Atypical Presentation of Infective Endocarditis: Reactive Arthritis as the First Clinical Clue.

The clinical manifestations of infective endocarditis (IE) may be diverse and atypical, frequently mimicking rheumatologic or autoimmune disorders. The diagnosis of Staphylococcus aureus bacteremia may be delayed if early symptoms are nonspecific, despite the significant risk of underlying IE. We report a 29-year-old previously healthy male who presented with a sudden onset of fever, polyarthralgia, bilateral knee effusions, and a widespread erythematous petechial rash. Leukocytosis, thrombocytopenia, transaminitis, and significantly increased inflammatory markers were found during his initial

Reactive Arthritis Due to Leptospirosis in a 11-Year-Old Adolescent Boy.

Reactive arthritis is defined as a sterile inflammation of the joint space, following a remote infection, which can be bacterial or viral in origin. Although leptospirosis is not a frequent cause, it has been reported as a potential trigger. We herein report an 11-year-old boy who presented with fever, jaundice, and acute onset of right hip pain with restricted movement. Laboratory investigations were done to evaluate for infectious causes. IgM antibodies for Leptospira were equivocal, suggesting the possibility of an acute infection. This case highlights that reactive arthritis can develop ea

Scrub typhus associated with reactive arthritis: A case report and literature review.

Scrub typhus (tsutsugamushi disease) is an acute febrile illness caused by Orientia tsutsugamushi, often found in Asia and Oceania. The presence of an eschar, characterized by a crust, is a key diagnostic finding. Many symptoms of this disease are already known, however reactive arthritis following scrub typhus is very rare. Here, we present a case of 79-year-old man who was referred to our hospital because of continuous fever and left shoulder pain. We found 4-fold rise in Orientia tsutugamushi-specific IgG titer using paired serum samples and Orientia sp. genes by real-time PCR from a crust

Reactive arthritis before and after the onset of the COVID-19 pandemic.

Most accepted definitions of reactive arthritis (ReA) consider it a type of spondyloarthritis (SpA) precipitated by a gut or urogenital infection. A wider definition considers any arthritis that occurs after a mucosal surface infection as ReA. There is limited consensus regarding a working definition, status of HLA-B27, or even classification criteria for ReA. This may also contribute to a lack of systemic studies or clinical trials for ReA, thereby reducing further treatment recommendations to expert opinions only. The emergence of post-COVID-19 ReA has brought the focus back on this enigmati

Update on Post-Streptococcal Reactive Arthritis: Narrative Review of a Forgotten Disease.

PURPOSE OF THE REVIEW: This topical review attempts to build the concepts of PSRA as an independent entity and discuss prevalent diagnostic criteria. It utilizes a search strategy to collate all clinical features of PSRA reported from across the world and also discusses laboratory and treatment options in brief. RECENT FINDINGS: There are several immune-mediated diseases described after acute streptococcal infections. Post-streptococcal reactive arthritis (PSRA) is a sterile, self-limiting arthritis that occur as an immune sequelae to streptococcal infection. Though PSRA resembles the arthrit

Disseminated tuberculosis associated with reactive arthritis of Poncet in an immunocompetent patient.

Cutaneous tuberculosis is a rare extrapulmonary manifestation of tuberculosis which, like disseminated tuberculosis, commonly occurs in immunocompromised patients. Poncet reactive arthritis is a seronegative arthritis affecting patients with extrapulmonary tuberculosis, which is uncommon even in endemic countries. We report a previously healthy 23-year-old male patient with watery diarrhea associated with erythematous ulcers on the lower limbs and oligoarthritis of the hands. Histopathological examination of the skin showed epithelioid granulomatous process with palisade granulomas and central

Reactive oxygen species in psoriasis and psoriasis arthritis: relevance to human disease.

Psoriasis (Ps) is a chronic, immune-mediated, skin inflammatory disease affecting up to 3% of the population worldwide. Different environmental triggers initiate this complex multifactorial syndrome. Many individuals affected by Ps (6-26%) develop inflammatory disease in other organs, often in the joints as in psoriasis arthritis (PsA). Animal models that reflect the typical Ps syndrome, including both skin and joint pathology as in Ps and PsA, are valuable tools for dissecting disease pathways leading to clinical manifestations. In this context, we developed a new acute Ps and PsA-like diseas

Prolonged remission of chronic reactive arthritis treated with three infusions of infliximab.

Reactive arthritis is generally self-remitting but can be chronic. Predictors of chronicity include HLA B27 positivity and triggering infections with Yersinia, Salmonella, Shigella, or Chlamydia. Nonsteroidal antiinflammatory drugs and local steroids are the mainstays, with some evidence for disease modifying antirheumatic drugs. Limited evidence suggests effectiveness of tumor necrosis factor antagonists. We report the case of reactive arthritis following a Chlamydia urinary tract infection progressing to a chronic course, treated with a short course of high dose tumor necrosis factor antagon

A case of reactive arthritis in a Ranger Indoctrination Program (RIP) student.

