Clinical predictors of obstructive sleep apnea.

OBJECTIVE: To identify physical findings that can be standardized to predict the presence and the severity of obstructive sleep apnea (OSA). STUDY DESIGN: One hundred seventy-two patients who answered questionnaires with responses that suggested they might have OSA were included in this prospective study. METHODS: All patients underwent a physical examination and polysomnography. The physical examination included the measurement of four parameters used by anesthesiologists to identify patients likely to have difficult intubation to determine if these same parameters predict OSA. We recorded …

Snoring, obstructive sleep apnea, and surgery.

Snoring and OSA syndrome are prevalent and important causes of sleep disturbance. Snoring, historically considered to be only a habitual ann … jority of patients. Surgical correction of nasal obstruction is advocated in conjunction with sleep apnea surgery when nasal obstruction exists. In OSA patients with retrolingual airway obstruction, a number of surgical procedures have been performed, with or without UPPP, with some improvement over UPPP alone. MMO has been effective in the treatment of …

Progressive retropalatal narrowing preceding obstructive apnea.

Pharyngeal occlusion during obstructive apnea is thought to be an inspiratory-related event; however, occlusion also occurs in the absence of negative intrathoracic pressure. We hypothesized that inspiratory-related pharyngeal occlusion would be preceded by significant expiratory narrowing. Eight sleeping patients with obstructive apnea were studied. Pharyngeal caliber, airflow, and esophageal pressure (Pes) were simultaneously monitored during three to four consecutive breaths preceding occlusion (between 3 and 22 events were studied per subject). Relative changes in retropalatal airway cross…

[Hypersomnia. Sleep apnea syndrome and somnolence].

A RECENTLY DEFINED SYNDROME: Sleep apnea syndrome is defined as 5 episodes of apnea or 10 episodes of hyopnea per hour of sleep; the cardinal symptom is excessive diurnal somnolence. Two factors could explain sleep apnea syndrome: hypoxemia and fragmented sleep. ROLE OF HYPOXEMIA: Early studies focused on hypoxemia, but few took into account the effect of subjective complaints of somnolence. Further investigations based on objective measurements introduced a certain degree of complexity distinguishing between sleepiness and the capacity for voluntary vigilance. FRAGMENTARY SLEEP: More recent w…

Pediatric adenotonsillectomy for obstructive sleep apnea.

Forty-four children who underwent adenotonsillectomy for obstructive sleep apnea (OSA) were studied. The diagnosis was confirmed polysomnographically. Patients with other medical problems or complications of OSA were excluded. The overall complication rate was 32%. Significant airway complications occurred in 16%. Factors associated with development of statistically significant airway complications were acute airway compromise, age < 3 years, thin body habitus, and both oxygen (O2) desaturation and carbon dioxide (CO2) retention seen polysomnographically. Although many OSA patients can safely …

Cephalometric assessment in obstructive sleep apnea.

It is reported that some specific craniofacial characteristics are associated with obstructive sleep apnea syndrome (OSAS). To test this finding, the present study developed and assessed the feasibility of a craniofacial index score (CIS) in differentiating patients with OSAS from habitual snorers. Anthropometric measurements and lateral head radiographs were obtained on 24 male and 4 female patients with OSAS who had physician-diagnosed OSAS (respiratory disturbance index (RDI) >20), and 25 male and 5 female habitual snorers (RDI <20). Thirteen cephalometric and four anthropometric measure- m…

Obstructive sleep apnea-induced supraventricular tachycardia.

Obstructive sleep apnea has been associated with various cardiac arrhythmias; however, supraventricular tachycardia has not been reported to occur in this disorder. This case report describes a patient who developed episodes of supraventricular tachycardia during periods of apnea and oxygen desaturation. With the initiation of nasal continuous positive airway pressure during sleep, the arrhythmia was abolished. The etiology and possible mechanisms responsible for the supraventricular tachycardia are discussed.

Sleep apnea syndrome and cerebral hemodynamics.

The dynamics of cerebral blood flow velocity (CBFV) during sleep were investigated in the right middle cerebral artery of 10 patients with sleep apnea syndrome (SAS) (mean age, 37 years) and 10 healthy control subjects (mean age, 32 years) throughout the entire sleep period. A computer-assisted pulsed (2 MHz) transcranial Doppler ultrasonography system was modified for continuous long-term and on-line recording of cerebral hemodynamics. Concurrently, simultaneous polysomnography, continuous BP recordings, and measurement of the end-expiratory carbon dioxide were undertaken. CBFV showed compara…

Apnea prolongation via short-term inhibition.

