The epidemiology of benzodiazepine misuse: A systematic review.

BACKGROUND: Benzodiazepine misuse is a growing public health problem, with increases in benzodiazepine-related overdose deaths and emergency … ified 1970 publications, and 351 articles were eligible for data extraction and inclusion. In 2017, benzodiazepines and other tranquilizers were the third most commonly misused illicit or prescription drug in the U.S. (approximately 2.2% of the population). Worldwide rates of misuse appear to be similar to those reported in the U.S. Factors associated with misuse …

Benzodiazepine Use: The Underbelly of the Opioid Epidemic.

In the United States, benzodiazepine medication use is the secondary epidemic to opioid drug use and carries serious consequences as well, even though its use is enabled by well-intended clinicians. Benzodiazepine drugs are intended for short-term use, not to exceed 2 to 4 weeks; yet, it is common for clients to be taking benzodiazepine medications for up to 10 years. In addition to dependence or addiction, adverse effects include depression, emotional blunting, ataxia, aggression, irritability, nervousness, and cognitive impairment. These medications also contribute to increased risk for fall

[Benzodiazepine Dependency in the Elderly: How to Deal with it].

Benzodiazepine/hypnotic abuse and dependency increase with advanced age. Especially in the elderly, severe adverse events accompany the regular intake of these substances. Sedation, cognitive and psychomotor effects, falls, accidents and injuries are very frequent. The withdrawal of the medication is often associated with major complications. Therefore, prevention is important. These substances should not be prescribed against minor sleep and anxiety disorders and their use should not be restricted to 3–4 weeks. Withdrawal symptoms can include insomnia, anxiety, memory and concentration impair

Employing mirtazapine to aid benzodiazepine withdrawal.

Insomnia and depression are frequently encountered in patients during withdrawal from substances. While there are no approved medications for treating them, off-label attempts to address these phenomena with mirtazapine have shown some promising results. This case describes the use of mirtazapine as an aid in benzodiazepine withdrawal and its potential benefits in alleviating insomnia and depression in a 32-year-old man. It was found to ameliorate sleep myoclonus that was thought to be associated with his withdrawal syndrome. It is hoped this report will generate interest and stimulate further

[Relation between benzodiazepine abuse and trauma].

People have abused drugs from the moment they found out about them. Abuse of substances increased during crises (war, displacement, catastrophes, different stress-induced circumstances, fear for existence, absence of prospects and hope). Benzodiazepines are most frequently abused psychopharmaceuticals due to their easy accessibility. The war and aggression on Bosnia and Herzegovina have made unforeseeable consequences, they have caused great migration of population, deaths, wounding, separation of families, unemployment and social poverty. All these factors have had influence on mental health …

Cognitive and sedative effects of benzodiazepine use.

This paper reviews the effects of benzodiazepines (BZs) on the performance of tasks measuring human cognitive abilities. The paper reviews the most common cognitive side effects of BZs: increased sedation, decreased attention, and anterograde amnesia. In particular, this paper focuses on recent findings regarding time course-related effects on BZ-induced deficits in explicit and implicit human memory performance. Specifically, we reviewed recent research indicating that both explicit memory and priming are impaired by BZs if the encoding task takes place near the time of the theoretical peak p…

Alcohol withdrawal: what is the benzodiazepine of choice?

OBJECTIVE: To review the literature concerning the use of benzodiazepines for treatment of alcohol withdrawal and to determine if the current literature assessment justifies the use of lorazepam as first-line therapy. DATA SOURCES: A thorough review of the literature was performed with an online database (BRS Colleague). Articles directed at the targeted issue were chosen and additional references were obtained from the bibliographies of these articles. STUDY SELECTION: Clinical trials and case reports concerning the use of chlordiazepoxide, diazepam, and lorazepam in alcohol withdrawal trea…

Alcohol and GABA-benzodiazepine receptor function.

gamma-Aminobutyric acid (GABA)A is a major inhibitory neurotransmitter in the mammalian CNS. GABAA ergic synapse is also an important site of action for a variety of centrally acting drugs, including benzodiazepines and barbiturates. Several lines of electrophysiological, behavioral, and biochemical studies implicate GABAA ergic synapse in the actions of alcohol. In electrophysiological studies, alcohol has been reported to enhance GABA-mediated responses in cortical neurons, spinal cord and substantia nigra, albeit, negative results have also been reported. In behavioral studies, GABAmimetics…

Value of flumazenil in benzodiazepine self-poisoning.

