Atrial fibrillation (AF) is associated with a higher incidence of mortality, stroke, and coronary events than is sinus rhythm. AF with a rapid ventricular rate may cause a tachycardia-related cardiomyopathy. Immediate direct-current (DC) cardioversion should be performed in patients with AF and acute myocardial infarction, chest pain due to myocardial ischemia, hypotension, severe heart failure, or syncope. Intravenous beta blockers, diltiazem, or verapamil may be administered to slow immediately a very rapid ventricular rate in AF. An oral beta blocker, verapamil, or diltiazem should be used …

OBJECTIVE: To review the pharmacology and clinical evidence of the use of vernakalant in the management of atrial fibrillation (AF).
DATA SOURCES: Peer-reviewed articles published in the English language were identified from MEDLINE and Current Contents databases (both 1966-March 5, 2008) using the search terms RSD 1235 and vernakalant. Citations from available articles and recent meeting abstracts were reviewed for additional references. During the preparation of this article, vernakalant was being reviewed by the Food and Drug Administration (FDA). Therefore, information posted on the FDA W…

Atrial fibrillation (AF) is the most common sustained arrhythmia in clinical practice. It consists in the triggering of 300 to 600 atrial waves per minute. The ventricular rate depends of the AV node refractory period that without treatment it is very high. This rapid response reduces the ventricular filling period with two consequences, the increase of venous pressures, and the reduction of the cardiac output, determining heart failure and myocardial ischaemia. The lack of atrial contraction, induces thrombus formation with high risk for systemic embolism. Cardioversion of atrial fibrillation…

Atrial fibrillation risk has been strongly associated with increasing age and visceral obesity. These characteristics are strongly associated with diabetes, decreased heart rate variability, and chronic inflammation. Lone atrial fibrillation (LAF) on the other hand exhibits a predilection for the physically fit and the middle aged, especially males. Given these opposing features it is postulated that pathologic AF is due to cardiac fibrosis and other age related changes while LAF is due to physiologic neurohormonal changes related to autonomic tone, insulin sensitivity, and electrolyte imbalan…

Atrial fibrillation is the most relevant arrhythmia in daily clinical practice. The pathophysiology is determined by multiple independent reentrant wavelets in both atria. Repetitive triggers and an underlying substrate may favor the initiation and maintenance of atrial fibrillation. Mapping studies in patients with drug-refractory paroxysmal atrial fibrillation identified potentials from the ostia of pulmonary veins as a main source of triggers that initiate atrial fibrillation. In ongoing clinical trials, catheter ablation of pulmonary vein foci is used to eliminate atrial premature beats an…

The cardioversion of chronic atrial fibrillation to sinus rhythm carries a thromboembolic risk of 1.5-6%. These events occasionally occur at the time of cardioversion, but more often happen hours or days later. These strokes and other embolic events may occur even where atrial thrombus has been excluded before cardioversion and it has become apparent that, although atrial electrical activity may be restored by cardioversion, normal mechanical atrial function may take longer to recover. Numerous studies have addressed the role of anticoagulation following cardioversion in patients with atrial f…

Atrial fibrillation is an extremely rare cause of falls and syncope in the elderly. The routine use of ambulatory ECG monitoring to search for atrial fibrillation in elderly patients who fall is not recommended. Among elderly patients with atrial fibrillation who fall, short pauses of less than 3 seconds are nonspecific and are as common in patients who fall as they are in those who do not. Furthermore, most pauses are not associated with symptoms. Although the decision to implant a pacemaker for extremely long pauses is often straightforward, the decision to implant a pacemaker in patients wh…

Factors associated with an increased risk of thromboembolic events in patients with atrial fibrillation (AF) include increasing age, rheumatic heart disease, poor left ventricular function, previous myocardial infarction, hypertension and a past history of a thromboembolic event. Patients with AF should be considered for anticoagulation or antiplatelet therapy based on the patient's age, the presence of other risk factors for stroke and the risk of complications from anticoagulation. In general, patients with risk factors for stroke should receive warfarin anticoagulation, regardless of their …

