar fasciitis, and Morton neuromas can be demonstrated with ultrasound. In most cases, a focused ankle or foot ultrasound can be performed more rapidly and efficiently than magnetic resonance imaging. Dire…

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1997eng
Ankle arthroscopy has become a standard procedure for a variety of indications. Joint distraction is applied by many authors. A recent retrospective multicentre study provoked the following questions. Is there an indication for diagnostic arthroscopy? Can arthroscopic surgery of the ankle joint be routinely performed without joint distraction? Does the range of motion increase after arthroscopic treatment of an anterior impingement syndrome? A series of 122 consecutive patients were prospectively studied in a protocol that included preoperative history taking, physical examination, and standar…

Arthrodesis of the ankle joint is still an important operation in the treatment of painful arthrosis, chronic infection, and malalignment instability when these cases have been treated unsuccessfully with other therapeutic procedures. To date, prosthetic replacement of the ankle joint has been an alternative only in rheumatoid patients. Concerning the long-term results, ankle arthrodesis is much better than alloarthroplasty. Because of the good functional results achievable by arthrodesis when done in the right position, there is less often a need for prosthetic replacement than in the hip or …

The foot and ankle are frequently injured in athletic activities. In addition to acute injuries such as ankle sprains and fractures, a variety of overuse injuries are seen. A thorough knowledge of the mechanism of injury and the course of the healing process is essential for the comprehensive rehabilitation of such injuries. Recovery progresses through several phases. If selected appropriately, modalities and therapeutic exercise can work together to facilitate the healing process at each phase. Many injuries could be averted by preventive exercise, judicious use of taping and bracing, and rec…

Arthroscopy : the journal of arthroscopic & related surgery : official publication of the Arthroscopy Association of North America and the International Arthroscopy Association1992eng
The long-term results of 30 operative arthroscopies of the ankle performed from 1983 to 1989 are the basis of this study. The most frequent lesions are synovitis and osteochondral defects of the talus. The treatment consists of lavage, synovial debridement, osteochondral debridement, and the removal of loose bodies. Of the 30 cases, 86.7% obtained excellent or good results and 88% of the athletes returned to their sport.

Synovial chondromatosis is a rare disorder presenting a typical radiological appearance. We reviewed three patients with this disease in the ankle. Two of these patients were reexamined after 7 and 27 years, respectively. Although both had several calcified juxta-articular bodies in the ankle none of them had any clinical symptoms. This indicates that surgical intervention should be performed only if mechanical signs and symptoms are present.

The technique and results of 11 arthroscopies of the ankle joint are presented. The main indications were post-traumatic pain and osteochondritis dissecans of the talus. As a result of arthroscopy, a radionegative loose body was found, exploratory arthrotomy was avoided in one case, incisions and operative procedures were changed, and loose bodies were removed by arthroscopic operation in one case.

The ankle is one of the more vulnerable and commonly injured joints in athletic activities, with sprains accounting for between 30 and 50 per cent of all reported injuries among athletic teams. Despite this high volume and opportunity for experience, the ankle remains one of the most poorly understood and inadequately treated sites of injury.

The important points in evaluation of the traumatized foot and ankle are discussed in detail. Also considered are specific injuries, including contusion; injuries of the musculotendinous unit, ligaments, and capsule; stress fractures; and puncture wounds.
