Tongue-tie, breastfeeding difficulties and the role of Frenotomy.

UNLABELLED: A greater emphasis on exclusive breastfeeding in recent years has re-ignited the historical debate over the role of ankyloglossia (tongue-tie) in infants with breastfeeding difficulties. Several prospective cohort studies and randomized control trials have been published in this area in the last 10-15 years. We critically evaluated recent evidence and attempted to answer two important clinical queries in this area, that is, (i) whether ankyloglossia is associated with breastfeeding difficulties and (ii) whether frenotomy helps mother-baby dyad in such setting? CONCLUSION: Neonates…

Posterior tongue tie: that is a thing?

PURPOSE OF REVIEW: The purpose of this review is to examine the current research of the posterior tongue tie and how it relates to breast feeding, solid feeding, and speech. RECENT FINDINGS: Recent findings show that the posterior tongue tie may play a role in effective breast feeding. SUMMARY: Ankyloglossia is the term used for the restriction of the movement of the tongue that impairs certain functions such as breastfeeding or bottle feeding, feeding with solids, and speech. Cadaver studies have shown that there can be a restriction of the tongue and oral tissues in some people relative to…

[Lingual frenectomy with ND:YAG laser. case report].

Ankyloglossia is considered a congenital patology, whose treatment indicated is a frenectomy, this surgical technique consists in remove the tissue that joins the tongue with the floor mouth. This technique allows the patient a considerable improvement to limitations caused by this anomaly such as difficulties in suction, pronunciation, chewing and difficulty in touching the lower lip with the tip of the tongue. Currently the use of laser technology and its benefits in modern dentistry, allows the realization of surgical procedures free of bleeding, with very little pain and inflammation of th…

Delayed Hemorrhage Following Laser Frenotomy Leading to Hypovolemic Shock.

Ankyloglossia is a failure of the tongue to release from the oral floor with reported consequences that include breastfeeding difficulties and speech impediments. Frenotomy is a commonly performed procedure for the treatment of ankyloglossia. Laser (light amplification by stimulated emission of radiation) is one of several mediums used to perform frenotomies. Although most frenotomies are uncomplicated, there remains a small possibility of complication, such as infection, pain, ductal injury, and hemorrhage, even in expert hands. Because frenotomies are most often performed in infants, postope…

A Prospective Randomized Control Trial of Lingual Frenuloplasty with Myofunctional Therapy in Patients with Maxillofacial Deformity in a Polish Cohort.

Introduction: There are few publications concerning ankyloglossia in mixed-aged groups utilizing myofunctional therapy and frenuloplasty in patients undergoing orthodontic treatment and maxillofacial surgery. While it is well known that ankyloglossia is mainly diagnosed in babies, research on functional and structural disorders in different age groups is less common. Thus, there is a high need for specific information about the influence and effectiveness of frenuloplasty with myofunctional therapy on the stomatognathic function and final treatment outcome for a wider variety of patients, espe…

Clinical outcomes after the use of complete autologous oral mucosa equivalents: preliminary cases.

OBJECTIVE: Previously, we reported how to obtain complete autologous oral mucosa equivalents (CAOMEs) composed of an autologous plasma scaffold and fibroblasts together with immature keratinocytes able to build an oral epithelium with a structure similar to that of the oral mucosa. In this study, we present the clinical outcomes after applying our CAOMEs as grafts. STUDY DESIGN: Four patients who needed a CAOME to restore a defect of oral mucosa were selected. Two of the patients suffered from ankyloglossia, and the other 2 required a restoration of the keratinized gum of the alveolar rim. To…

Tongue-tie and breastfeeding.

OBJECTIVE: To evaluate the efficiency of maternal breastfeeding and maternal pain pre- and post-lingual frenulum release procedures in infants with ankyloglossia. METHODS: Infants under 4 months of age with tongue-tie who were actively breastfeeding, and their mothers (mother-infant dyads) were recruited. Infants' ankyloglossia severity was evaluated using the Coryllos® ankyloglossia tongue-tie grading scale. Each mother completed a pre-procedure questionnaire where breastfeeding efficiency was evaluated using the LATCH® criteria. Each mother also reported a numeric score of pain with feeding…

Expression of mouse Tbx22 supports its role in palatogenesis and glossogenesis.

TBX22 belongs to the T-box family of transcription factors and was originally found in an in silico approach designed to identify new genes on the human Xq12-q21 region. Mutations in TBX22 have been reported in families with X-linked cleft palate and ankyloglossia (CPX), but the underlying pathogenetic mechanism remained unknown. We have identified mouse Tbx22 and analyzed its expression during embryogenesis by reverse transcriptase-polymerase chain reaction and in situ hybridization. In mouse embryos, it is expressed in distinct areas of the head, namely the mesenchyme of the inferior nasal s…

Incidence of oral tie diagnosis and treatment in Southwestern Ontario: A survey study.