The efficacy of the benzodiazepine-antagonist flumazenil (Ro 15-1788) was evaluated in 26 patients with coma due to benzodiazepine self poisoning, alone or in combination with alcohol or other psychoactive drugs. 77% of the patients responded to the administration of a mean dose of 1.73 mg (range 0.2 to 8 mg) with immediate awakening or an improvement of at least 2 coma grades. In the patients without response (n = 3) or with minor improvement (n = 3) other psychoactive drugs turned out to be predominantly responsible for their comatose state. Adverse effects of flumazenil treatment such as al

Are the effects of benzodiazepines on discrimination and punishment dissociable?

Studies have shown that benzodiazepines (BZs) both disrupt discrimination and increase resistance to punishment. Using a delayed response task, we provide evidence that effects of BZs on discrimination cannot be fully explained by deficits in either short or long term memory, or by intolerance for delay of reward. A schedule with rewarded, nonrewarded (Time out: TO) and conflict components was used to investigate effects in rats of compounds active at the BZ receptor on successive discrimination and punished responding in parallel. The GABA transaminase inhibitor ethanolamine-O-sulphate exerte…

On the brain endocoid for benzodiazepine recognition sites.

Human and rat brain contain a neuropeptide with 105 amino acid residues which inhibits the binding of 3H-diazepam and other specific benzodiazepine recognition site ligands to crude brain synaptic membranes. DBI injected intracerebroventricularly in thirsty rats which are subjected to a conflict test (Corda et al., 1983), lowers the threshold for behavioral suppression by punishment. In this test DBI acts like an anxiogenic endocoid for the benzodiazepine recognition sites. The large abundance of lysine and arginine residues in the DBI molecule suggest that this polypeptide functions as a prec

[Behavioral disorders secondary to benzodiazepine therapy].

This report adds three cases to those previously published of paradoxical reactions to benzodiazepines. The clinical syndrome, more or less complete, is fairly stereotyped, with behavioural disorders, tendency to feelings of hostility and aggressiveness and total anterograde amnesia. Personal and environmental factors seem to play a prominent role in its development. The classical contra-indication of benzodiazepines following such reactions is discussed, and the hypothesis that mamillary-thalamic benzodiazepine receptors play a part in their genesis is envisaged.

Effects of combined alcohol and benzodiazepine: a review.

Both human and animal studies relating to the combined effects of alcohol and benzodiazepine (BZD) are reviewed. Although the combination of alcohol and BZD is sometimes associated with drug-induced deaths, drug overdoses and traffic accidents or fatalities, epidemiological information is lacking on the true extent of the combined abuse and on the patterns and prevalence of use of these two drugs. Since BZD are widely used for the short- and long-term treatment of alcoholics, these patients are deemed more at risk of developing BZD or alcohol/BZD dependence than the general population. There i

Interaction between methylxanthines and the benzodiazepine receptor.

3H-Flunitrazepam (500 pM) was used to estimate the inhibitory effects of caffeine, theophylline, theobromine and 3-methylxanthine on binding to rat brain homogenates. Caffeine and theophylline showed a low affinity competitive type of inhibition in vitro, but no inhibition in vivo. Low concentrations of caffeine failed to show clear inhibitory capabilities against low concentrations of 3H-flunitrazepam. The results suggest that only the toxic central nervous system effects of methylxanthines are mediated via the benzodiazepine receptor.

Reducing the risk: barbiturate substitution with a benzodiazepine.

A study was carried out in general practice in 85 habitual barbiturate takers, many of whom had been using this hypnotic for over 30 years, to assess the clinical practicability and acceptance of barbiturate substitution with flurazepam. Each barbiturate capsule or tablet being taken by patients was replaced by 15 mg flurazepam, either immediately or by gradual substitution. Patients were followed-up over a period of 6 months. The results showed that 5% of the patients were able to stop hypnotic therapy after initial substitution with flurazepam, a further 7% were maintained on occasional flur
