A Contemporary Perspective on Long-Term Complications of Bariatric Surgery.

Bariatric (metabolic) surgery results in durable weight loss and metabolic remission, but long-term safety is underappreciated. This narrative review synthesizes contemporary evidence on late complications across procedures, including nutritional and mineral deficiencies, sarcopenia, bone health disorders, dumping syndrome, nephrolithiasis, and mental health outcomes (depression, anxiety, substance use, and suicidality). Risks concentrate after malabsorptive operations, yet restrictive procedures are not exempt. We outline mechanisms (malabsorption, hormonal and bile-acid signaling, microbiota

Evaluation of pregnancy outcomes in patients with a history of bariatric surgery.

INTRODUCTION: Obesity and overweight are significant risk factors for perinatal morbidity and mortality, and an increasing number of women of reproductive age are being offered bariatric surgery. The present study investigated the outcomes of pregnancies and births after bariatric surgery (BS). MATERIALS AND METHODS: All patients who gave birth at our clinic between 2018 and 2023 were included in the study. Individuals who had undergone BS previously were identified using the hospital database, and their medical birth records were obtained for data on pregnancy, delivery, and perinatal result

Effect of Successful Bariatric Surgery on Maternal Cardiovascular Outcomes.

BACKGROUND: Obesity is a common comorbidity among women of childbearing age. With increasing obesity, more females are considering bariatric surgery (BS) as a form of weight loss. The effect of successful bariatric surgery (sBS) on maternal cardiovascular outcomes remains to be elucidated. METHODS: Patients were extracted from the National Inpatient Sample database. We mainly stratified patients into two groups-BS candidates and sBS. We matched patients by age, race, and cardiovascular risk factors using propensity-score matching. The primary outcome was a composite and defined as major adver

Nutritional Optic Neuropathy: Bariatric Surgery Gone Wrong.

A 52-year-old woman presented with a two-week history of progressive and painless bilateral central blurring of vision, associated with malaise and loss of appetite. Bariatric surgery was done 18 months prior, which was complicated by malnutrition. Ophthalmic examination showed reduced visual acuity and color vision (red-green color vision deficiency) bilaterally with no relative afferent pupillary defect. Examinations of the anterior segment and fundus were unremarkable. A Humphreys visual field test revealed bilateral central scotoma. Optical coherence tomography of the retinal nerve fiber l

The Hidden Cost of Bariatric Surgery: Wernicke's Encephalopathy and Polyneuropathy.

Bariatric surgery is a recognised intervention for severe obesity, a global epidemic associated with numerous comorbidities like diabetes, hypertension, and cardiovascular diseases. Procedures such as Roux-en-Y gastric bypass facilitate significant weight loss, improving metabolic health and quality of life. However, these surgeries carry hidden costs, particularly concerning nutritional deficiencies that are often overlooked in preoperative assessments. This case report presents a 27-year-old patient who, after undergoing gastric sleeve surgery, developed neurological symptoms due to thiamine

Cancer incidence, type, and survival after bariatric surgery.

BACKGROUND: Many types of cancer have been found to be associated with being overweight or obese. Literature has demonstrated a reduction in cancer risk in patients who have undergone bariatric surgery. OBJECTIVES: To compare the incidence and types of new cancer diagnoses, cumulative cancer incidence, cancer risk, and overall survival in patients with obesity who underwent bariatric surgery with that of those who did not. SETTING: Community-based academic medical center. METHODS: We retrospectively compared the rates and types of new incident cancers in a bariatric surgery cohort (Bariatri

Ambulatory bariatric surgery: a prospective single-center experience.

BACKGROUND: Ambulatory bariatric surgery has recently gained interest especially as a potential way to improve access for eligible patients with severe obesity. Building on our previously published research, this follow-up study delves deeper in the evolving landscape of ambulatory bariatric surgery over a 3-year period, focusing on predictors of success/failure. METHODS: In a prospective single-center follow-up study, we conducted a descriptive assessment of all eligible patients as per our established protocol, who underwent a planned same-day discharge (SDD) primary sleeve gastrectomy (SG)

Treatment of esophageal adenocarcinoma in patients with a history of bariatric surgery.

BACKGROUND: The relationship among obesity, bariatric surgery, and esophageal adenocarcinoma (EAC) is complex, given that some bariatric procedures are thought to be associated with increased incidence of reflux and Barrett's esophagus. Previous bariatric surgery may complicate the use of the stomach as a conduit for esophagectomy. In this study, we presented our experience with patients who developed EAC after bariatric surgery and described the challenges encountered and the techniques used. METHODS: We conducted a retrospective review of our institutional database to identify all patients

Impact of Bariatric Surgery on Complications After Prolapse Surgery.