BACKGROUND: Ankyloglossia describes a short lingual frenulum restricting tongue mobility. There are no universally accepted diagnostic criteria, and the rate of diagnosis and treatment are increasing. Many frenotomies are performed in the outpatient setting, making it difficult to estimate ankyloglossia incidence. This study aims to estimate the rate of diagnosis and common indications cited for treatment of ankyloglossia in a large catchment in Southwestern Ontario, Canada. METHODS: A survey was administered to all patients presenting to a pediatric otolaryngology clinic and five community p…

Management of Tongue-Tie Using Conventional Technique for Speech Clarity and Correcting Malocclusion: A Case Report.

Ankyloglossia, which is a congenital condition also referred to as tongue-tie, is described as a small lingual frenum that restricts tongue movement and its function. The main purpose of the frenum is to restrict the movement of the cheek, lip muscles, and tongue throughout the development of fetuses and maintain equilibrium between the developing bones, lip musculature, and tongue. The constriction of the buccal musculature counteracts the outward pressure that the tongue applies to the teeth. Arch width maintenance requires a state of equilibrium between these two muscle groups. Therefore, a…

Human craniofacial malformations are a class of common congenital anomalies in which the etiology is heterogeneous and often poorly understo … us cleft palate and bifid or absent uvula. Our recent reevaluation of the family has indicated that ankyloglossia (tongue-tie) is also a feature of X-linked cleft palate in some of the affected males and carrier females. Ankyloglossia has previously been associated with X-linked cleft palate in an Icelandic kindred in which a gene responsible for cleft palate ( …

Use of High-Power Diode Laser in Neonatal Lingual Frenulum Release Surgery: Clinical Approach and Assessment of Healing in a Case Series.

Ankyloglossia is a congenital condition characterized by restricted tongue mobility due to an abnormal lingual frenum, which may impair breastfeeding in newborns and infants. Early diagnosis and treatment are essential to prevent functional complications. This case series describes 35 newborns and infants with ankyloglossia who underwent frenectomy using a super-pulsed dual-wavelength diode laser. The procedures were performed using a diode laser operating at wavelengths of 810 nm (1.2 W) and 980 nm (1.4 W), delivering a final output power of 1.1 W, in super-pulsed mode (20 Hz frequency, 0.05 …

Cell-cell interaction determines cell fate of mesoderm-derived cell in tongue development through Hh signaling.

Dysfunction of primary cilia leads to genetic disorder, ciliopathies, which shows various malformations in many vital organs such as brain. Multiple tongue deformities including cleft, hamartoma, and ankyloglossia are also seen in ciliopathies, which yield difficulties in fundamental functions such as mastication and vocalization. Here, we found these tongue anomalies in mice with mutation of ciliary protein. Abnormal cranial neural crest-derived cells (CNCC) failed to evoke Hh signal for differentiation of mesoderm-derived cells into myoblasts, which resulted in abnormal differentiation of me…

Mutation analysis of TBX22 reveals new mutation in Tunisian CPX family.

Cleft palate with ankyloglossia (CPX; OMIM 303400) is inherited as a Mendelian semidominant X-Linked disorder. Linkage studies resulted in mapping CPX to Xq13-q 21-31 region. TBX22 was identified as causing CPX. We report a new mutation in a Tunisian family and the first Arab family with X-Linked cleft palate and ankyloglossia. The family includes 6 affected members, 4 males and 2 females. Linkage study was performed using 9 microsatellite markers surrounding the CPX locus with a maximum lod score 1.81 at theta=0 with several markers. Sequence analysis of TBX22 gene revealed a novel change c.3…

Lower lip cleft, bifid tongue and fibrolipoma: a case report of rare congenital anomaly.

A non-syndromic bifid tongue with fibrolipoma, ankyloglossia, and clefts of the lower lip and alveolus is rare. We describe a case in which a bifid anterior tongue was separated by a fibrolipomatous mass. We excised the mass and repaired the lip and the tongue with satisfactory results.

Management of Postoperative Hematoma Following a Lingual Frenectomy: A Case Report.

Ankyloglossia, commonly known as tongue-tie, is a congenital condition characterized by a short, thick lingual frenum that restricts tongue mobility, leading to functional and developmental issues. Surgical intervention, such as a lingual frenectomy, is commonly used to restore tongue function. However, postoperative complications like hematomas can pose significant risks. This case report discusses the management of a sublingual hematoma in a 15-year-old male following a conventional lingual frenectomy. Postoperatively, the patient developed swelling, erythema, and restricted tongue movement,…