Musculoskeletal complaints comprise the majority of cases encountered by military physicians when evaluating young active duty Soldier-athletes. This is a case of reactive arthritis in a 19-year-old active duty Soldier-athlete whose failure to improve with conservative therapy initiated further investigation. When evaluating what appear to be routine overuse injuries, it is important to actively include other potential causes of musculoskeletal complaints in the differential diagnosis. Further investigation of disease in patients whose symptoms and complaints do not improve with routine conser

C-reactive protein: the underlying cause of microvascular dysfunction in rheumatoid arthritis.

OBJECTIVE: RA is a chronic autoimmune inflammatory condition associated with increased cardiovascular morbidity and mortality. Endothelial dysfunction, a marker of early atherosclerotic disease, occurs in some inflammatory diseases but this relationship has not been previously explored within the microvasculature of patients with RA. We therefore assessed forearm microvascular endothelial function in patients with RA and determined its relationship to RA disease activity and inflammation. METHODS: A total of 128 RA patients with no previous history of cardiovascular disease were evaluated. En

[Cytokines and smoking in the cases of reactive arthritis during urogenital chlamydiosis].

The aim of the work was the determination of blood serum cytokines level and identification of possible association of the latter with smoking in the case of reactive arthritis (ReA) developed on the background of urogenital chlamydiosis. 74 patients with ReA were tested. 19 patients had acute ReA (disease duration not exceeding 6 months from manifestation of chlamydiosis) and 55 - chronic ReA (disease duration exceeding 6 months from manifestation of chlamydiosis). 56 of the tested patients were systematic smokers. Control group was presented by patients with urogenital chlamydiosis, but with

Management of sexually acquired reactive arthritis in 19 North Thames GUM clinics.

Our objective was to describe how genitourinary medicine (GUM) clinics in the North Thames region manage sexually acquired reactive arthritis (SARA), and to compare management with national guidelines. A self-completed questionnaire survey and retrospective case note review was conducted between September and October 2001. Clinicians in 33 clinics were asked to describe their clinic's policy on the management of SARA, and to review the last five cases seen or the last cases seen in the preceding two years, if less than five. Nineteen (58%) clinics took part. There were inter-clinic variations

Increased type I collagen degradation correlates with disease activity in reactive arthritis.

OBJECTIVE: The assay for the cross-linked carboxyterminal telopeptide of type I collagen (ICTP) has been shown to reflect increased type I collagen degradation in patients with rheumatoid arthritis. To look for increased collagen degradation in other inflammatory rheumatic diseases, we studied plasma ICTP in patients with reactive arthritis (ReA). METHODS: ICTP was determined by radioimmunoassay from 69 ReA patients. ICTP data on 56 patients aged > or = 20 years were compared with normal ICTP values available for that age group. RESULTS: The median (range) plasma ICTP concentration of the pa

Reactive arthritis following an outbreak of Salmonella typhimurium phage type 193 infection.

OBJECTIVES: To determine the occurrence and the clinical picture of reactive arthritis (ReA) following an outbreak of Salmonella typhimurium. METHODS: An outbreak of S typhimurium phage type DT 193 occurred in several municipalities in Finland in 1999. A questionnaire which had a specific emphasis on musculoskeletal symptoms was mailed to all 78 subjects with a positive stool culture. Based on the answers, all subjects with recent joint complaints were clinically examined or interviewed by telephone. RESULTS: Sixty three of 78 subjects (81%) returned the questionnaire. Of these 63 subjects,

Reactive arthritis associated with group C and group G beta-hemolytic streptococci.

OBJECTIVE: Group A beta-hemolytic streptococci (GAS) are known to be capable of evoking sterile arthritis. Reactive arthritis (ReA) has been reported sporadically following primary infection with group C and group G beta-hemolytic streptococci (GCS, GGS). We prospectively studied 4 cases of ReA secondary to throat infection with GCS and GGS. METHODS: Four patients with arthritis secondary to throat infection were seen. Three patients were Dutch, one was Indonesian; female/male ratio was 1/3; mean age was 30 years (range 18-46). Diagnostic evaluation included culture of throat swab and serolog

Granzyme A-immunoreactive cells in synovial fluid in reactive and rheumatoid arthritis.

Perforin and granzyme A co-localize in the cytotoxic granules of killer cells like cytotoxic T lymphocytes (CTL). Perforin is the cytolytic pore-forming protein, whereas the function of the homodimeric serine protease granzyme A and other members of the granzyme family is still unclear. Granzyme A-immunoreactive cells formed 8 +/- 2% of the resting peripheral blood lymphocytes of healthy individuals. In contrast, granzyme A-positive cells formed 15% of peripheral blood mononuclear cells in patients with reactive or rheumatoid arthritis. However, 29 +/- 4% (p < 0.05 compared to normal periphera
