Carbachol, norepinephrine, and hypocapnia stimulate phosphatidylinositol turnover in rat tracheal slices.

BACKGROUND: The intracellular mechanisms involved in the alpha-adrenoceptor- or hyperventilation-induced bronchoconstriction remain unknown. Because there is a direct relationship between phosphatidylinositol (PI) metabolism and airway smooth muscle contraction induced by muscarinic agonists, the authors examined the effects of carbachol (CCh), norepinephrine (NE), and hypocapnia on PI turnover in the airway smooth muscle. METHODS: Rat tracheal slices were incubated in Krebs-Henseleit solution containing LiCl and [3H]myo-inositol in the presence of NE, CCh, or neither. The PCO2 in the solutio

Exercise induced bronchospasm in physically fit female students of Kerman University and their pulmonary function tests.

INTRODUCTION: High prevalence of respiratory symptoms and bronchial hyper-responsiveness has been reported in professionals athletes, particularly in relation to climate and environment. However, the airway response to exercise in active population has been poorly investigated especially in women. The aim of this study was to examine pulmonary function test changes in physically fit female students of Kerman University. METHODS: Sixty physically fit female students (19 ± 1.12 years old) were randomly selected out of 500 students. Each subject underwent the physical fitness test (Couper test)

Provocation by eucapnic voluntary hyperpnoea to identify exercise induced bronchoconstriction.

The International Olympic Committee Medical Commission (IOC-MC) requires notification for use of a beta(2) agonist at the Winter Olympic Games in Salt Lake City. This notification will be required seven days before the event and must be accompanied by objective evidence that justifies the need to use one. The IOC-MC has expressed the viewpoint that, at present, eucapnic voluntary hyperpnoea (EVH) is the optimal laboratory challenge to confirm that an athlete has exercise induced bronchoconstriction (EIB). The EVH test recommended was specifically designed to identify EIB. EVH has been performe

Effects of verapamil and sodium cromoglycate on bronchoconstriction induced by isocapnic hyperventilation.

Twenty-four patients who were known to develop bronchoconstriction in response to isocapnic hyperventilation (IH) took part in a double-blind, placebo-controlled trial to assess the effects of verapamil (5 mg) and sodium cromoglycate (SCG) 20 mg in the prevention of IH-induced bronchoconstriction. Both drug and placebo were inhaled in dry powder form and responses were assessed by serial measurements of forced expiratory volume in 1 sec (FEV1). Sodium cromoglycate was shown to offer significant protection whereas verapamil powder provoked bronchoconstriction in over 40% of patients and these p

Exercise-induced rhinitis in competitive swimmers.

BACKGROUND: Elite swimmers are at increased risk of asthma, which has been related to chronic effects of pool chlorine environment. However acute effects of swimming on rhinitis remain unknown. OBJECTIVE: We aimed to assess the nasal response to exercise in competitive swimmers compared with competitive runners. METHODS: Measurements of nasal symptoms, peak nasal inspiratory flow, lung function, dyspnea, and of airway inflammation were obtained before and after a training session of 19 international-level swimmers and 13 professional runners. Exercise-induced rhinitis was defined as a fall i

A doctor's ability to assess the severity of childhood asthma by simple clinical features.

AIMS: To ascertain whether the severity of childhood asthma can be reliably assessed by simple clinical features, 94 newly diagnosed, school-aged asthmatic children were investigated. METHODS: The study included parental interviews, physical examination, skin prick tests, lung function studies, including a brief visual interpretation of the flow-volume curve, and a 6-min exercise challenge test on a treadmill, which was used as a reference. RESULTS: Baseline lung function studies showed a concave-shaped flow-volume curve in 40 (43%) patients, reduced maximal mid-expiratory flow (MMEF) in 25

[Determination of bronchial reactivity by inhalation administration of cold air and pharmacologically active substances].

The indications for testing of nonspecific bronchial reactivity and the factors influencing the airway responsiveness are presented. In order to achieve a better quantification of airway responsiveness two new techniques are shortly reviewed: 1. Estimation of accumulative dose by using a combined method of bronchial provocation and simultaneous measurement of airway resistance and 2. Eucapnic cold airway hyperventilation, especially in suspected clinical cases of exercise-induced asthma.

Morning-to-evening variation in exercise-induced bronchospasm.

BACKGROUND: Exercise is one of the most common triggers of asthmatic symptoms. Many factors, including hyperventilation, determine the prevalence and severity of exercise-induced bronchospasm (EIB). However, the influence of time of day has not been adequately described. OBJECTIVE: We sought to compare morning and evening EIB and minute ventilation during exercise (VE). METHODS: Twenty-two patients with stable asthma and 12 control subjects underwent exercise challenge at 7 am and 6 pm. The time of the first challenge was randomly assigned; the second challenge was performed within 1 week of

Increased prevalence of exercise-induced airway symptoms - A five-year follow-up from adolescence to young adulthood.