Central apneas during sleep are frequently of longer duration than predicted and persist despite high levels of chemical stimuli. We provide evidence that suggests that this apnea prolongation represents a central inertia in the control system mediated by reciprocal inhibition of inspiration.

Obstructive sleep apnea and related disorders.

OSAS, a common cause of disrupted sleep and EDS, result from repetitive closure of the upper airway during sleep. It probably represents the most severe syndrome related to obstruction of the upper airway; less severe forms include UARS, a syndrome characterized by the need for increased effort to breath but no prominent apneas or hypopneas, and primary snoring. Initial clues to the presence of OSAS and related disorders are derived from the history and include loud snoring, EDS or insomnia, and witnessed apneas. Some patients, especially women, may complain mostly of tiredness or fatigue, and…

[Sleep apnea syndrome and morbid snoring].

Regular, continuous snoring usually does not have any sleep disturbing effects; however, discontinuous snoring may be associated with increased upper airway resistance and leads to recurrent arousals, sleep fragmentation and excessive daytime sleepiness. Loud snoring, recurrent obstructive apneas and oxygen desaturation during sleep with excessive daytime sleepiness are the hallmarks of the obstructive sleep apnea syndrome (OSAS). A tentative diagnosis of disturbed breathing during sleep can be made from a careful history and confirmed by a sleep study. Continuous positive airway pressure (CPA…

[Upper airways and sleep apnea syndrome].

The imaging techniques of the upper airway (UA) now permit a description of the characteristics of pharyngeal collapse during the course of obstructive apnoea. The start is oropharyngeal with active movements anterior and posterior of the soft palate and a falling back of the tongue. The extension occurs almost systematically towards the hypopharynx. A displacement of the hyoid bone and of the cervical spine is noted synchronously with thoracoabdominal movements. These imaging techniques of UA show the occurrence of passive pharyngeal collapse during certain types of central apnoea. In snorers

Prosthetic therapy for sleep apnea syndrome.

Twenty patients with sleep apnea syndrome were examined polysomnographically with and without an Esmarch sleep apnea appliance. The relationship between the therapeutic effect of the device and dentofacial morphologic characteristics of the patients were analyzed with the use of diagnostic casts and cephalometric radiographs. The apnea index or number of apneic episodes per hour decreased significantly (p < 0.0001) after insertion of the appliance. The diagnostic casts and cephalometric analyses revealed that the device was most effective for patients who exhibit a tendency toward micrognathia…

[Sleep apnea syndrome in intensive care].

The aim of this work was to study the existence of special characteristics in the sleep apnoea syndrome (SAS) discovered following a stay on the Intensive Care Unit. This retrospective study of 25 casenotes of SAS patients who were resuscitated has enabled a comparison with 182 SAS patients who have never had acute respiratory failure. The intensive care consisted of controlled ventilation, following intubation, in a clinical context of acute respiratory failure with major problems of conscious level. The diagnosis of SAS was made using conventional or computerised polysomnography, or a comput

Pharyngeal fat in obstructive sleep apnea.

Although most patients with obstructive sleep apnea (OSA) are obese, it is not known how obesity contributes to airway collapse during sleep. The purpose of this study was to determine whether the volume of adipose tissue adjacent to the pharyngeal airway in humans is related to the degree of OSA. We studied 30 subjects, nine without OSA and 21 with OSA; two subjects were studied before and after weight loss. Adipose tissue was detected with magnetic resonance imaging using T1-weighted spin echo sequences. The volume of adipose tissue adjacent to the upper airway was determined by measuring th…

[Diurnal sleep apnea in myotonic dystrophy].

We investigated diurnal sleep apnea in myotonic dystrophy with respiratory inductive plethysmography. Five of eight patients met criteria for sleep apnea syndrome and had central apnea mainly. In a case showing periodic breathing with apnea like Cheyne-Stokes type breathing, the duration of apnea and breath was even and the tidal volume went waxing and waning regularly. In the other four cases, central apneas were observed in sequence, but the duration of apnea and the tidal volume changed variously. Large breaths between apneas elevated arterial oxygen saturation rather than stable breaths wi…