IMPORTANCE: Obesity is a risk factor for pelvic organ prolapse (POP) and independently associated with perioperative complications following prolapse surgery. Although weight loss surgery (WLS) may lead to weight loss and mitigate obesity-related comorbidities, it is associated with chronic malabsorption, which may also lead to increased perioperative complications. There have been no studies evaluating the impact of prior WLS on POP surgery outcomes. OBJECTIVE: This study aimed to describe the effect of WLS on complications after POP surgery. STUDY DESIGN: We performed a retrospective cohor

The effect of bariatric surgery type on cardiac reverse remodelling.

INTRODUCTION: Bariatric surgery is effective in reversing adverse cardiac remodelling in obesity. However, it is unclear whether the three commonly performed operations; Roux-en-Y Gastric Bypass (RYGB), Laparoscopic Sleeve Gastrectomy (LSG) and Laparoscopic Adjustable Gastric Band (LAGB) are equal in their ability to reverse remodelling. METHODS: Fifty-eight patients underwent CMR to assess left ventricular mass (LVM), LV mass:volume ratio (LVMVR) and LV eccentricity index (LVei) before and after bariatric surgery (26 RYGB, 22 LSG and 10 LAGB), including 46 with short-term (median 251-273 day

Iatrogenic eventration of the hemidiaphragm in a post-bariatric surgery patient.

The aim of this report is to describe the management of an iatrogenic diaphragmatic eventration following surgery to relieve neurogenic symptoms of thoracic outlet syndrome in a patient with a prior history of sleeve gastrectomy. We discuss the case of a 46-year-old woman with a 6-month history of gastro-oesophageal reflux and dyspnoea. Imaging demonstrated a left hemidiaphragm eventration and hiatus hernia. The patient underwent laparoscopic plication of the left hemidiaphragm, repair of the hiatus hernia, and an omega loop gastric bypass, with satisfactory resolution of her symptoms. This de

Bioavailability of Orally Administered Drugs After Bariatric Surgery.

PURPOSE OF REVIEW: Oral drug absorption after bariatric surgery is likely to be altered, but the impact of different bariatric surgery procedures on individual drugs is not uniform. The aim of this article is to describe factors influencing the bioavailability of orally administered drugs after bariatric surgery and to provide readers with practical recommendations for drug dosing. We also discuss the medications that may be harmful after bariatric surgery. RECENT FINDINGS: The fundamental factors for enteral drug absorption are the production of gastric acid; the preserved length of the inte

[Nutritional Deficiencies in Bariatric Surgery: A Guide for General Practitioners].

Obesity is a pandemic that affects nearly a third of the world's population, and its medical treatment has not achieved satisfactory or sustained results over time. Its counterpart, bariatric surgery (BS), has seen significant growth in recent years due to its favorable outcomes in weight loss and the resolution of comorbidities associated with obesity. Changes in diet, associated with the anatomical and physiological alterations resulting from BS, can lead to complications such as protein-calorie malnutrition and micronutrient deficiencies. Therefore, non-specialist physicians must be informe

Duodenal Varices: A Rare Cause of Bleeding in a Bariatric Patient.

We report a rare but potentially fatal complication of duodenal variceal bleeding in a patient status postsleeve gastrectomy. A 52-year-old woman with a history of sleeve gastrectomy presented with melena. Upper gastrointestinal endoscopy revealed bleeding varices in the duodenum that was clipped. After being referred to our institution for further management, CT scans were re-reviewed and revealed large varices in the fourth/fifth segment of the duodenum and proximal jejunum. The patient was referred to surgery. This case highlighted the potential ectopic variceal bleeding after bariatric sur

Impact of Bariatric Surgery on Thyroid Morphology and Thyroid Function.

INTRODUCTION: Few studies have evaluated the impact of bariatric surgery (BS) on thyroid function and morphology, and how it correlates to inflammatory and metabolic markers. We aimed to evaluate all those parameters together. METHODS: A longitudinal study included 70 patients with severe obesity. The bariatric group (BG) enrolled 40 patients who underwent BS, and the control group (CG) enrolled 30 patients who did not undergo BS. Both were submitted (pre- and 2nd-year) to thyroid ultrasound and laboratory analyses to determine the levels of thyroid hormones, inflammatory, and metabolic marke
