Congenital aponeurotic ptosis is the result of a failure of the distal end of the aponeurosis to attach in the usual position. There is a ptosis of varying degree disproportionate to the amount of levator function. Unlike dystrophic congenital ptosis, the amount of levator function is good, usually exceeding 9 to 10 mm. Corrective surgery was performed in 12 cases of congenital aponeurotic ptosis, including identical twins with unilateral symmetrical ptosis, supporting the concept of a developmental mesodermal anomaly.

In a neonatal postmortem histological study of scalp stabs made with a fetal blood sampling blade the aponeurosis was punctured in up to 38% of cases even when it was more than 2 mm deep. This and other indirect and direct evidence discussed suggest that five of the seven reported cases of major bleeding after fetal blood sampling in labour were due to blood clotting defects and that these should be considered urgently in the management of future cases.

BACKGROUND: The palmar aponeurosis is extremely adherent to the skin above it. Many of the pre-tendinous coarse fibers enter the dermis at an angle, not just in the palmar creases but also throughout the palm. It's difficult to distinguish whether Dupuytren's illness starts in the skin's dermis or the palmar aponeurosis since the skin adheres so closely to the palmar fascia. We present a novel technique for the surgical management of sizeable medial upper eyelid colobomas that entails advancement of an ipsilateral upper lid flap sustained by a levator aponeurosis-Müller-conjunctiva pedicle and combined with a lateral semicircular flap from the affected lid. The technique was used in 4 consecutive pediatric patients of mean age 21.2 months and mean coloboma size 12.3 mm and resulted in sufficient cornea protection, rapid rehabilitation, and satisfying cosmesis.

The work substantiates the operative access in aponeurectomy for the Dupuytren's contracture realized by a wave-shaped incision in the zone of pathologically changed aponeurosis depending on the individual disposition of nodulous indurations and on a certain height of skin grafts in relation to the base. The operation technique is also described with special reference to possible errors resulting in serious sequelae. The complications observed are also described. Their number is less than that in the data of other papers which shows the positive result of the work as a whole.

The authors present a case of unilateral eyelid congenital tarsal kink associated with corneal ulcer. The tarsal kink was corrected by a horizontal tarsal incision, performed half the thickness of the tarsus along its entire horizontal extent. Terminal levator aponeurosis fibers were reattached to the tarsal incision line to create sufficient everting force for correction of the entropion. The tarsal kink disappeared and eyelid positions remained stable during a follow-up period of 2 years.

Different modifications of flaps based on the dorsal metacarpal arterial system have been reported as reliable tools for reconstruction of dorsal and palmar phalangeal soft tissue defects. For simultaneous reconstruction of the extensor aponeurosis and joint in complex injuries of the dorsal index, we used a distally based pedicled fasciotendocutaneus second dorsal metacarpal flap, including the complete vascularized extensor indicis tendon in 2 cases. Clinical results of this procedure have not been reported so far.