BACKGROUND: Exercise-induced airway symptoms are common in adolescents. Little is known about the development of symptoms from adolescence to early adulthood. Therefore, we studied the prevalence, incidence, and remission of exercise-induced airway symptoms (including wheeze, cough, chest and throat tightness, hoarseness, and stridor) in adolescents, over a five-year period. METHODS: In 2011, all adolescents aged 12-13 years in Uppsala (n = 3 838) were invited to answer a questionnaire on exercise-induced airway symptoms. All responding adolescents (n = 2 309) were invited to answer the same

Eucapnic voluntary hyperventilation in diagnosing exercise-induced laryngeal obstructions.

Exercise-induced laryngeal obstructions (EILOs) cause exercise-related respiratory symptoms (ERRS) and are important differential diagnoses to exercise-induced asthma. The diagnostic method for EILOs includes provocation to induce the obstruction followed by a verification of the obstruction and the degree thereof. The objective of the present study was to examine if a eucapnic voluntary hyperventilation (EVH) test could induce laryngeal obstructions laryngoscopically identical in subtypes and development as seen during an exercise test. EVH and exercise testing with continuous laryngoscopy we

Exercise test using dry air in random adolescents: Temporal profile and predictors of bronchoconstriction.

BACKGROUND AND OBJECTIVE: Guidelines recommend exercise tests using dry air to diagnose exercise-induced bronchoconstriction (EIB). Lung function changes subsequent to these tests have not been investigated in a general adolescent population, and it remains unknown whether signs of airway inflammation, measured using exhaled nitric oxide (FeNO), can predict a positive response. The aim of this study was to investigate the temporal aspect of decline in forced expiratory volume in 1 s (FEV1 ) after an exercise test using dry air, and to investigate predictors of EIB. METHODS: From a cross-secti

Analysis of refractory period after exercise and eucapnic voluntary hyperventilation challenge.

We compared specific airway conductance (SGaw) and the FEV1 after repetitive exercise or repetitive eucapnic voluntary hyperventilation (EVH) challenges. Replicate challenges were matched in terms of inspired air conditions and minute ventilations (VE) in order to determine the degree of refractoriness after each type of challenge in patients with exercise-induced asthma. Ten patients exercised or hyperventilated dry, room temperature air at matched VE on two study days. When the patients FEV1 had returned to 90% of baseline or better, or at 3.75 h if FEV, had not returned to 90% of baseline,

A 56-Year-Old Man With Emphysema, Rash, and Arthralgia.

A 56-year-old man presented to the pulmonary clinic with dyspnea and hypoxemia on exertion. He was an avid biker and skier who had noticed a significant decrease in high-level physical activity over the past 3 years. He reported dyspnea, desaturations at altitudes higher than 9,000 feet, dry cough, tachycardia, and palpitations with exercise. Review of systems was also notable for gluten-intolerance, Raynaud's phenomenon, recurrent skin lesions and joint swelling, pain, and stiffness in the areas overlying the jaw, wrists, knees, and ankles (after capsaicin exposure). He denied fever, chills,

Self-reported symptoms and bronchial hyperresponsiveness in elite cross-country skiers.

INTRODUCTION: Respiratory symptoms in relationship to exercise, bronchial hyperresponsiveness (BHR), and exercise-induced asthma (EIA) are very common in elite winter athletes. Symptom-based screening for BHR would facilitate selection of athletes with possible EIA. OBJECTIVES: The aim of the present study was to evaluate the diagnostic accuracy of self-reported symptoms as predictors of BHR in an unselected population of adult elite cross-country skiers. METHODS: Forty-six Swedish adult skiers competing at national or international level were included. They had a mean (SD) training volume i

Current concepts in the diagnosis and management of exercise-induced bronchospasm.

Exercise-induced bronchospasm (EIB) is a common occurrence in individuals with asthma, though it can also affect individuals without asthma. It occurs frequently in athletes. Common symptoms include coughing, dyspnea, chest tightness, and wheezing; however, there can be a variety of more subtle symptoms. The differential diagnosis of EIB is broad and includes several pulmonary and cardiac disorders. During the initial evaluation, a complete history, physical examination, and spirometry should be performed. In most patients with EIB, the baseline spirometry is normal; therefore, bronchoprovocat

Teenage asthma after severe early childhood wheezing: an 11-year prospective follow-up.

The role of factors related to early wheezing and their associations with subsequent development of asthma are controversial. We reevaluated 81 children who had been prospectively followed up since hospitalization for wheezing at less than 2 years of age. The baseline data on characteristics of the children, family-related factors, and viral causes of wheezing were collected on entry into the study. At the median age of 12.3 years, current symptoms suggestive of asthma and allergy were recorded. As part of the clinical examination, an outdoor exercise challenge test and skin prick tests to com

The role of cough and hyperventilation in perpetuating airway inflammation in asthma.

Air flowing through a pipe exerts frictional stress on the walls of the pipe. Frictional stress of more than 40 N/m2 (velocity equivalent of air 113 m/s) is known to cause acute endothelial damage in blood vessels. The frictional stress in airways during coughing may be much greater, however, since the velocity of air may be as high as speed of sound in air. We suggest that high levels of frictional stress perpetuate airway inflammation in airways which are already inflamed and vulnerable to frictional stress-induced trauma in patients with asthma. Activities associated with rapid ventilation
