PURPOSE: To investigate the effect of secondary intraocular lens (IOL) implantation on binocular vision in patients with unilateral aphakia uncorrected consecutively for 3 years or more.
METHODS: This study included nine patients who underwent secondary IOL implantation, but had remained in a unilateral aphakic state, without being corrected, for at least 3 consecutive years at the time of the secondary IOL implant. We compared the efficacy of monofocal and multifocal intraocular lenses (IOLs) as a means of optically rehabilitating eyes with monocular congenital cataracts using a primate model. A congenital cataract was simulated in the right eye of 20 monkeys from birth to 2 weeks of age with an opalescent contact lens. Fifty pediatric aphakic patients, ranging in age from 7 days to 11 years, were fitted with the Silsoft extended wear contact lens. Compared to other extended wear lenses, the Silsoft lens has several advantages. These include easier handling, a less traumatic fitting procedure for the child, the use of fluorescein for evaluation of the lens-cornea relationship and a low loss rate. This lens appears to be safe, with no permanent adverse effects on the cornea.

INTRODUCTION: the ultrasound prenatal diagnosis of aphakia is a difficult diagnosis and often requires a genetic study of the karyotype.
CASE REPORT: we present a rare case of prenatal bilateral aphakia, confirmed after bird. The patient was observed by ultrasound during the 23rd week of pregnancy. Through transabdominal ultrasound the lens could not be visualized bilaterally. The remaining anathomy, explorable by ultrasound, was still regular. When aphakia is suspected, genetic counseling is essential.