Atrial fibrillation is a common arrhythmia. It is associated with increased morbidity and mortality, in part secondary to an increased rate of thromboembolism. Considering the arrhythmia's duration, a chronic and paroxysmal type can be recognized. Only the former will be discussed here. Treatment of atrial fibrillation should be aimed at restoring sinus rhythm. This may improve exercise tolerance and alleviate symptoms. In addition, it may obviate the need for antithrombotic therapy. Electrical cardioversion is the method of choice to restore sinus rhythm in patients with chronic atrial fibril…

L-tetrahydropalmatine (Rotundium) is an alkaloid of Corydalis turtschaninovii. Some animal experiments had demonstrated that Rotundium had a good antiarrhythmic effect in blocking the calcium channel and that it was a class IV antiarrhythmic agent, similar to that of Bepridil in literatures. This is a clinical report of Rotundium (oral given) on an atrial fibrillation (AF) patients. The therapeutical results were evaluated by 24 hours ambulatory electrocardiographic monitoring and ECG. The results showed that the effective rate of Rotundium on paroxysmal atrial fibrillation and sustained atria…

After reviewing these published studies, I think one can conclude that in the patient population with chronic and/or paroxysmal atrial fibrillation, warfarin is beneficial and the benefits outweigh the risks. However, it is important to point out that patients at high risk for embolic disease, for example, dilated cardiomyopathy patients, were rarely included in these trials. The five trials mentioned above do not address this patient population. Current opinion is that these individuals should receive anticoagulation despite the lack of objective evidence that anticoagulation is beneficial if…

PURPOSE: To evaluate the different aspects of chronic atrial fibrillation (CAF) in elderly patients.
METHODS: Forty nine patients with CAF (18 men) aged 66 to 87 years (76.1 years on average) were studied. Their evaluation consisted of clinical examination, blood tests, electrocardiogram, chest X-Ray and echocardiogram.
RESULTS: The frequency of CAF in the out-patient Geriatric Clinic was 4.8% and it rose with aging. Cardiocirculatory diseases were observed in 39 (79.6%) patients: 25 (51.0%) had systemic arterial hypertension, 11 (22.4%) had mitral valve disease and six (11.2%) had aortic va…

This review deals with the mechanisms by which digitalis exerts its "opium-like" action on the ventricular rate in patients with atrial fibrillation. To understand the effect of digitalis on ventricular rate and rhythm, it is essential to learn more about the basic electrophysiologic principles responsible for: atrial fibrillation as such, and the scaling function of the atrioventricular (AV) node-His system. It may be assumed that during atrial fibrillation, the atrial excitatory process results in randomly spaced impulses that reach the AV junction from random directions with random strength…

BACKGROUND: Atrial anti-tachycardia pacing (A-ATP) of the right atrium (RA) has been shown to decrease the burden of atrial fibrillation (AF) in patients with dual-chamber pacemakers. The aim of this study is to identify the novel predictors of effective A-ATP for terminating AF in patients with AF undergoing catheter ablation.
METHODS: This study included 41 consecutive patients undergoing a first ablation procedure for paroxysmal (PAF: n = 21) or persistent (PEF: n = 20) AF. We prospectively evaluated predictors of AF termination after A-ATP. The coefficient of variation (CoV = SD/mean × 10…

BACKGROUND: Achieving long-term successful outcomes with ablation of persistent atrial fibrillation (AF) remains a clinical and procedural challenge. We aimed to assess 2 ablation strategies for persistent AF: pulmonary vein antral isolation (PVAI) in sinus rhythm after direct current cardioversion versus PVAI and ablation targeting complex-fractionated atrial electrograms while in AF.
METHODS AND RESULTS: Between June 2009 and July 2013, patients with continuous persistent AF for ≥3 months were prospectively randomized to either direct current cardioversion before PVAI and posterior wall/sep…
