Cerebral function monitoring in neonates with perinatal asphyxia--preliminary results.

INTRODUCTION: This study was based on foreign studies which confirmed the importance of amplitude-integrated electroencefalographic monitoring (aEEG) in the early prediction of the future neurological development of newborn infants with hypoxic syndrome. Our aim was to confirm the correlation between the type of aEEG trace and the level of brain damage in newborn infants in the early hours after the hypoxic event and to introduce this method into routine practice. MATERIAL AND METHODS: With 56 newborn infants having suffered a perinatal hypoxic event (the average umbilical arterial pH was 6.9

Magnesium therapy in birth asphyxia.

OBJECTIVE: Glutamate plays a critical role in the hypoxic ischaemic neuronal death. Two mechanisms of glutamate- induced neuronal death have been identified. One is rapid cell death that occurs in minutes and the second is delayed cell death that occurs over hours and is initiated principally by the activation of the N-methyl D-Aspactate (NMDA) receptor. Magnesium (Mg) is an NMDA receptor blocker. Systemic administration of Mg after a simulated hypoxic ischaemic insult has been shown to limit neuronal injury in several animal models. However, before embarking on to the use of Mg for neuronal p

Fluid restriction for term infants with hypoxic-ischaemic encephalopathy following perinatal asphyxia.

BACKGROUND: Current recommendations to control the consequences of hypoxic-ischaemic encephalopathy following perinatal asphyxia include the careful management of fluids, with avoidance of fluid overload and thus avoidance of cerebral oedema. Recommendations for fluid restriction in a neonate are based on the experience of restricting fluid intake in adults or older children. The extrapolation from studies in adults, older children and animals to the human neonate is fraught with hazard due to the different physiology and mechanisms of injury. OBJECTIVES: The objective of this review was to d

Cerebral hypothermia for prevention of brain injury following perinatal asphyxia.

The possibility that hypothermia has a therapeutic role during or after resuscitation from severe perinatal asphyxia has been a longstanding focus of research. Early studies using short periods of cooling had limited and contradictory results. We now know that resuscitation can be followed by a "latent" phase, characterized by transient recovery of cerebral energy metabolism, before secondary deterioration occurs with seizures, cytotoxic edema, and cerebral energy failure 6 to 15 hours after birth. Recent experimental studies have shown that moderate cerebral hypothermia initiated as soon as p

Serum macro creatine kinase type 2 in asphyxiated newborn infants.

The presence of macro creatine kinase type 2 (MCK2) activity was noted in the serum of seven out of 32 newborn infants with perinatal asphyxia or birth trauma. MCK2 isoenzyme, when present, represented 15-35% of the total creatine kinase (CK) activity. The clinical and biochemical features of the seven MCK2-positive and 25 MCK2-negative newborns were compared. The infants with MCK2 activity were all males and clinically appeared to be more severely injured, requiring longer hospitalization. Total CK activity was similar in the two groups and CK-MB and CK-BB isoenzyme fractions were present in

Prediction of the prognosis in neonatal asphyxia by photo-evoked eyelid microvibration.

Neonatal asphyxia causes hypoxic-ischemic encephalopathy and may damage the central nervous system. We studied neonatal asphyxia with photo-evoked eyelid microvibration (PEMV) which is a blink reflex elicited by photic stimuli. The latency of PEMV changed according to the severity of encephalopathy and the degree of unconsciousness. All of the neonate who had showed disappearance of PEMV died. When the prolonged latency persisted, the infant had a strong probability of sustaining neurological sequelae. PEMV provides useful information for predicting the outcome of neonatal asphyxia.

[Perinatal factors predicting hypoxic-ischemic encephalopathy in asphyxic newborn infants].

Between 1 May 1984 and 30 April 1985, 1,354 term or posterm neonates being white and having asphyxia in controlled delivery were studied to predict development of neurological signs and its seriousness. In each case correlations were estimated between the existence of neurological signs and its grade and 100 gestational, obstetric and neonatal factors of potential predictor. The discriminatory analysis indicated that the more significants predictors were: Apgar at five minutes, prolonged labor, twins, resuscitation, DIP I, cephalhematoma, caput succedaneum, umbilical artery pH, urgent cesarean

Thiopentone induced coma after severe birth asphyxia.

The aim of this study was to determine the feasibility of inducing a prolonged coma in severely asphyxiated newborn babies by the infusion of high dose thiopentone. In six severely asphyxiated babies the electroencephalograph (EEG) and blood pressure were monitored continuously. Thiopentone was infused at a rate sufficient to suppress completely the EEG providing the mean blood pressure remained above 35 mm Hg; it was continued until there was no evidence of cerebral oedema for 24 hours. In two the infusion was stopped prematurely because of hypotension that was unresponsive to treatment. In t

Periventricular low attenuations: a normal finding in computerized tomographic scans of neonates?

Twenty-five of the initial computerized tomographic (CT) scans of 63 neonates had "definite" abnormalities. These abnormalities included dilated ventricles, advanced hypodensities extending beyond the periventricular areas, and varieties of hemorrhage. Thirty-six, with otherwise normal intracranial structures, were found to have localized periventricular frontal or frontooccipital areas of low attenuation. These areas were confined to the white matter of the periventricular area and have been identified by others as indicating "periventricular leukomalacia." In follow-up clinical studies on 26

Intimate partner violence during pregnancy and its association with birth asphyxia in hospitals of Tigray region, Ethiopia.

BACKGROUND: Birth asphyxia is the main cause of neonatal mortality and morbidity worldwide. Some studies indicate intimate partner violence during pregnancy is a risk factor for birth asphyxia. In Ethiopia, intimate partner violence during pregnancy is reported to be high. Despite this high prevalence, there is a lack of data about the association of birth asphyxia and intimate partner violence. The aim of this study was to assess the prevalence of intimate partner violence during pregnancy and its associated factors with birth asphyxia in health facilities in the Tigray region of northern Eth
